Job DescriptionDescription: Teachers are responsible for the design, planning, and implementation of a quality early childhood education classroom and the implementation of the program’s philosophy and curriculum. The position requires skill and expertise in curriculum planning, staff supervision, child assessments, and the ability to accept and work effectively with children and adults from diverse populations. REPORTS TO: Site Supervisor JOB CLASSIFICATION: Non-Exempt Classroom Duties: • Plan, prepare, and implement activities that are appropriate for the age level, skill and social development of the children served., • Complete planning activities for an assigned group of children and be a resource for materials, • Prepare, implement and ensure lesson plans meets and Individualized Learning Plans (ILP) reflects program philosophy and standards (i.e. Creative Curriculum, Common Core, Early Learning Foundations, Anti-bias curriculum, etc.), are developmentally and age appropriate (i.e. PITC, RIE, DAP, Desired Results, Performance Standards, etc.)., • Complete the developmental screenings (DRDP’s) based on enrollment and follow-up schedules., • Implement Individual and Family Service Plans (IFSPs). Interact, initiate, and encourage the children in all the activities during the day., • Adhere to and participate in the program evaluation of program using the Environment Rating Scale, CLASS, Desired Results, and/or NAEYC accreditation standards., • Be familiar with and adhere to the program’s policies, health and safety regulations and emergency procedures and be ready to implement them if necessary., • Maintain positive relationships with parents through daily verbal contact and/or written notes and document any concerns., • Submit written items for the parent newsletter., • Complete home visits (Early Head Start staff) and parent-teacher conferences., • Attend parent meetings as requested by supervisor., • Involve parents by recognizing their talents and soliciting their participation., • Relay to other classroom staff and supervisor any information or concerns expressed by parents., • Supervise Associate Teacher, Sub-Pool Teacher Aides or volunteers in the classroom as assigned by supervisor or in absence of supervisor., • Know emergency procedures and be prepared to take responsibility for carrying them out (Fire drills, earthquake, CPR, first aid). Job Description In the role of an Advanced Care Provider (Physician Assistant/Nurse Practitioner) you will provide concierge emergency medical care to Sollis Health patients in our clinics with the supervision/oversight of an attending Physician. This is a pivotal role as part of a growing organization providing seamless care to both adult and pediatric patients for cases that would usually be treated in a traditional emergency room. Attends and contributes to patient care, staffing conferences and other related meetings. 4. Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the patient. 5. Consults with other members of the rehabilitation team and the physician to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient. 6. Adapts program and/or plan of care according to the needs of the individual patient. 7. Promotes a safe environment and enforces the elimination of fire and safety hazards. 8. Orders supplies and equipment as necessary. 9. Cleans, maintains, identifies, and reports the need for repair of equipment, as necessary. 10. Puts Patient Service first, ensuring that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individual s needs and rights. 11. Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the speech language pathology treatment plan, and completes all related documentation and record keeping regarding these services. Services include but are not limited to application of therapeutic techniques in areas such as: cognitive-communication speech intelligibility oral motor skills swallowing disorders/dysphagia perceptual abilities orientation memory pragmatics psychosocial expression functional communication case management skills in the home and community 12. Coordinates the acquisition of adaptive communication devices and instructs patients in their use. 13. Facilitates identification of hearing deficits and appropriately refers for assessment, diagnosis rehabilitation and/or personal amplification. 14. Selects appropriate activities suited to the individual s physical and mental capacity, intelligence level and interest. 15. Instructs patients in the skills and techniques required for participation in therapeutic activities, and evaluates patient progress, attitudes and behavior as related to rehabilitative potential. 16. Performs routine reexamination as needed/required to modify/progress plan of treatment. 17. Coordinates care in the home and community with other members of the interdisciplinary team in collaboration with the referring physician. 18. Performs other related duties as required. SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS: 1. Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility. 2. Must possess or be eligible for a Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) or be eligible to participate as an ASHA Clinical Fellow. 3. Must have a valid driver s license or reliable, independent transportation to and from the patients residence. JOB SKILLS: 1. Good organizational and time management skills. 2.
