Job Description Program Population: The Housing for Health Recuperative Care Center, located on the LAC Outpatient Center campus, is seeking qualified RNP and PA’s with a keen sense of empathy, enthusiasm, and passion for caring for the homeless population. The RNP/PA will be offering routine medical care in an outpatient setting for the residents of the Recuperative Care Center. The facility will provide interim housing to individuals who would benefit from this resource, such as those being discharged from a hospital admission, ED, or clinic setting, and are especially vulnerable to further decompensation or relapse of their medical conditions. Position Purpose: Under the direction of the Chief Medical Officer, and in compliance with the agency’s standardized procedures, the Nurse Practitioner (NP) is responsible for providing high-quality, patient-centered care to patients of JWCH, Wesley Health Centers. The NP will deliver primary and preventive healthcare services, including patient assessments, diagnosis, treatment, and management of acute and chronic conditions. The role involves working collaboratively with a multidisciplinary team to ensure comprehensive care that addresses the medical, social, and emotional needs of patients. The NP will also engage in health promotion, patient education, and disease prevention activities to improve overall community health outcomes. Key Responsibilities: • Provide direct patient care, including performing physical examinations, obtaining and reviewing medical histories, identifying diagnoses, and developing and implementing appropriate treatment plans., • Conduct initial intake assessments and ongoing evaluations; monitor, reassess, and manage acute and chronic medical conditions, including addressing newly identified health concerns., • Diagnose and treat acute illnesses, infections, and injuries, and continue treatment plans initiated by hospitals, emergency departments, or primary care providers., • Order, perform, and interpret diagnostic tests (e.g., laboratory tests, EKGs, imaging), and ensure timely follow-up and evaluation of abnormal results., • Educate and counsel patients on disease management, treatment plans, medications, potential side effects, and interactions, using appropriate tools and methods tailored to individual needs., • Ensure informed consent is obtained for assessments and treatments, and provide clear, comprehensive education related to care and follow-up instructions., • Coordinate care with primary care providers, specialists, and interdisciplinary teams to support optimal patient outcomes; participate in care conferences and discharge planning., • Facilitate referrals and collaborate with internal and external partners, including Housing for Health teams, and review referral outcomes as appropriate., • Provide specialized, patient-centered care to designated populations, including individuals experiencing homelessness; identify barriers to care and housing, and support goal setting and care planning., • Utilize motivational interviewing, de-escalation techniques, and supportive communication to enhance patient engagement and adherence to care plans., • Provide training and education to clinical support staff, including community health workers and nursing attendants, as needed., • Identify operational or clinical workflow challenges, training gaps, or collaboration issues, and communicate them to leadership., • Participate in quality assurance activities, including periodic review of referrals, abnormal findings, and follow-up processes., • Maintain compliance with all applicable laws, regulations, and professional standards, including the California Nurse Practice Act., • Maintain current clinical knowledge through continuing education, training sessions, and professional development activities., • Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors., • Participate in required safety and compliance trainings and apply learned practices in daily work activities., • Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace., • Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards., • Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role., • Participate in JWCH’s customer service training called AIDET on an annual basis., • Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions., • Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes., • Current CPR Certification from American Heart Association., • Current, unrestricted RN and NP License in the state of California., • Current and unrestricted DEA Certificate, • At least 2 years of experience as a Nurse Practitioner in a clinical setting, preferably in internal medicine., • Strong knowledge of and practice in the methods of harm reduction, • Experience working directly with individuals with both moderate to severe mental health and substance use diagnoses., • Excellent clinical skills in performing patient assessments, diagnosis, and treatment planning., • Ability to prescribe medications and develop treatment plans based on patient needs and best practices., • Strong understanding of state and federal healthcare regulations and compliance requirements., • Ability to work collaboratively with a multidisciplinary healthcare team., • Excellent verbal and written communication skills., • Strong interpersonal skills and ability to build rapport with patients and healthcare team members. The program is open to licensed primary care medical, dental, and mental and behavior health providers who are employed or have accepted an offer of employment at an NHSC-approved site. Awardees receive up to $50,000 in exchange for two years of service if they practice in a HPSA of 14 above (Tier I). Awardees can also receive up to $30,0000 in exchange for two years of service if they practice in a HPSA of 13 and below (Tier II).
