Key Responsibilities\n\n • Conduct thorough assessments to identify the mental health needs of patients, utilizing evidence-based practices to develop individualized treatment plans.\n, • Maintain clear and effective communication with supervisors, team members, family members, and guardians regarding patient progress and treatment adjustments.\n, • Prepare and submit a structured schedule of therapy sessions, home visits, and related activities to ensure continuity of care and monitoring of patient progress.\n, • Coordinate home visits to gather information through observations, chart reviews, and staff interviews, using a comprehensive approach to patient assessment.\n, • Implement and monitor the effectiveness of treatment plans, engaging in regular evaluations to ensure high standards of patient care.\n, • Participate in interdisciplinary team meetings to discuss patient cases, share insights, and collaborate on treatment approaches.\n, • Document detailed progress notes on a monthly and quarterly basis, ensuring all records are accurate and readily accessible.\n, • Develop and provide training sessions to ensure that all staff and team members are informed and competent in implementing treatment procedures and documentation standards.\n, • Engage in follow-ups with patients through interviews and home visits, reinforcing their support system and addressing ongoing needs.\n, • Act as a liaison between the mental health community and the wider public, promoting awareness and accessibility of mental health resources.\n, • Contribute to the enhancement of training materials and procedures aimed at improving the quality of our mental health programs.\nPatient Population / Client Focus\n In this role, you will serve a diverse range of clients, including children, adolescents, and adults facing various challenges such as anxiety, depression, trauma, and relationship issues. Our goal is to provide personalized care that respects the individuality of each client, ensuring a safe and supportive environment for their healing journey.
