The Foster Care and Adoption Case Planner plays a crucial role in assessing needs, promoting safety, and supporting reunification efforts for children in foster care. What does a day on the job look like for the Foster Care and Adoption Case Planner? • Manages an assigned caseload of children in the Foster Care + Adoption program and provides casework services to ensure the child’s safety and well-being and support the child’s permanency planning goal, • Maintaining the goal to reunite the child in foster care with their family of origin while concurrently planning for other permanency routes for the child if reunification is not possible., • Completing, at minimum, monthly home visits with the youth in their foster home based on the child's level of care associated with their needs, • Make monthly contact with all identified parents of the youth in foster care, • Scheduling and facilitating visitation between parents and youth in foster care and siblings of the youth who may be placed in an alternate setting, • Providing casework services and support to birth parents in meeting court ordered requirements to achieve family reunification. Facilitating concurrent planning to ensure timely permanence for child if reunification is not possible, • Working closely in collaboration with local district case manager and other service providers in facilitating child’s permanency plan, • Managing the transition of children in placement from one setting to another (e.g. birth parents’ home to foster home, foster home to birth parents’ home or adoption home etc.), • Participating in Family Court proceedings to facilitate child’s permanency plan, • Supporting foster parent’s skill development in order to best meet the needs of children in their care, • Maintaining progress notes, case record documentation, and required reports in accordance with district requirements and NYS regulations, • Participates in a department on-call rotation The Qualified Candidate will have: • A Bachelor’s Degree in Social Work (BSW) plus two years experience in child welfare preferred, or equivalent combination of education and experience OR a Master’s Degree in Social Work (MSW) with experience in child welfare, • A valid New York State driver’s license and continued automobile insurance required, • Strong interpersonal and conflict resolution skills, • Accessibility to reliable transportation during working hours, • The ability to work as part of a team Shift Information: Must have reliable transportation for frequent travel throughout Western New York, primarily Erie County, and must be available to work a flexible schedule that includes regular evening hours for home visits and community events. SUMMARY The Care Coordinator-HH+ works with an assigned caseload of clients who are enrolled in the health home program and qualify for Health Home Plus (HH+). The Care Coordinator- HH+ is responsible for delivering an enhanced level of care coordination services to HH+ clients, including at least two (2) face-to-face visits per month and two (2) other core services. Clients enrolled in HH+ may also be enrolled in the Health and Recovery Plan (HARP). For HH+ clients that are also enrolled in HARP, the Care Coordinator- HH+ also assists with accessing Home and Community Based Services (HCBS) and Community Oriented Recovery and Empowerment Services (CORE). The Care Coordinator-HH+ conducts the NYS Eligibility Assessment, develops, and maintains the HCBS Plan of Care, and links clients to HCBS and CORE providers in the community. The Care Coordinator- HH+ also provides education to clients about HARP benefits and assists other care coordinators at JFS with linking their HARP enrolled clients to HARP services. WHAT YOU'LL DO A representative summary of tasks to be performed is provided below. The employee may be asked to perform job-related tasks other than those specifically stated in this description. The duties and responsibilities of the position are to be carried out in a manner that is consistent with the mission, values, and operating principles of Jewish Family Services. • Maintain a caseload of clients enrolled in Health Home Plus (HH+) and HARP and provide care coordination services in compliance with health home policy and standards of care., • Work with the program manager to determine and establish HH+ eligibility for each client on their assigned caseload., • Conduct home visits with clients and travels into the community to meet with clients in other community-based settings, including medical provider appointments, hospitals, residential settings, and other community service provider offices., • Conduct comprehensive assessment and develop a patient-centered Plan of Care to address client needs. Assist clients with achieving their goals, in accordance with the Plan of Care., • Complete crisis plan with all client on assigned caseload and review with client at least annually., • Conduct annual case review with client and members of the client’s care team to review the Plan of Care and address barriers that prevent client from meeting their Plan of Care goals., • Successfully complete the HARP Assessment training in the Uniform Assessment System for New York (UAS-NY). Performs HARPs assessments/reassessments; maintains an active Health Commerce System (HCS) account., • Monitor the delivery of HCBS services to ensure clients are following through with goals identified in their HCBS care plans., • Assist client with coordination of appointments including but not limited to scheduling, rescheduling, providing appointment reminders and arranging transportation., • Works closely with the interdisciplinary care team including primary care providers, medical specialists, mental health providers, residential services, substance abuse treatment program, etc., • Conduct research on community resources and government benefit programs to determine eligibility criteria, provide appropriate referrals, and perform follow up activities for referrals., • Utilize culturally sensitive and linguistically appropriate strategies to engage and deliver services to clients., • Accurately document all interactions with clients and all efforts made towards client engagement. Submit all progress notes within 48 hours of the client encounter., • Collaborate and case conference with the MCO HARP Care Manager, HCBS, and CORE providers to ensure on-going client linkage and engagement in HCBS and CORE., • Effectively utilize electronic systems, including Netsmart, HEALTHeLink, PSYCKES, and ePACES., • Provide education to other members of the Care Coordination Division team on HARP, HCBS, and CORE and assists with transferring of newly HAREP enrolled clients between caseloads as needed., • Attend agency and department in-service training and staff meetings as well as any other agency related activities as required., • Effectively support health home programs on an as needed basis. QUALIFICATIONS Education and Experience: • Bachelor's degree in any of the following: child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreation therapy, rehabilitation, social work, sociology, or speech and hearing AND, • Bachelor's level education or higher in any field with three years of experience working directly with persons with behavioral health diagnoses OR • Bachelor's level education or higher in any field and two (2) years of experience as a Health Home care manager serving the SMI or SED population. Job Description Program Summary: The Child and Family Services Residential Program offers a supportive environment for up to 46 children across three specialized programs. Focused on healing and growth, our dedicated staff provides treatment in small-group settings, helping children and families overcome obstacles and address traumatic experiences that have impacted their ability to live at home. Through comprehensive care, treatment, and supervision, we equip children and families with the skills needed for a successful transition, either back to their home or to an alternative living arrangement. Position Summary: We’re seeking a compassionate and skilled Registered Nurse to support and monitor the physical and mental health of children and adolescents in our Residential Treatment Facility. In this full-time role, you’ll provide direct care, manage medications, coordinate medical appointments, and work closely with our clinical team to ensure every resident receives the highest quality care in a safe, supportive environment. As the Residential RN you will: • Work with Pediatrician/Psychiatrist/Psychiatric Nurse Practitioner to address health care concerns, • Treat all minor illnesses; provide emergency first aid when necessary and notify Physician of all acute illnesses and emergencies requiring immediate medical attention, • Arrange clinic appointments for Pediatrician and all medical referrals including intake, annual and discharge physical and dental examinations for each resident, • Inform Nursing Supervisor, Residential Treatment Facility Medical Director and Pediatrician of results of all clinic visits, • Monitor and complete all ongoing medical charts and immunization records in a timely fashion, • Complete necessary medical reports for outside agencies or schools, • Implement prescribed orders for treatment, medication and referral; administer all prescription medications for residents; which includes maintaining daily documentation of all medication administration records, • Support living unit staff as required, • Actively participate in Therapeutic Crisis Intervention training and updates, and implement lower level techniques effectively and appropriately, • Use physical intervention appropriately and safely, • Write event reports correctly and in a timely manner What qualifications have prepared you for this role? • Registered Nursing Degree from a recognized accredited school of nursing, • Must be registered and licensed in NY State as an RN, • Ability to relate well to children and other staff members, • Possess a valid New York State Driver’s License and continued automobile insurance, • Ability to deescalate and manage difficult behaviors of children, • Ability to initiate or participate in physical interventions when necessary Competitive Pay Rate of $36 per hour Full Time Shift Available: • Sunday, Friday 6:00pm-630am & Wednesday flex day around 9:00am-9:30pm, 10:00am-10:30pm similar., • Wednesday, Thursday, & Friday 6:00pm - 6:30am C+FS offers full-time employees the below benefits to support our employees and their families and help to create a healthy work environment. • Employer Paid Life Insurance, • Medical, Two Plan Options, • Dental, Two Plan Options, • Vision Insurance, • Wellness Program and Incentives, • Health Savings Account (HSA) and Quarterly Employer-Contributions, • Healthcare Flexible Spending Account (FSA), • Dependent Care FSA, • Employee Referral Bonus, • Qualifying Employer for Public Service Student Loan Forgiveness Program (PSLF) Child and Family Services provides equal employment opportunities (EEO) to all employees and applicants for employment without regard to race, color, religion, sex, national origin, age, disability, veteran or familial status, or genetics.
