Job Description POSITION SUMMARY: The Behavioral Health Consultant (BHC) functions as part of the primary care treatment team. This individual provides behavioral health consultation, assessment and brief intervention to all patients in the primary care behavioral health setting. The BHC will administer and review screenings and assessment tools to identify psychosocial, psychological and developmental struggles. It is the role of the BHC to advocate for all patients and to connect patients and families to community resources as needed. The BHC will uphold the health center mission by assuring the delivery of high quality clinical services that result in better health outcomes, strong consumer satisfaction; and that help maintain the fiscal and operational capacity of the Behavioral Health Department. Team members are expected uphold the health center mission by assuring that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. In addition, staff must support FPCSN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff. The BHC provides care to patients across the lifespan. ESSENTIAL DUTIES & RESPONSIBILITIES: • Brief Consultations – Assist primary health care providers in recognizing and treating mental health disorders and psychosocial struggles within the primary care setting., • Assess the clinical status of patients referred by primary care providers., • Provide options counseling and referral to prenatal care as needed., • Collaborate/consult with Nurse Practitioners & other clinical staff regarding all patients seen by Behavioral Health Consultant., • Work closely with Providers at the site to identify at risk patients and collaboratively develop plans to prevent further psychological or physical deterioration., • Crisis Intervention – Be immediately available to the Family Practice and Counseling Network staff during working hours in the event of any psychiatric emergency., • Coordinate care of patients with medical/social work/outpatient and outreach staff to treat and manage patients’ chronic emotional and/or health struggles efficiently and effectively., • Refer patients to other levels of care or additional services as needed., • Participate in monthly clinical supervision with an IBH Director., • Participate in monthly clinical/administrative group supervision within Integrated Behavioral Health Department., • Attend and participate in team meetings, case conferences and interdisciplinary work groups/huddles as scheduled within primary care/behavioral health departments., • Ability and cooperation to provide BHC coverage at other clinic sites as needed and requested by Director., • Maintain competencies in mental health as required by all licensing and funding entities; maintain PA license to practice., • Charting - Utilize electronic medical record to document all patient interactions and complete all charting required by contracted funding sources to include assessments, progress notes and billing in a timely fashion., • Productivity – Meet productivity standards as established by the corporation, • Member of Family Practice and Counseling Network’s Multi-disciplinary Team- Attend and participate in meetings and quality improvement activities as required., • Respond to patient or co-worker complaints and work toward a positive resolution of any dispute., • Serve as a member of site committees as requested. Required Education and Experience • Masters in Social Work, Ph.D./Psy.D. in Psychology, or a related field., • Current Pennsylvania licensure as a Licensed Clinical Social Worker, Licensed Clinical Psychologist, or Licensed Professional Counselor., • Excellent working knowledge of behavioral medicine and evidence-based treatments for medical, behavioral, and mental health conditions., • Maintain and uphold Social Work/Psychology ethical and legal guidelines., • Ability to work with a culturally diverse population., • Ability to make quick and accurate clinical assessments of mental and behavioral health conditions via brief patient contact., • High level of comfort in working with primary care medical providers using a consultative style., • Ability to complete clinical documentation with concise, clinically relevant information in a timely fashion., • Demonstrates good time management skills and follow through., • Ability to design effective psycho-education curricula and lead effective psycho-educational group visits., • Ability to work effectively as a team, interfacing with patients, primary care providers, behavioral health staff, as well as with administrative and support staff., • Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent, and equitable manner, • Ability to effectively represent health center’s interests in the community and maintain effective working relationships among co-workers, public, private, and professional groups., • Ability to create a space for staff and those we serve to feel physically and emotionally safe., • Ability to support individuals on their paths to recovery and healing and resist re-traumatization of staff and patients., • Ability to pass required background screenings and credentialing verifications., • Required Immunizations: Hep B Vaccination and TB/PPD Test., • Optional Immunizations: COVID Vaccination and Flu Vaccination., • Current valid CPR Certification required., • Maintain current valid applicable position credentials. Job Description Job Title: Nurse Practitioner PROGRAM DESCRIPTION: WES at Philly Home at Girard (2100 W. Girard Ave, Phila PA) and the Riverview Wellness Village (7979 State Road, Phila PA) is an outpatient level of behavioral health care co-located within a shelter for those with housing needs and individuals living in recovery housing within the Riverview Wellness Village in Philadelphia. These housing programs are part of the Pathways to Wellness initiative within the Philadelphia Wellness Ecosystem. Services include care coordination, case management, individual and group therapy and psychiatric care for individuals with a history of substance use. Individuals challenged with substance use disorders often struggle with either seeking treatment or attending treatment consistently. Grounded in a "meet people where they are approach", this program aims to provide individuals with convenient and consistent access to behavioral health services where they reside. The WES multidisciplinary team collaborates with housing providers, primary care physicians, employment supports, substance use treatment providers and more in this integrated program. All team members will adhere to the WES philosophy of Diversity, Equity and Inclusion and adhere to the mission to promote total wellness and provide high-quality behavioral health services to culturally diverse populations through a combination of compassion, innovative thinking, nimble and adaptive strategy, and engagement with clients, staff, and provider partners. GENERAL OVERVIEW OF KEY ROLES & RESPONSABILITIES: The Nurse Practitioner for the Philadelphia Model is primarily responsible for providing (under the supervision of the Program Director and Medical Director) psychiatric evaluations and medication management as well as coordination of mental health care with the Interdisciplinary teams at both Philly Home and Riverview Wellness Village. This Nurse Practitioner will consult with the WES Medical Director, Program Director, and staff regularly regarding new admissions, residents' individualized treatment/care plans, and related matters for the purpose of assuring quality of care and maximizing treatment goals. ESSENTIAL & CORE FUNCTIONS: • Comply with all WES Outpatient Policies and Procedures and perform all duties and responsibilities at the designated program location(s)., • Work collaboratively with the WES Medical Director Program Director, Medical Director and inter-disciplinary care team to ensure compliance with all applicable federal and state regulations as it pertains to providing mental health care and services to the residents of Philly Home at Girard and Riverview Wellness Village., • Ensure initial psychiatric evaluation is completed within the timeframe directed per admission policy and co signed by the Program Psychiatrist., • Provide ongoing psychiatric evaluation and medication management support as medically appropriate., • Confer with Medical Director and staff regularly regarding clinical techniques used, resident progress, and related matters for the purpose of assuring quality of care, as well as providing on-going treatment to residents., • Attend regular case conferences with team members for consultation and input related to resident needs., • Responsible for ensuring the documentation of progress notes and evaluations for residents are in fu II compliance with clinic policies, federal and state laws, and regulations always., • Treats residents, fellow employees, campus providers and visitors courteously and with respect., • Other related duties as assigned. Must have current, working knowledge of information and techniques utilized to diagnose and treat diseases, withdrawal management and psychiatric disorders. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive mental health-care measures. Must have strong, active listening, oral comprehension and communication skills and be able to document care in Electronic Health Record (EHR). JOB SUMMARY The Drug and Alcohol Program Supervisor’s primary responsibilities will be the following: Provide clinical oversight for the designated Drug and Alcohol Program team under the direction of the Program Director. Provide individual and group clinical supervision in accordance with and/or the regulatory standards and guidelines of CBH, DDAP and OMHSAS to both licensed and unlicensed Substance abuse counselors in various environments and modalities. The emphasis of the Drug and Alcohol Program is multi-faceted and includes meeting clinical benchmarks for quality care and productivity, ensuring proper care is being provided in accordance with service delivery contracts and providing ongoing staff development. Duties may include establishing of clinical practice policy, case consultation, creating and reviewing clinical documentation and facilitating in-service training, oversight and management of Drug and Alcohol Program services. Collaboration with other departments is a vital component of Drug and Alcohol Program Supervisor’s role. The position can also include direct service by way of individual, family and group therapy to APM clients. Duties and Responsibilities: 1. Supervision a. Provide minimum one-hour individual supervision for SAC (substance abuse counselor) a month b. Provide minimum one-hour group supervision a month to SACs c. Both must be documented within 48 hours of providing supervision d. Conduct case consultation conferences with doctor and SACs and timely document the details on EHR e. Implementation of the policies and procedures for the operation of Drug and alcohol outpatient services in compliance with all Federal, State, County, and Funder requirements 2. Case load a. Provide therapeutic services to a caseload using a variety of therapeutic modalities b. Manage the SACs team and assign new clients c. Apply for BHSI for clients who doesn’t have insurance 3. Case load review every month for all SACs in the team a. Discharges are completed upon 60 days of not receiving counseling services b. Labs up to date for all clients (weekly/ bi-weekly) c. Treatment plans must be developed based on ASAM criteria d. Releases and other relevant documentations are up to date e. SACs have sufficient caseload f. All therapists providing minimum 3 sessions a month to a client or based on ASAM criteria g. Counselors are spending 70% of their work time at office h. Encouraging in-person services i. All services requiring client signatures must be in –person (exceptional case must be documented and discussed with supervisor) j. Enter diagnosis in Credible during the Intake session k. All services fully documented (signatures) within 7 days and must be ready for billing 4. Training a. Get list of all mandated trainings from Admin Director b. Follow up on training every month with SACs during the supervisions c. All SACs and supervisor must complete the required trainings before the annual performance evaluations d. Make sure SACs complete 25 hours of training a calendar year apart from the mandatory trainings e. Make appropriate training recommendations for the SACs f. Supervisor completes Performance evaluation and Individual training plans for all SACs before the end of the year g. Supervisor will train SACs in all documentations used at APM 5. Billing a. Supervisor must collect the billing cycle schedule from Billing Supervisor b. Ensure all SACs sends billing sheet within 72 hours of providing service c. Ensure all SACs sends Timesheet before the billing closing date and time 6. Documentation a. Supervisor must review all Treatment plans, evaluations, Intakes, family/ couple/ group therapy and approve for billing b. Supervisor must review progress notes randomly to ensure they are in compliance with treatment plan goals and charts are in good standing c. Complete case consultations with SACs and Dr (when needed) and document in Credible within 48 hours d. Complete incident reports for CBH and APM and send the documentation to Director of Operations 7. Supervision/ meetings with Directors a. Must be available for meetings with Directors once a week for 1-2 hours and as needed. b. Must submit a weekly report of your team during the meeting 8.
