The model prioritizes quality outcomes and preventive care over visit volume. You will practice in a single neighborhood-based center supported by a fully integrated care team that reduces administrative burden and allows focus on patient care. Dedicated support includes a Medical Scribe, Medical Assistant, and Social Worker, along with on-site lab services and specialist consultation access. This is a full-time outpatient position, Monday through Friday, 8:00 AM to 5:00 PM. Physicians typically see 13–16 patients per day. Appointment lengths are structured intentionally to allow meaningful engagement — 20-minute routine visits and 40-minute new or diagnostic evaluations. The patient population is primarily Medicare, with an emphasis on coordinated chronic disease management, proactive preventive care, and measurable quality improvement. Compensation: Base Salary: $215,000 – $245,000 plus quarterly quality-based incentive. Sign-on bonus available.
