Key Responsibilities\n\n • Conduct comprehensive and ongoing assessments of patients to identify needs and measure progress toward their therapeutic goals.\n, • Facilitate effective communication among all stakeholders, including supervisors, family members, and collateral contacts, ensuring a collaborative approach to care.\n, • Develop and provide a clear schedule of appointments, home visits, and observations to supervisors for efficient case management.\n, • Coordinate home visits that include observing client interactions, conducting chart reviews, and performing staff interviews to gather relevant data for analysis.\n, • Assist in the implementation and monitoring of individualized program plans, ensuring adherence and effectiveness of therapies.\n, • Participate actively in Interdisciplinary Team Meetings to stay aligned with best practices and update on patient progress.\n, • Complete accurate monthly and quarterly progress notes to document client progress and treatment effectiveness.\n, • Organize and lead training sessions to ensure all staff are proficient in implementing program plans and maintaining accurate documentation.\n, • Provide follow-up interactions, including interviews and home visits, to support ongoing client needs and adjust treatment plans as necessary.\n, • Maintain thorough records to ensure compliance and availability of critical information for stakeholders.\n, • Act as a community liaison, engaging with public mental health resources to enhance our outreach efforts and support clients in achieving their goals.\n, • Contribute to the development of training materials and procedural guidelines to continuously improve service delivery.\nPatient Population / Client Focus\n Our LPCs work with a diverse population, including children, adolescents, adults, and families facing a broad spectrum of mental health challenges. You will have the opportunity to develop specialized skills in areas such as family therapy, couples therapy, substance misuse, and trauma-informed care, tailoring your approaches to meet the unique needs of each client.
