Position: Licensed Vocational Nurse (LVN/LPN) Reports To: Director of Patient Care Services Schedule: Thursday – Saturday-Day shift and Graveyard available About the Role As a Licensed Vocational Nurse at Cabrillo Hospice, you will provide compassionate, direct patient care under the supervision of a registered nurse. You will follow individualized care plans, administer treatments, and collaborate with our interdisciplinary team to ensure optimal patient outcomes. Key Responsibilities • Deliver direct patient care in accordance with the State Nurse Practice Act., • Implement nursing practices based on comprehensive patient assessments and care plans., • Document patient care accurately and promptly., • Provide patient and family education regarding diagnoses, treatments, and self-care., • Coordinate with hospice services, including aides, OT, PT, MSW, ST, dietitians, and supervisors., • Use equipment and supplies efficiently and effectively., • Manage pain and other symptoms, evaluating patient responses to treatments and medications., • Participate in personal and professional growth, quality improvement initiatives, and team development., • Graduate of an accredited practical or vocational nursing program., • Minimum of 3 years nursing experience; hospice, community health, or medical/surgical experience preferred., • Current LPN/LVN license in the state., • Committed to professional standards and nursing best practices., • Strong communication, organizational, and interpersonal skills., • Valid driver’s license with insured, reliable vehicle., • Current CPR certification.
