Gabriel House Supportive Housing Program is a building which was developed and is operated by the Addiction Rehabilitation Center Foundation, Inc., a nonprofit agency. Gabriel House is a residential building located on West 122nd street and the corner of Adam Clayton Powell Jr. Blvd in the Central Harlem section of Manhattan. The facility will provide 30 units of supportive housing to homeless or at-risk of homelessness, single adults (age 18 and over) diagnosed with HIV/AIDS, serious mental illness (SMI) or who are diagnosed as mentally ill and chemically addicted (MICA), currently known as persons with co-occurring disorders. CAMBA, Inc. is the on-site services provider at Gabriel House and provides case management and 24/7 security. Job Description Apicha Community Health Center aims to improve community health by providing access to comprehensive primary care, preventive health services, mental health, and supportive services. The center is dedicated to delivering high-quality, culturally competent services that enhance the quality of life. Position Summary: Works as part of the Care Team; Responsible to ensure highest quality of patient care. Provides support to the Nursing staff and Medical Providers in addressing the health care needs of our patients. Program staff will ensure that all apartments are in close proximity to public transportation and accessible to other amenities such as health care, shopping, laundry, and other necessary services. STHP housing program provides a model of service that addresses the interrelated problems of HIV and homelessness. Through a congregate residence at 27 Winthrop, and the apartments in the community, we provide transitional housing, case management support, and housing readiness training in a supportive environment, which will help clients develop the skills to find and maintain permanent, stable housing. The housing program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 109-unit residence will provide housing for 39 individuals referred by the NYC Human Resources Administration (HRA), and the remaining 70 units will be apartments for moderate-income households. The Resident Manager is responsible for the supervision of the residents and the overall operation of the residence during designated shifts. S/he will work collaboratively with the Clinical team and the Program Director to ensure that resident needs are being met, as well as the safety of the residence. The successful candidate will have strong communication skills along with experience working with individuals diagnosed with mental illness and/or substance use disorder.
