A little about Reno-Sparks Indian Colony The Reno-Sparks Indian Colony represents descendants of three Great Basin Indigenous peoples: the Numu (Northern Paiute), Newe (Western Shoshone), and Washeshu (Washoe). These communities lived in harmony with the land for generations before European contact, following seasonal cycles and forming small, efficient social groups for survival. Each group’s name means “The People” in their own language, reflecting their deep cultural identity and connection to the Earth. They maintained strong traditions, respected neighboring tribes, and engaged in trade throughout the region. What it's like to be a Case Manager at Case Manager As a Full-Time Case Manager at the Reno-Sparks Indian Colony, you will play a vital role in the Behavioral Health Division, conducting comprehensive assessments to identify patient needs and develop tailored service plans. Your responsibilities will include collaborating with a dedicated team, monitoring client progress, and ensuring that essential services are delivered effectively. You will document progress in our Electronic Health Record system, facilitate interdisciplinary treatment meetings, and help clients access necessary community resources. Upholding the highest standards of client confidentiality and privacy, you will also provide crisis intervention and develop integrated treatment plans. Your work will extend to collaborating with various court systems and participating in community presentations on mental health. What we're looking for in a Case Manager To excel as a Full-Time Case Manager at the Reno-Sparks Indian Colony, a blend of technical and interpersonal skills is essential. Proficiency in Electronic Health Record systems is critical for documenting client progress and maintaining accurate medical records. Strong organizational abilities will help manage monthly reports, treatment plans, and community resource coordination effectively. Excellent communication and collaboration skills are crucial, as you'll work closely with the Behavioral Health Division Team, treatment providers, and clients, ensuring their needs are met with integrity. A foundational knowledge of counseling principles and cultural competencies related to diverse populations will enhance your effectiveness. Additionally, the ability to analyze complex situations, devise creative problem-solving strategies, and make informed decisions will set you apart. A valid Nevada driver’s license and current CPR certification will also be necessary, ensuring you are well-equipped to support clients in various settings and situations. Knowledge and skills required for the position are: • Completes a comprehensive case management assessment at time of admission, to identify patient needs, goals, abilities, resources and supports as the basis for service planning., • Collaborates with the Behavioral Health Division Team and evaluates, facilitates and monitors services and clients’ progress in relationship to established goals and objectives., • Document progress in Electronic Health Record and maintains assigned medical records as required by medical record procedures., • Provides monthly monitoring overview and assures that all services have been delivered and properly accounted for, • Coordinates and facilitates the interdisciplinary treatment planning meeting and completes the individual plan of service, • Submits monthly schedule, mileage, monthly reports and quarterly service report to manager as necessary., • Facilitates community resource availability based on client needs (day program, guardians, etc.) to assure maximum social integration, • Ensures client confidentiality and privacy (42 CFR Part 2 & Health Insurance Portability and Accountability Act (HIPAA)) are maintained with all patients, • Reviews and coordinates with treatment team and provider on decisions involving hospitalization, respite and/or transfers/level of care changes. Implements plan of action and maintains ongoing dialogue with behavioral health treatment team regarding status changes., • Facilitates client benefits (e.g. SSI, Medicaid, SSB, Medicare), • Responsible for the development of integrated treatment plan, life skills, etc., • Provides crisis intervention as needed, • Develops appropriate clinical linkages and referrals., • Complete and process any medical record requests for Behavioral Health Department client records., • Provide Positive Parenting Group/Class for clinic patients, • Complete and process any alcohol and drug residential treatment paperwork and referrals to IHS for approval; including residential treatment program applications, travel accommodations and IHS referral requests., • Facilitate community presentations related to mental health and substance abuse., • Collaborate with RSIC Tribal Court, attend meetings and ensure tribal court clients’ needs are being met., • Attend RSIC Child Protection Team (CPT) and Elder Protection Team (EPT) monthly meetings and assist with referrals for behavioral health services., • Collaborate with any tribal, county, city or state court systems to ensure client services and needs are being met., • Monitor, complete and input MSQSs (Medical Stability Quick Screen) for pain management clients., • Performs other duties as assigned by Manager., • Bachelor’s degree in human services field (in social work, counseling, psychology or in the specific area of training and licensing discipline) from an accredited college or university, • Be able to develop and maintain a good working relationship with clients and staff while maintaining confidentiality (42 CFR Part 2) under the Health Insurance Portability and Accountability Act (HIPAA)., • Proven knowledge of theories, principles, practices and techniques of counseling/case management, • Demonstrated ability to analyze problems, devise solutions and make sound decisions related to the behavioral health program, • Knowledge of diversity, ethnic, cultural values and attitudes for the service population., • Requires clinical management skills with an emphasis on communication, collaboration and interpersonal skills, • Twenty-one (21) or older at the time of hire and eligible for participation in the RSIC Vehicle Insurance Program: Must have a valid Nevada driver’s license., • Must pass a recent TB test Please Note: Hiring preference is provided to qualified members of the Reno-Sparks Indian Colony followed by members of other federally recognized tribes.
