Job Description Job Description Position: Crisis Stabilization Counselor (QMHP/LMHP) Reports To: Program Director (Immediate Supervisor) Four Family LLC is a community-based mental health agency committed to delivering high- quality support services to the residents of the Tidewater area. Our mission is to empower clients to become strong advocates for themselves and their families while cultivating a respectful, empathetic, and supportive working environment for all team members. We are currently seeking a dedicated, reliable, and task-oriented Crisis Stabilization Counselor to join our team. In this role, you will work directly with individuals experiencing a mental health crisis that may threaten their community living situation. The primary goal of this position is to offer emergency support to help clients remain safe in their communities, preventing the need for hospitalization. Duties and Responsibilities: Provide intensive crisis stabilization services in clients’ homes or community settings based on individual needs Deliver short-term, goal-directed counseling to stabilize mental health conditions and prevent hospitalization due to psychiatric instability Monitor psychiatric symptoms and, when necessary, refer clients for crisis evaluation or inpatient admission if they are at imminent risk of harm Ensure medication adherence and assist in facilitating psychiatric evaluations during crisis interventions Refer clients to appropriate community resources and professional mental health or medical services to support their stabilization Collaborate with other service providers to coordinate care, ensuring comprehensive support for each client Complete and maintain accurate documentation including progress notes, discharge reports, service authorization documents, incident reports, and any other required case documentation Ensure that all documentation reflects the individual needs and progress of each client and is submitted within agency deadlines Attend clinical supervision sessions and agency meetings as scheduled Adhere to agency policies and procedures at all times Perform other duties as assigned to meet the needs of the program and clients Immediate Supervisor: The Crisis Stabilization Counselor reports directly to the Program Director. The Program Director is responsible for overseeing the performance and development of the counselor, providing guidance, and ensuring alignment with agency goals and policies. Minimum Knowledge, Skills, Abilities, and Professional Qualifications: Education: Bachelors degree in a human services field (e.g., Psychology, Sociology, Social Work, etc.) Certifications: Current QMHP-A certification or LMHP certification required Licensure: Active registration with the Virginia Board of Counseling, maintaining the necessary education, experience, and continuing education requirements Experience: o Previous experience working with adults who have serious mental illness, including crisis intervention o Strong clinical skills, particularly in providing daily living skills training for adults with mental illness Skills and Abilities: o Ability to utilize a person-centered approach in all interactions o Proficiency in clinical interviewing and identifying risk factors for suicide, violence, and other high-risk behaviors o Familiarity with mental health skill-building interventions and their application o Strong written and verbal communication skills for documentation and collaboration with service providers Other Requirements: o Valid Virginia Driver’s License o Ability to work flexible hours, including on-call availability o Organizational skills to meet documentation deadlines and maintain accurate client records Schedule: Hours vary based on client needs; on-call availability is required as part of this role. Work Location: Services will be provided across multiple locations, including clients’ homes and community settings.
