Apply today and help us continue delivering exceptional care to those who need it most.Responsibilities • MAJOR RESPONSIBILITIES, • 1.Patient Support:, • a.Will receive the patients from Patient Development department through a warm handoff process;, • b.Attend the patients’ doctor visits to provide support, address concerns, and facilitate communication between patients and healthcare providers;, • c.Conduct home visits to assess patients’ needs, deliver resources, and provide additional assistance as required;, • d.Conduct an initial set up home visit for patients newly approved for home health care. Provide information on the program, as well as facilitate caregiver onboarding;, • 2.Resource Delivery:, • a.Deliver comprehensive patient binders to patients’ homes containing plan of care and training information;, • Communication:, • a.Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care;, • b.Make courtesy calls to patients to check on their well-being, address immediate concerns, and maintain open lines of communication between the patient and the Giving Home Health Care clinical team;, • c.Conduct milestone calls to track patient progress, update them timely regarding their care, approvals, caregiver status, and any other updates to their care;, • Advocacy:, • a.Act as a patient advocate by collaborating with healthcare providers to address patient concerns and ensure patients receive high-quality, personalized care;, • b.Advocate for patients’ needs and rights within the organization, promoting a patient-centered approach to healthcare deliveryMinimum Qualifications, • Strong work ethic and relationship building skills., • Self-motivated with the ability to manage time appropriately., • Strong communication skills inside of the office and outside., • Experience with MS Office products (Outlook, Word, Excel, etc.), • Minimum associate level education preferred.
