May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual as qualified to do so., • Assesses patients’ needs; identifies and initiates appropriate occupational therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional occupational therapy practice and interdisciplinary teamwork., • Observes and evaluates treatment affect, accomplishment of short- and long-term goals. Recommends change to physician, if needed., • Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services., • Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention., • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors., • Reports patient progress and barriers to discharge in each patient team conference, • Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate. Performs appropriate documentation., • Demonstrates knowledge and proficiency of occupational therapy modalities., • Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. The certified occupational therapy assistant performs only those services planned, delegated, and supervised by the registered occupational therapist. Roles & Responsibilities • Participates in the ongoing evaluation of patients functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) as delegated by the registered occupational therapist. Participates in the ongoing evaluation of the home environment for hazards or barriers to more independent living as delegated by the registered occupational therapist., • Participates in teaching new skills or retraining patients in once familiar daily activities that have been lost due to illness or injury, in accordance with organization policy., • Maintains appropriate clinical records, clinical notes, and reports to the registered occupational therapist any changes in the patients condition, • Submits clinical documentation in accordance with Organization policy, • Follows treatment program and goals for improved patient function as established by the registered occupational therapist. Documents patients response to treatment plan and progress toward established goals, • Maintains contact/communication with other personnel involved in the patients care to promote coordinated, efficient care. Documents such communication in accordance with Organization policy, • Attends and contributes to in services, case conferences, and other meetings as required by organization's policy to ensure coordinated and comprehensive plans of care for the patients of the Organization., • Identifies patient and family/caregiver needs for other home care services. Required Qualifications: * Graduate of an Occupational Therapy program accredited by the American Occupational Therapy Association. * Valid and current Occupational Therapy License in good standing. * Minimum of 2 years of clinical experience working with adults with physical disabilities. * Reliable transportation, valid driver’s license, and state-required auto insurance. * Must be medically cleared for communicable diseases and have up-to-date immunizations before engaging in direct patient contact. * Ability to pass a comprehensive background check, including criminal history, license verification, and drug screening. Desired Qualifications: * Advanced practice certification in dysphagia (preferred but not required). * At least one year of experience working with a frail or elderly population (training will be provided if not met). * Experience with adaptive therapy equipment and assistive technology for individuals with physical disabilities. * Strong teamwork and communication skills to collaborate effectively with a multidisciplinary team. * Proficiency with electronic health records and basic computer skills.
