Job Description Position Overview: The Deputy Chief Medical Officer (DCMO) provides executive-level clinical and operational leadership to ensure high-quality, cost-effective, and culturally competent care delivery for Philadelphia’s behavioral health Medicaid members. Serving as a key partner to the Chief Medical Officer, the DCMO has primary responsibility for comprehensive clinical oversight, including quality of care, safety, clinical effectiveness, and medical management. Job Description The Clinical Care Manager (CCM) Specialist- Children’s Complex Care (High-Fidelity Wraparound) is part of a specialized team, responsible for providing high intensity care management, that works collaboratively and directly with members and providers to ensure effective, holistic, member-centric care. As a member of CBH’s Complex Care team, training and certification in High-Fidelity Wraparound, an evidence-based care management intervention, will be provided. The CCM Specialist will also be responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, and treatment quality monitoring. As a member of the Children’s Complex Care team, you will have an opportunity to apply your understanding of behavioral health and your cultural competence to work collaboratively with CBH members, their caregivers, their supports, other professionals, and community partners to screen/assess, and develop individualized, family-anchored, comprehensive, and strength-based plans of care. The CCM Specialist must utilize clinical knowledge of best practices and evidence-based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, CCM Specialists consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. As a High-Fidelity Wraparound (HFW) Facilitator, the CCM Specialist assists members and their families in learning to accomplish goals they have identified by exploring the strengths and culture around each need they have. The CCM Specialist facilitates monthly interagency planning meetings to assist the member and their family in planning around and working towards meeting their needs. The CCM Specialist will transfer their Wraparound skills to the member and their family, by teaching them to think outside of the box and use natural and community supports. Essential Functions: • Work directly with members, natural supports and providers in the community, including in members’ homes., • Facilitate care coordination. Job Description The Clinical Care Management Specialist/Integrated Care Specialist provides clinical recommendations and collaborative support to team members and physical health care staff involved in the Integrated Care Plan (ICP) Program. The Clinical Care Management Specialist reviews clinical information using a holistic care framework, makes recommendations for appropriate levels of care, creates Integrated Care Plans and coordinates services for individuals referred to the ICP team. Job Description Position Summary: CTT is mandates to provide service to those individuals within the mental health system who present the greatest degree of severity of systematology as evidenced by their multiple physical, psychiatric and legal conditions, extensive use of services and lack of follow through with treatment. This population requires the highest level of medical necessity as defined by the state Adult Environmental Matrix. The Teams that provide multidisciplinary clinical review and assessment of individuals who may be young adults, heavy users of service, or actively involved in the criminal justice system and may have co-occurring behavioral health diagnoses are generic and specialized. The Case Management specialist provides case management supports based on the Community Support Program of Philadelphia with assisting individuals in developing skills required to enhance his/her comfort and abilities to function as a productive member of the community. The CM Specialist takes the lead in one of five areas of specialty: vocational, housing, D&A, forensic or Benefits/ Entitlements, and works in collaboration with the primary case manager to identify goals, develop, implement and monitor the service plan. The CM Specialist will ensure that the mission, goals and philosophy of the CTT are operationalized within the team. Job Description Position Summary: The Behavioral Health and Justice Division (BHJD) is committed to working towards reducing criminal justice involvement for Philadelphians with behavioral health challenges, including Serious Mental Illness (SMI). BHJD organizes its work according to the Sequential Intercept Model, identifying opportunities to intervene at various points of criminal justice involvement to deflect and divert individuals away from further penetration of the system and into the behavioral health system. By helping connect individuals to needed treatment and support services that promote stability in the community, BHJD believes that Philadelphians are less likely to be arrested and can achieve health, well-being, and self-determination. BHJD also aligns with Stepping Up and focuses its efforts on lowering the number of Philadelphians with SMI admitted to the Philadelphia Department of Prisons (PDP), reducing their length of stay, connecting them to treatment/supports upon release, and reducing recidivism. The Forensic Support Team (FST) of BHJD was established to meet the unique needs of Philadelphians who are either: 1. Adjudicated as not competent to stand trial on only misdemeanor charges and committed to a State hospital (e.g., Norristown State Hospital) or 2. Adjudicated as non-restorable. The FST works within PDP and Norristown State Hospital (NSH) in order to assist individuals while in jail, when transitioning from jail to NSH, and when reentering the community typically under the court’s jurisdiction. To support this forensic population, the FST helps individuals navigate the Philadelphia judicial and behavioral health systems while also seeking to address each individual’s unique legal, behavioral, medical, physical, cultural, and/or financial needs. The Clinical Assessor becomes an important part of a team comprised of: one (1) FST Supervisor (a licensed psychologist); one (1) FST Coordinator, three (3) Clinical Assessors, and one (1) Post-Doctoral Psychology Resident. The Clinical Assessor will complete clinical assessments and provide system navigation for justice-involved individuals with behavioral health challenges who fall into either of the aforementioned priority groups. In this capacity, the Clinical Assessor is responsible for determining the appropriate level of care (LOC) for the individual via a procedure that is consistent with a strengths-based, biopsychosocial recovery model. The procedure relies on State hospitals and jails to provide access to the individual as well as clear documentation and consultation about the individual so an informed decision can be made about the appropriate LOC and the individual’s risk for violence. The decision about the LOC is based on the individual’s: aspirations, goals for recovery, strengths, support system, risk for future violence, psychiatric diagnosis, medical diagnosis, psychiatric history, cognitive or physical limitations, results from any psychological measures or medical tests, current medications, incident reports, participation in recovery services, current treatment team input, and privileges earned. With this information and documentation, the Clinician Assessor collaborates with the Philadelphian and their treatment team in an effort to agree on an appropriate LOC as well as a re-entry plan and, when necessary, present the plan to the appropriate criminal court. Duties and Responsibilities: The Clinical Assessor helps individuals with SMI "navigate" the maze of treatment programs, the court system, and support services upon reentry into the community. The Clinical Assessor reduces barriers that keep individuals with SMI from accessing timely treatment by identifying behavioral health needs and directing individuals to appropriate resources. An effective Clinical Assessor is a: Relationship builder Building effective working relationships is the foundation of effective forensic navigation. To improve coordination of care for people with mental illness and to reduce barriers to reentry, navigators must work with different groups of people: individuals who are incarcerated, healthcare teams within the PDP, criminal justice partners, families, NSH staff, and resource providers within the community. It is important to build effective working relationships with all of these groups. Clinical care coordinator The Clinical Assessor conducts forensic evaluations and coordinate care for individuals who are either: 1. Adjudicated as not competent to stand trial on only misdemeanor charges and committed to a State hospital (e.g., NSH) or 2. Adjudicated as non-restorable. They coordinate with the court system, NSH, and treatment providers to determine the most appropriate and effective interventions and levels of care, and they provide the needed linkages. They plan reentry for individuals who will be returning to the community. Moreover, Clinical Assessors enhance the continuity of care as individuals transition between PDP and NSH. Creative problem solver The Clinical Assessor finds creative solutions to barriers. Some individuals with mental illness utilize commonly used services, but others have unique needs that require initiative and creative thinking to locate the appropriate resources. Organized information manager The Clinical Assessor manages and tracks a large amount of information. Assessments, referrals, legal statuses, and treatment histories are just a few things the Clinical Assessor needs to track and, by law, this information must be kept confidential and secure. It is important that the Clinical Assessor stays organized and uses a systematic approach to information management. Specific duties • Perform level of care and other-discharge related assessments for individuals adjudicated not competent to stand trial and/or non-restorable that are detained at the PDP or NSH, • Attend treatment team meetings for individuals detained at NSH to monitor progress and/or changes in clinical presentation, • Facilitate access to re-entry services and programs, • Identify individuals for diversion and share with appropriate justice partners, • Review legal status, commitment type, charges, competency opinions and findings, and clinical needs for all individuals on the waitlist to NSH on an ongoing basis, • Maintain accurate, confidential records documenting services provided and unmet needs using appropriate databases, • Remain current with resource and referral information, • Maintain list of community resources, • Build and maintain relationships with key individuals (e.g., PDP staff, Public Defender), and provide updates upon marked changes in an individual’s clinical or legal status to trigger appropriate interventions, • Coordinate with courts to ensure timely court hearings and legal proceedings, particularly when an individual’s clinical or legal status changes, • Provide behavioral health literacy and continuity of care training to both behavioral health providers and criminal justice personnel, • Provide updated progress reports for criminal court hearings and attend those hearings when necessary This position requires a master’s level degree in social work, psychology, or a related clinical focused field with at least two years of experience working with individuals with serious mental illness and/or in clinical-forensic assessment. Knowledge of and experience in working with the Philadelphia criminal justice system and/or the Philadelphia behavioral health system are preferred. The position requires demonstrated skills in communication, problem solving, decision making, and report writing. The individual possesses the ability to skillfully communicate verbally and in writing with a variety of multidisciplinary partners, including judges, attorneys, jail staff, provider agencies, psychiatrists, nurses, and social workers. Job Description OVERVIEW OF THE POSITION: Skilled, Independent Contractor will provide trauma-informed therapy to underserved populations community-based mental health outpatient clinic and in school-based outpatient satellite sites. This includes mental health assessment and treatment of a diverse caseload of children ages 4 to 17 experiencing a variety of behavioral health issues. FUNCTIONS: • Provide weekly client-centered, trauma-informed, individual, group, and family therapy as per the specifications outlined in the contract., • With client consent, make concerted effort to engage identified support system in treatment., • Provide initial and ongoing assessment of presenting issues, diagnostic impressions, and recommended course of treatment., • Create treatment plan and routinely monitor progress in collaboration with client., • Manage own tax and financial affairs., • Coordinate care with other involved professionals to ensure continuity of care., • Enter intake and therapy appointments into EMR at least 48 hours in advance., • Maintain accurate records of performance metrics and time spent including minimum billing expectations commensurate with scheduled hours. This includes entering progress notes within 3 days of service, including supportive documentation (e.g., treatment plans, safety plans, case management notes, briefs, triage notes, discharge summaries, no show or cancellation notes)., • Encourage consistent attendance by routinely reviewing attendance policy and providing weekly reminder calls. Discharge inactive cases in a timely manner to promote access., • Refer for community resources including expanded mental health programming as needed., • Provide crisis intervention including safety planning as needed. This includes follow up and coordination of care when individuals go to the CRC or acute level of care., • Complete all mandatory training to maintain state, CBH, and NIAC credentialing. Confirm and update client eligibility. Prepare and submit 837P electronic billing files and CMS1500 paper claim forms. Download and review response files to the 837 billing files. Post electronic and manual payment files as received. Perform all billing and cash posting tasks in a timely manner in order to ensure maximum revenue and facilitate cash flow. Provide the Accounts Receivable Manager and Chief Revenue Officer with the necessary information to resolve all billing and funding discrepancies. PHYSICAL DEMANDS: Minimal in nature QUALIFICATIONS: Associate degree preferred; Three to Five years’ experience in Health Care Billing functions, preferably in a Behavioral Health and/or non-profit environment, in an Electronic Health/Medical Records system; Ability to run reports necessary to correct and update billing and funding source data; Ability to work with numbers and function with minimal supervision. SPECIFIC DUTIES: • Complete all CBH Monthly and Weekly (CBE/PE) billing, including analysis to determine service type to bill and completion of roll-up services., • Rebill services from CBH to Medicare as required via HCFA 1500 forms and complete all relevant documentation., • Download, review and document all 277 files from CBH after each 837 file submission., • File TPL Discrepancies with CBH as needed based on the 277 Rejections for CBH., • Submit requests for Medicare Exceptions to CBH for all newly admitted or newly eligible Medicare clients who are dual-eligible and are receiving services from Russian-speaking therapists., • Download and post CBH 835 RA files including review, analysis, and correction of any incorrectly or unposted services., • Review, correct, and resubmit services rejected on the CBH 835 RA, or refer to A/R Manager for adjustments when indicated., • Review, correct, and resubmit services on the monthly CBH Open Services Reports, or refer to A/R Manager for adjustments when indicated., • Submit TPL Discrepancies to CBH for resolution., • Submit appeals for CBH services over 90 days as needed, and upload billing file with services within the required time frame when/if approved., • Review the CBH Unbilled Claims report on a monthly basis., • Generate and submit to DBH (Philadelphia County) all electronic requests for MHX Clearance for new Clients weekly., • Download and review the response file to the MHX Clearance submission., • Identify Clients that are ineligible for MHX Clearance for missing demographic data and flag in Avatar., • Follow up with Clinical Staff to obtain missing demographic data elements that prevent PATH from obtaining an MHX Clearance for the client, including missing Social Security Numbers., • Download the MHX Feedback Report on a weekly basis., • Identify and flag in Avatar Clients that have been newly approved for MHX Clearance for PATH on the MHX Feedback report., • Using source reports, create monthly Intake service charges to be billed to DBH., • Using source reports, create monthly Anonymous charges, and adjust clients’ services’ that will be billed as Anonymous., • Generate and submit billing files to DBH for Outpatient, CRR, BCM, and Administrative Case Management services by the end of the first full week of the month., • Download and review the ta1, 999, and 277 response files for the DBH billing two (2) business days of the submission of the 837 files., • Transfer 277 rejections into the DBH 277 workbook including the reason code(s) for rejection., • Analyze, correct, and resubmit DBH claims rejected on 277 files, or refer to the A/R Manager for adjustments., • Download the 835 RA for DBH within one (1) business day of its availability., • Document and calculate amounts approved by Program and/or age for DBH and enter in the DBH-OAS Approval Summary Workbook for the current Fiscal Year., • Analyze, correct, and resubmit DBH claims rejected on the 835 RA, or refer to A/R Manager for adjustments., • Review, confirm, and approve DHS services in the DHS Placement Verification System on the first working day of the month by 4:00 PM., • Follow-up and coordinate any corrections to DHS services with either DHS, RTF Clinical Staff, or both until the issue is resolved., • Review and resolve discrepancies on the Credit Balance report weekly., • Review the Service Code, Service Fee, Guarantor Definitions, and Program files in Avatar monthly to ensure accuracy in all areas., • Maintain a Cash Receipt Log by month., • Balance the Cash Receipts Log against the Monthly Posting report the first week of the month to ensure accuracy. Job Description OVERVIEW OF THE POSITION: Skilled, Independent Contractor certified in Cognitive Behavioral Therapy by the Penn Beck Community Initiative will provide trauma-informed therapy to underserved populations on-site at community-based mental health outpatient clinic. This includes diagnostic assessments and individual, group, and family therapy for a diverse caseload of adults ages 18+ experiencing a variety of behavioral health issues. The Independent Contractor will provide monthly group consultations to therapists who are in the process of being CBT certified by the Penn Beck Community Initiative. FUNCTIONS: • Provide weekly client-centered, trauma-informed, individual, group, and family therapy as per the specifications outlined in the contract., • Provide monthly group consultations to clinicians who are in the process of being CBT certified by the Penn Beck Community Initiative., • Complete and provide data to the Penn Beck Community Initiative as required., • With client consent, make concerted effort to engage identified support system in treatment., • Provide initial and ongoing assessment of presenting issues, diagnostic impressions, and recommended course of treatment., • Create treatment plan and routinely monitor progress in collaboration with client., • Manage own tax and financial affairs., • Coordinate care with other involved professionals to ensure continuity of care., • Enter intake and therapy appointments into EMR at least 48 hours in advance., • Maintain accurate records of performance metrics and time spent including minimum billing expectations commensurate with scheduled hours. This includes entering progress notes within 3 days of service, including supportive documentation (e.g., treatment plans, safety plans, case management notes, briefs, triage notes, discharge summaries, no show or cancellation notes)., • Encourage consistent attendance by routinely reviewing attendance policy and providing weekly reminder calls. Discharge inactive cases in a timely manner to promote access., • Refer for community resources including expanded mental health programming as needed., • Provide crisis intervention including safety planning as needed. This includes follow up and coordination of care when individuals go to the CRC or acute level of care., • Complete all mandatory training to maintain state, CBH, and NIAC credentialing. Job DescriptionThe IBHS Clinical Care Manager – Neurodevelopmental Services (CCM-ND) is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Care Manager must utilize clinical knowledge of best practices and evidence-based treatments, including Applied Behavior Analysis, for individuals with Autism spectrum and other neurodevelopmental disorders, to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, Clinical Care Managers consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. Essential Functions: Depending upon your team assignment, some or all of these may apply. • Be knowledgeable about behavioral health care services, system partners, service continuum and established medical necessity criteria and apply that knowledge in all work activities., • Complete Utilization Management activities including but not limited to: interagency meetings, social determinants interviews with members, treatment team meetings, case consultations, care coordination, provider meetings, reviews, treatment linkage., • Complete expected number of UM activities per day/shift as specified under team specified under team specific responsibilities in established timeframes., • Authorize and extend treatment authorizations when Medical Necessity Criteria is met., • Consult with Physician Advisor, Psychologist Advisor and/or Clinical Supervisor as clinically appropriate., • Generate denial letters based on physician consults and in accordance with state regulations as needed., • Complete discharge reviews in a timely manner., • Work directly with members, natural supports and providers in the community during 30-75% of work hours or 6-15 days per month., • Facilitate care coordination with system partners to address social determinants, barriers to wellness, and other needs of members, including connection to appropriate community based services., • Demonstrate an understanding of Evidence-Based Practices (EBPs) and connect members to appropriate evidence-based treatment at admission, during treatment, and at discharge., • Monitor member’s length of treatment and work collaboratively with providers and other systems to address barriers to member receiving treatment in a less restrictive environment., • Collaborate with other CBH departments to ensure quality of care, promote placement of members in the most appropriate services and to facilitate consistent longitudinal care management., • Document provider quality concerns in accordance with CBH policy/procedure., • Conduct provider meetings at least 4 times per year., • Use data to inform clinical decision-making., • Maintain documentation in CBH’s electronic system consistent with agency and industry standards., • Understand and participate in grievance/complaint hearings as assigned., • Respond to care coordination correspondence within 24-48 hours or sooner if necessary., • Work collaboratively with team members including assisting with the training of newly hired employees as requested, covering work when peers are out of the office as assigned and finding coverage when absent., • Attend all meetings as assigned and actively prepare for and participate in supervision and clinical case consultation., • Maintain an accurate daily log of work completed., • Contribute to projects and other duties as assigned. Team Specific Responsibilities: • Manage ABA-Designated Providers within the IBHS Level of Care, consult to teams managing other Levels of Care, as needed or assigned (e.g., Child Acute, RTF)., • Complete an average of 8-15 utilization management (UM) activities per day., • Complete reviews of service authorization requests, including review of Written Orders, Physician consultation per Review Guidelines, provider notification, referral submission (if required), denial letter (if required), authorization generation (if required), and log of review., • Participate in at least 8 inter-agency or inter-disciplinary team meetings per month for members being recommended for or receiving ABA Services, • Ensure collaboration and continuity of care for members with multiple services or in multiple child-serving systems., • Facilitate collaboration with community-based treatment team for members experiencing a crisis., • Conduct crisis consultations for members receiving ABA, as needed, including supporting member through access to higher levels of care and return to community-based services., • Conduct progress monitoring reviews for members in need of intensive care management, or as indicated by provider outlier performance., • Complete service authorizations completely and accurately, as assigned., • Education: Master’s degree in Applied Behavior Analysis, Psychology, or Special Education preferred. Master’s degree in a closely related field (e.g., Social Work, Counseling) with at least 18 credits in graduate-level coursework in ABA accepted., • License/Certification: BCBA or BCBA-eligible within 18 months ( 1.5 years) of accepting position preferred.
