Job DescriptionSalary: $38,000-$40,000 based on experience Phoenix Behavioral Health is seeking to hire a full-time Case Manager/Group Counselor for our Mental Health Partial Care program. The position's main responsibilities include running psycho-educational groups on-site groups, and group documentation, progress notes, biopsychosocial assessments, and coordinating case management supports for individuals who struggle with mental health and co-occurring disabilities. The job is site-based and consists of running psychoeducational groups and providing basic case management support for your caseload. Required Skills/Abilities: • Ability to work as a team member and have management, communication, organizational and interpersonal skills, • Ability to work as a team, be adaptable, multi-task, and be flexible in relation to department needs., • Experience with running groups., • Experience with providing case management and collaborating with external providers., • Experience with Timely documentation of group note, weekly summary notes, assessments, and treatment plans., • Knowledge of Joint Commission standards within the Department, • Demonstrates Proficiency in Verbal and written Communication Skills, • Knowledge of State and Federal Statutes Regarding Patient Confidentiality Laws, • Knowledge of Drug Free Workplace Policies, • Knowledge of Workplace Violence, • Knowledge of current technology, • Knowledge of Mental Health and Substance Use Disorders, • Familiar with community resources Education and Experience: • Bachelors degree in a related field preferred, Associate degree or HS degree with experience will be considered., • 2+ years of working with people with behavioral health needs (trauma, mental illness and/or substance abuse), • Experience in providing services in the mental health or Substance Abuse community, • Experience working on a multidisciplinary team, • Tenacity and ability to think out of the box and work creatively to engage participants, • Knowledge of the criminal justice system and community resources, • Computer literate. Proficient in Microsoft Office applications, • Excellent verbal and written communication skills
