Department: ISH Salary: $80,000 Location: Union, NJ Work hours: Monday - Friday 9:00am-5:00pm Position Overview: Team Leader-Licensed, LCSW, LPC or LMFT (APN, RN, MD or Licensed Psychologist) provides the lead clinical and administrative supervision for CSS services. Under the direction of the Program Director coordinates, with support of other licensed clinical staff on the team, intake/referrals, assessment, person-centered planning and clinical supervision. Supervises program day-to-day service provisions and provides direct services for people who have serious mental illness. Essential Job Functions Create and Maintain: • A wellness and recovery-oriented environment by using person-first language, respectful documentation, person-centered planning, and evidence-based practices, • Perform a comprehensive rehabilitation needs assessment. Ensures timely completion of written comprehensive rehabilitation needs assessments (CRNA) by LCSW, LPC, LMFT) for each service recipient by the 14th day of admission, every six months for the first year after the initial assessment, and annually thereafter., • Contribute to the development, implementation, monitoring, and updating of the individualized rehabilitation plan. Ensures timely completion of and contributes to the development, implementation, monitoring and updating of each service recipients individualized rehabilitation plan (IRP). Each individualized rehabilitation plan and subsequent revisions shall be reviewed, signed and dated by a physician or fully licensed practitioner authorized by State law to recommend a course of treatment (LCSW, LPC, LMFT, MD, licensed psychologist)., • Provides therapeutic rehabilitative skill and resource development. Performs and ensures the CSS team members perform therapeutic skills development training with the aim of promoting community integration and restoring each individual served to the maximum possible functional level by improving functional, social, interpersonal, problem-solving, coping, and communication skills., • At a minimum, ensure the CSS team offers and makes available, or arranges for the services appropriate to meet service recipient needs as identified in each persons’ individualized recovery plan. Performs and ensures the CSS team members offer the full range of services as outlined in N.J.A.C. 10:37 to specifically include services outlined in B-4.4 Other services., • Perform and ensure the team performs evidence-based practices that are appropriate for implementing community support services. This may include Motivational Interviewing, Cognitive Behavioral and Educational Strategies, Permanent Supportive Housing, IMR and WRAP., • Perform crisis intervention. Perform and ensure the team performs crisis intervention services for persons served with acute psychiatric needs whose circumstances and conditions require rapid intervention. Provide guidance regarding accessing emergency response services. These crisis intervention services will consist of face-to-face, short-term interventions with a service recipient who is experiencing increased distress and/or an active state of crisis. Interventions and strategies include:, • Appraisal and assessment of, but not limited to, individual’s daily living skills, and teach individuals needed skills in the areas of budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, self-advocacy and ways to maintain housing., • Consult and refer to other services beyond the scope of CSS., • Contributing to the development and implementation of the service recipient’s crisis contingency plan and psychiatric advance directive, • Brief, situational assessment, • Verbal interventions to de-escalate the crisis, • Assistance in immediate crisis resolution, • Mobilization of support systems, • Clinical supervision and administrative supervision of staff. Ensure that clinical interventions are evidence based; stage based and lead to recovery goal attainment., • Facilitates team meetings focusing on individuals, program issues and team building., • Ensures program admission levels and referrals are processed in timely manner., • Outreach to newly enrolled individuals and encourages those reluctant to accept mental health services through interventions with them and/or family and friends concerned with their welfare., • Ensures each service recipient has a written Consumer Service Agreement that is reviewed by the service recipient prior to acceptance and signed by both the service recipient and CSS team upon the individual’s admission and that clearly articulates the roles and responsibilities of the CSS team and the service recipient., • Coordination and monitoring of the program’s day-to-day operations, documentation, contributes to staff orientation and training, contributes to staff performance appraisal., • Schedules on-call rotation schedule for team members. Provides back-up support for emergencies in consultation with clinical staff as needed, • Responsible for the collection and monitoring of outcomes, QCMR, NJMHAPPS and other quality assurance data pertinent to the program. Ensures that the team uses data to improve services, • Ensures timeliness of paperwork done by the team, completing monthly audits, and ensuring that all paperwork is completed according to the CSS DMHAS standards., • Technical assistance and education to members of the team in reference to best clinical practices. Utilizes best practices materials and strategies for individual and group interventions, in conjunction with the team. Actively seeks own supervision from supervisor at least monthly. Education/Certifications: • Licensed physician, psychiatrist, psychologist, advance practice nurse, or registered nurse, all licensed by the applicable State professional boards, or, • Practitioner of the healing arts licensed in New Jersey, including, but not limited to, a licensed clinical social worker (LCSW), licensed professional counselor (LPC), or licensed marriage and family therapist (LMFT), who shall possess a Master's degree from an accredited college or university in social work, rehabilitation counseling, or other related behavioral health or counseling program., • Valid driver’s license required, No more than one moving violation within the past 12 months, • Benefits: Medical, Dental, Vision, 403b, basic life and AD&D, flexible spending accounts, EAP, • Eligible for medical benefits after 30 days of employment, • Flexible work schedules, clinical training series, leadership development program, • 10 paid holidays (an 11th after 2 years of employment), generous vacation and sick time Bridgeway Behavioral Health Services is committed to inclusive hiring and dedicated to diversity in our work and workplace culture.
