Job Description Job Summary: We are a home health company seeking a dedicated Clinical Supervisor with at least two years of management experience to join our team. In this role, you will be responsible for overseeing and coordinating the activities of our clinical staff to ensure the delivery of high-quality patient care in a compassionate and comforting environment. You will provide guidance, support, and supervision to clinical personnel, ensuring adherence to company policies and procedures, industry standards, and regulatory requirements specific to hospice care. The Clinical Supervisor will also play a key role in training and developing staff, fostering a positive and collaborative work environment, and promoting continuous improvement in clinical processes. Qualifications: • Bachelor's degree in Nursing or a related field (Master's degree preferred), • Valid registered nurse (RN) license, • At least two years of experience in a clinical leadership or supervisory role within hospice care, • In-depth knowledge of clinical best practices, regulations, and guidelines specific to hospice care, • Excellent leadership, communication, and problem-solving skills, • Strong organizational and time management abilities, • Ability to manage and prioritize multiple competing priorities, • Proficiency in electronic health records (EHR) and other relevant software Responsibilities: • Provide day-to-day supervision and leadership to clinical staff, including nurses, medical assistants, and other healthcare professionals, within a hospice home health setting, • Ensure compliance with company policies, protocols, and industry regulations specific to hospice care, • Collaborate with the management team to develop and implement clinical policies and procedures tailored to the unique needs of hospice patients, • Conduct regular performance evaluations and provide constructive feedback to staff, emphasizing the importance of compassion, empathy, and treating patients with dignity and respect, • Develop and deliver training programs to enhance the clinical skills and knowledge of staff in the context of hospice care, • Coordinate and oversee the orientation and onboarding process for new clinical employees, emphasizing the importance of building relationships and providing emotional support to patients and their families, • Serve as a resource to clinical staff, answering questions and providing guidance on complex cases or situations specific to hospice care, • Monitor and evaluate the quality and efficiency of clinical operations in a hospice home health setting, implementing improvements as necessary to ensure the highest standard of care, • Foster a positive and collaborative work environment, promoting teamwork, empathy, and support among clinical staff in providing end-of-life care, • Stay abreast of the latest developments in healthcare, clinical practices, and regulatory requirements specific to hospice care, and ensure staff are kept informed and trained accordingly, • Collaborate with other healthcare professionals and departments in the coordination of holistic and interdisciplinary care for hospice patients and their families, • Participate in relevant meetings, committees, and professional development opportunities as required to enhance clinical knowledge and stay up-to-date with industry trends in hospice care We are looking for a motivated and compassionate individual with at least two years of home health experience and two (plus years) of strong leadership experience to join our team as a Clinical Supervisor in our home health company. We want people to join our team in the role of Organ Recovery Coordinator with previous experience as a registered nurse (RN) in an ICU or critical care setting. Using their nursing background, Organ Recovery Coordinators (ORCs) oversee the process of procuring the gift of organ donation. In collaboration with the clinical team, ORCs assess the compatibility of organs for recipients, and expedite the surgical and technical processes to package and transfer the organs for transplant quickly and safely. They work alongside hospital personnel and surgeons to facilitate the lifesaving gift of organ transplantation. Key responsibilities this position will perform include: • Evaluates patient suitability for organ donation by reviewing medical records and consultation with medical staff., • Coordinates organ placement and allocation with transplant programs and surgeons., • Coordinates and assists in the surgical recovery of organs and peri-operative management – including logistics such as arranging transportation, ensuring surgical packaging, and preparing lab specimens for shipment., • Provides support to donor families and conducts medial/social history interviews., • Updates donor information into electronic medical record, • Other duties as assigned. The Organ Recovery Coordinator (ORC) has a work schedule of 12-hours per shift. Employees will work either day or night shifts (from 8:00 – 8:00). Employees working night shift receive an additional $5 per hour. Employees should plan to work 15 days per month. Work schedules are provided 30 days in advance. Organ Recovery Coordinators (ORC) annualized compensation of $150-170k A $5,000 sign-on bonus is offered to qualifying candidates. The ideal candidate will have: • Two plus years prior experience in a critical care setting., • Ability to exercise independent judgement and multitask., • Exceptional teamwork, interpersonal, communication, and conflict management skills., • Registered Nursing (RN) License Required, • Valid Driver’s License with the ability to pass MVR underwriting requirements, • This role requires flexibility, including availability for day and night shifts, weekends, and some holidays, based on organizational needs. Coordinators should be prepared to adjust their schedules to meet the demands of the role, ensuring 24/7 donor management and patient care. Intake activities include performing assessments, development and implementation of person-centered Plans of Treatment to enroll applicants into HCBA Waiver (Home and Community Based Alternatives), CCT (California Community Transitions), and ALW (Assisted Living Waiver). Collaborates with a CCT Social Worker (SW) on mutual applicants for CCT and/or ALW enrollments. Also, collaborates with HCBA case management team, which consists of a HCBA Registered Nurse (RN) case manager and two HCBA Social Work case managers, on updating required documents for continued enrollment in HCBA waiver. Responsibilities: To perform this job, you must be able to perform the following duties successfully: • Review of Medical Records including a clients Plan of Treatment, menu of health services, medication, medical equipment and other community-based services to ensure that the right amount and type of care and products meet the clients medical needs., • Communicate with providers/ patient participants or health care department or HUD housing representatives both verbally and in written format regarding clients care needs. Utilize electronic health record as required by departmental procedures to document all face-to-face or telephone or Microsoft Teams communication with the client, their caregivers, medical providers and social service agencies., • Work with an interdisciplinary team caring for clients enrolled in HCBA waiver, California Community Transitions Program, Assisted Living Waiver Program, or Enhanced Care Management/ Community Supports., • Institute tracking of quality indicators and reports to improve patient outcomes., • Coordinate with HCBA Case Management team members (RN and Social Work Case Managers) via Microsoft Teams or in-person meetings, or joint visits to clients as needed., • Conduct monthly visits with participants via phone, teams, or in person, • Performs in-person visits with applicants/ caregivers to complete initial assessment for HCBA waiver., • Responsible for addressing and communicating timely with Department of Health Care Services Nurse Evaluators regarding deferred cases. Essential Requirements: • Current California State license as a Registered Nurse, • Valid California Drivers License, • Eligible to be insured under our liability policy with a clean DMV report, • Your own vehicle and proof of current auto insurance, • Proficiency using software programs, and familiar with Microsoft office product including Word and Excel, • Ability to work independently and with others at all levels within the organization and collaborate effectively., • Above-average interpersonal, problem-solving, written and oral communication skills, • Discharge planning, home health or hospice experience, • At least one year of work experience as an RN, • Inquisitive nature and analytical skills, • CCM certification Service Region/ Location: This role is located in Sacramento serving Sacramento County and surrounding counties including Yuba and Sutter to the north, Placer and El Dorado to the East, San Joaquin to the South and Yolo, Solano, Marin, Lake, and San Francisco to the West. Currently 20% of the time is related to direct home visits with travel, 80% of the time are telehealth video visits and or calls and documentation in the Sacramento office. This is not a remote role and requires you to work in our Sacramento office. Schedule: Full-time, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch. No weekend work, NO ON CALL WORK. Physical Requirements: Job offer is conditional upon successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically includes the ability to lift to 35 pounds without assistance. PPE is provided to all employees Compensation: Pay range is $47 to $53 per hour.
