We are recruiting for a Patient Safety Reviewer to join our team at Oxford Health NHS Foundation Trust. You'll work with the Patient Safety Team, Directorate Governance Teams, Family Liaison Officers and Complaint Managers to lead a high quality process for reviewing and identifying learning from Patient Safety and/or complex complaints. This is a full time role (37.5 hours per week) based at our Littlemore Mental Health Centre with travel required across our Trust directorates so you will need to have the ability to travel freely between sites. You'll be a key member of the incident response function in patient safety reviews and/or complex complaints ensuring that an effective review takes place in order to identify learning and required changes in practice to prevent or reduce the likelihood of reoccurrence. This is a challenging but rewarding role within a busy and small team (8 people) where you'll work collaboratively with clinical staff and patient/family to identify areas of improvement and make a positive impact in our aim to deliver our services successfully to the highest standard. Patient Safety Reviewer duties This role covers the Buckinghamshire Mental Health Directorate, however we do cover all directorates in the Trust which include Community, Mental Health and Forensic/Specialised Services so you will have the opportunity to gain knowledge of many different services/teams, and different types of incidents (although due to nature of our service self harm/suspected suicide are the majority of reviews). Patient Safety Reviewer duties include: • Be an expert in Patient Safety and reviews, techniques and practice, including Systems Engineering Initiative for Patient Safety (SEIPS) and Human Factors, • Monitor and interpret national guidance and policy in order to ensure that Trust procedures and practice comply or exceed best practice, • Review patient safety incidents and/or complex complaints, acting as a role model and expert advisor for other staff involved in the reviews ensuring that all reviews are carried out to the highest possible standards, • Produce high quality reports that identify the system and process factors that led to the incident/complaint occurring, suggesting safety actions for change in practice, • Analyse themes and patterns from incidents and complaints to support learning and changes, presenting these in internal forums, • Work flexibly as part of wider Patient Safety, undertaking duties and activities relevant to level of post to improve the experience of patients and staff Please refer to the job description attached for a comprehensive list of duties Working for our organisation Oxford Health is a great place to work.
