Job Description The Virtual Sales Representative (VSR) will be responsible for marketing and selling CARDAMYST via teleconference and/or telephone to targeted health care offices and providers, in close partnership with aligned field representatives. They will deliver impactful core selling messages and educational materials as part of their remote engagements, reinforcing in-person efforts and expanding overall reach. VSRs will need to be flexible regarding job responsibilities as they will include a variety of strategic tasks: amplifying call frequency with high-priority targets, triaging and qualifying targeted HCPs, supporting large geographic territories, and engaging aligned sales team micro-focus practitioners. Job Description Catholic Housing and Community Services JOB TITLE: Social Service Coordinator POSITION REPORTS TO: Senior Center Manager/Satellite Coordinator DEPARTMENT: CHCS Senior Centers FLSA STATUS: Exempt Primary Purpose of Job The primary purpose of this position is to be responsible for the direct and indirect provision of social services offered at the senior center. Essential Job Functions · Provide comprehensive social services to center participants and consumers living in the community. This may include but is not limited to, information and referral, intake, assessment, case planning, and follow up. In special circumstances, when indicated, make home visits to check on the participant’s status, follow up on family issues, or determine appropriateness for center participation. · Obtain information to determine consumer eligibility and assist in making application for entitlement programs and benefits. · Directly provide, or coordinate, registration of center consumers and assessment of strengths, social service needs (i.e., nutrition/food insecurity, transportation, etc), and other circumstances which may facilitate or impede healthy aging. · Provides supportive individual or group counseling to consumers and their families. · Evaluates consumers for assistance with transportation and completes required documentation to facilitate services. · Maintains and updates consumer files, contact/attendance logs, and supporting documentation in compliance with CHCS and PCA requirements and professional standards. · Utilize PCA Client tracking database, COMPASS, and other software to facilitate work. · Develops and maintains a resource file for referring consumers to services in the community. · Coordinates the provision of social services with the Senior Center Manager and Program Coordinator.
