PCP Nurse Practitioner JOB SUMMARY Delivering primary care and continuous healthcare services to PACE participants, utilizing the knowledge and skills required to assess, plan, and provide care in alignment with PACE Program standards. Works closely with the clinical care team to conduct assessments, reassessments, develop care plans, and deliver comprehensive primary care. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Provide high-quality primary care with a strong focus on individualized evaluation, starting with a comprehensive history and physical exam, rather than defaulting to broad diagnostic testing panels., • Collaborate closely with the IDT before referring participants to specialists, ensuring referrals are necessary and that continuity of care is maintained., • Collaborate with the Medical Director to manage specialist referrals, initiate appropriate treatment prior to referral when indicated, review specialist notes promptly, and develop and communicate participant care plans that incorporate clinical judgment and consult findings to ensure comprehensive care., • Manage and monitor all of your participants' medications, aiming to exceed expectations in medication reconciliation, and medication interactions. Registered Nurse Care Manager SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. Regional Medical Director JOB SUMMARY: The Regional Medical Director (RMD) provides strategic clinical leadership and oversight for multiple BoldAge PACE sites across a geographic area that can include several states, maintaining active medical licensure in each applicable state[BF1] . Regional Medical Directors will be licensed in multiple Regions to allow for coverage for other RMD’s vacations and in emergency situations. Working in partnership with the Chief Clinical Officer, the RMD ensures BoldAge Pace consistently delivers exceptional participant outcomes, regulatory compliance and operational efficiency. The Regional Medical Director role combines direct participant care with a local panel of patients at the provider’s home office [BF2] with broader regional leadership responsibilities, including the ability to provide coverage for other regional locations including but not limited to fulfilling needs related to APP practice[BF3] , in an emergency or when state regulations require active patient practice. In addition to maintaining a clinical panel, the Regional Medical Director provides administrative leadership by overseeing the Regional Quality Assessment and Performance Improvement (QAPI) program, collaborating with national clinical leadership to develop and maintain medical policies, guidelines, standing order protocols, and ensuring the efficient use of resources to achieve program goals. Job DescriptionDescription: The Center Manager ensures efficient and effective daily operations of the assigned office location(s). Major responsibilities include, but are not limited to, overseeing all administrative and clinical office direct reports; maintaining a professional and proficient workflow across all clinical and administrative office functions; and supporting respective providers in daily initiatives.
