Job Description Clerical Territory Patient Admissions Coordinator Location: Sacramento Office Position: Clerical Territory Patient Admissions Coordinator Position Type: Full-Time Remote/Virtual Position: Remote or On Site Coverage Area: Northern California Find Your Passion and Purpose as a Clerical Territory Patient Admissions Coordinator Salary: $28|HR to $32|HR Schedule: Weekends Holiday Rotation required Previous Home Health experience strongly preferred Knowledge of HCHB a plus Responsibilities The Territory Patient Admission Coordinator establishes and maintains communication with the clients, family members and referral sources, in a given territory. This role is responsible for partnering with agency staff, referral sources, and sales to achieve optimal patient satisfaction & outcomes. The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of the referral based off the predetermined criteria set by the Agency and facilitates obtaining all information and documentation necessary to complete a quality referral. This position’s assigned territory, may encompass one or more states in AccentCare’s operation and may change with the needs of the business. Be the Best Territory Patient Admissions Coordinator Clerical You Can Be If you meet these qualifications, we want to meet you! • High School or GED, • Minimum of one year experience in home care, hospice, or personal care services, required, • Prior use of Electronic Medical Records (EMR), HCHB preferred, • Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred Responsiblities • Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral., • Documents all referral related Information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency’s software solution to facilitate the referral process by completing required workflow task within the Company’s established time frames., • Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal., • Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients., • Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information., • Completes a “Follow Up” call to the Client/Caregiver/Referral Source to collect or confirm information. Example: confirms out of pocket expense, clarify teachable caregiver, and collect missing demographic and physician information. Be the Best Registered Nurse / Behavioral Health You Can Be If you meet these qualifications, we want to meet you! • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation., • An RN with a Bachelor's Degree in psychiatric or mental health nursing must have one year of recent nursing experience, recommended to be within the last 3 years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, a psychiatric Partial Hospitalization Program (PHP), or another out-patient psychiatric service., • An RN with a Associate's Degree in psychiatric or mental health nursing must have two years of recent nursing experience, recommended to be within the last 3 years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, a psychiatric Partial Hospitalization Program (PHP), or another out-patient psychiatric service. Required Certifications and Licensures: • Licensed to practice as a registered nurse in the state of agency operation., • Must possess and maintain valid CPR certification while employed in a clinical role., • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
