Qualifications, • Educational Background:, • Doctor of Medicine (MD), Doctor of Osteopathy (DO), or foreign equivalent., • Residency Training:, • Completion of a three-year postgraduate medical training from a U.S. accredited residency program in Family Medicine or Internal Medicine., • Experience:, • Open to both experienced and newly trained providers., • Board Certification:, • Board Eligible/Board Certified in Family or Internal Medicine., • Licensing:, • Must possess a current, unrestricted New York State license., • DEA Registration:, • Must possess active Drug Enforcement Agency (DEA) registration or be eligible for registration., • Medicare/Medicaid Enrollment:, • Must possess active Medicare/Medicaid Enrollment or be eligible to enroll., • Life Support Certification:, • BLS or ACLS required., • Communication Skills:, • Strong communication skills, both oral and written., • Technology Proficiency:, • Comfortable with Electronic Medical Record (EMR) system and Microsoft Office.
