ABOUT THE ROLE: The Peer Case Manager will be a key member of the clinical team, supporting workflow, coordination, and case management for members. This role will manage a caseload of members working toward a variety of goals and is an integral part of clinical and case management services at SPY, while also supporting Access Center programming as needed. WHAT YOU’LL DO: • Conduct intakes, assess member needs, and refer youth to appropriate services/programs such as housing, mental health, benefits, education, parenting resources, and employment, • Use a harm reduction, trauma-informed, and housing-first approach when working with youth, • Provide mediation, crisis intervention, and supportive counseling to youth as needed, • Participate in developing individualized, youth-driven case management goal plans and progress notes, • Follow up with youth to ensure referrals and services are successfully accessed and needs are met, • Assist youth at risk of or currently experiencing homelessness with completing Access Center Intakes and referrals to emergency or interim housing, • Advocate for youth to access mental health and social services, addressing barriers and connecting them to ongoing care, • Support Access Center Team in day-to-day operations and programming, integrating clinical insights into workflow, • Run Front Desk and other core service operations on a rotation basis, connecting members with needed programs and services and ensuring smooth intake flow, • Establish authentic, supportive relationships with SPY members, providing guidance, mentoring, and coaching during drop-in service hours, • Coordinate and facilitate group activities, life skills workshops, and therapeutic or wellness focused sessions that promote community and resilience, • Ensure seamless communication across sites between members, case managers, and service partners, integrating clinical and program information, • Promote health awareness and prevention through community education, groups, tabling, and visual communications, • Engage West Los Angeles neighborhoods by attending community meetings, providing resources, and contributing to discussions from a trauma-informed, culturally aware, and restorative approach, • Develop and facilitate community resources, workshops, and toolkits that support wellness, healing, and restoration, • Document clinical activities, service plans, and outcomes in an effective manner while strictly adhering to policy and procedure, • Track data and enter information accurately into appropriate systems, • Bachelor's Degree in Social Work, Psychology, Sociology or related field preferred., • 2-3 years of experience in case management, counseling or related experience preferred., • 1-2 years of experience accessing social services or experiencing systems involvement (homeless services, DCFS, Probation/Parole, DPSS, etc.), • 1-2 years of experience providing peer education and knowledge of issues relating to homelessness preferred, • 1-2 years of experience providing direct services to high-risk youth is a plus, • Ability to work independently and learn from mistakes, • Reliable transportation to travel between multiple sites; is punctual and organized, • Can utilize a harm reduction, trauma-informed approach, • Ability to work evenings, • Ability to obtain First Aid/CPR certification and Employment Eligibility Verification, • Successful completion of background (LiveScan) check, • Strong computer skills and knowledge including word processing, database operations, spreadsheets and other software systems., • Good verbal and written communication skills, including organizational, detail, and interpersonal, • Ability to work effectively with people of diverse races, ethnicities, sexual orientations, and gender identities in a multicultural and multi-disciplinary environment., • A strong interest in providing direct service and pursuing a career in the field of social work, • A commitment to and passion for the mission, vision, values and culture of SPY, such as the models of harm reduction, housing first, trauma informed care, and strength based approach, • A self-starter who is prepared to drive innovative solutions for an emerging organization, • An ability to work in a fast paced environment that requires effective collaboration with a variety of organizations and individuals, • Prolonged periods of sitting at a desk and working on a computer., • Changes in the environment, such as the office or outdoors.
