Job Description Position Summary Hamilton-Madison House’s Caregiver Services Program for Manhattan 1 catchment area serves informal/unpaid caregivers of older adults, and their loves ones, throughout Manhattan’s community districts 1-7, comprising all of Manhattan below 59th Street and the West side of Manhattan to the northern tip of Central Park. The program provides direct services to caregivers, offering supportive counseling, information and assistance with benefits/entitlements, support groups, caregiver training; The program conducts extensive outreach and provides limited respite and supplemental assistance services. Under the direction of the director, the Outreach Specialist is an integral part of the team, creating and implementing an outreach plan to engage prospective clients with the program; especially underserved communities. Engaging and cultivating relationships with care givers and care receivers as well as professionals. Along with Caregiver Service Program staff ensure that information and resources are widely disseminated; and develop collaborations with community partners, religious institutions and health care providers, hospitals, Elected Officials and Business EAP contacts to generate expanded services for caregivers. The position also will require leading educational presentations and representing HMH at community tabling events. Responsibilities • Assist Director in developing outreach material, implementing outreach plans, and analyzing data from surveys of constituents (e.g. caregivers, care receivers, referral sources)., • Provides Community Outreach and Partnerships/Collaborations., • Work with program staff to create and deliver workshops, on caregiver services, eligibility and HMH Caregiver Program., • Identify resources (e.g. training videos, tip sheets) that can be used for caregiver training or for social media platforms., • Maintain records of public information contacts and educ/ outreach workshops., • Update the agency website with training resources for caregivers on the caregiver services page., • Connect interested clients with contact information and if indicated, collect consent from potential clients., • Work with Director and team to maintain and expand existing community partnerships by being active and visible in the community and working closely with other professional, business, civic and private organizations as indicated., • Assist with the development of content for social media platforms to keep Manhattan 1 catchment area caregivers, referral sources, and stakeholders informed of Program activities and resources., • Create Partnerships to enhance service provision as well as breadth of social media platforms., • Represent HMH in outreach, planning and execution of special events, including workshops, health fairs, community events,, • Develop strategies to reach isolated or difficult to find caregivers., • May be required to work evenings or weekend days for outreach events as needed., • Other duties as determined by Director and required by funding source. Minimum Qualifications • MSW/BSW or related degree, • Preferably 1 year experience with older adults desirable, • Strong commitment to the mission of the Caregiver Services Program, • Experience facilitating workshops and providing outreach preferred, • Nonjudgmental, socially perceptive, empathic and a good listener, • Sensitivity to understanding the needs of caregivers and older adults., • Knowledge of client engagement, assessment, and the biological, psychological, and social aspects of aging and the impact of disabilities and illness on aging and the family, • Highly organized and detail oriented; able to plan, manage and successfully complete multiple concurrent projects, frequently with short deadlines, • Ability to handle complex and difficult situations in a professional manner. Job Description PURPOSE OF POSITION Community Care is a division of New York City Health and Hospitals which provides community-based health care services including: Health Home and Care Coordination, Certified Home Health Care (CHHA)/ Care Management, and Outreach/Engagement. We have over 400 clinical and non-clinical staff embedded throughout the NYC H+H enterprise who service over 9,000 patients located in group/private homes, as well as those without housing in the four boroughs of New York City. Our qualified licensed occupational therapists help get patients on the road to recovery in the comfort of the patients’ home environment. Through the rehabilitation process, patients can recapture the skills necessary to live safely at home, or in the community. AREAS OF RESPONSIBILITIES • Provides physical therapy services in accordance with the physician’s plan of care., • Assists the physician in assessing the patient’s functional level by applying appropriate tests and measurements, and advises the physician in the development and revision of the plan of care., • Evaluates patient and establishes/upgrades the plan of treatment by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests., • Treats patients through the use of therapeutic activities to relieve pain, develop and restore function, attain and maintain maximum performance, and utilize adaptive equipment with the functional limitations of the patient and in accordance with the plan of care., • Observes, documents and reports the patient’s responses to treatment and any change in the patient’s condition to the physician, and/or the coordinator of care., • Evaluates the home environment and makes appropriate recommendations with regard to safety issues., • Orders assistive devices and equipment and trains the patient in the safe and effective use of the equipment., • Instructs and advises patients, family members and other home health personnel in the phases of physical therapy in which they may assist the patient; and in the safe use of assistive devices, and the completion of the home exercise program., • Demonstrates and teaches alternate techniques to perform the activities of daily living, transfers, and mobility and positioning., • Regularly reviews the rehabilitation needs with patients; initiates/notifies the coordinator of care and/or the physician regarding the need for revisions of the patient’s plan of care. SUMMARY OF DUTIES AND RESPONSIBILITIES The qualified licensed physical therapist will provide a comprehensive evaluation using evidence-based individualized treatment goals with patient input to reach their optimal functional level. They will provide home safety assessments, functional techniques to enhance critical skills such as strengthening, range of motion, balance, endurance, transferring from/to various surfaces, ADL/IADL’s, as well as education in the proper and safe use of assistive devices such as canes, walkers, transfer devices, wheelchairs, etc. Following are the working conditions: • Daily home visits require walking, driving or public transportation between visits., • May be required to climb stairs to make home visits and communicate with the office regularly before and at the end of the day and possibly between patient visits., • Travel between/to other boroughs may be required as needed., • Will be required to carry an agency issued laptop, clinical bag and cell phone. Minimum Qualifications New York State license and current registration to practice as a Physical Therapist; and, One year of post-license experience as an Physical Therapist. Plan and draft correspondence and proposals for prospects and current donors; draft gift agreements; and manage and help implement the stewardship process for Steinhardt donors who have made gifts for scholarships, fellowships, program support, and professorships. The position will assist with development and implementation of programs and engagement opportunities directly related to raising funds, potentially to include assisting with Dean's Council meetings, committee meetings, receptions, and presidential dinners. Required Education: Bachelor's Degree Required Experience: 2+ years experience with public relations, client relations, fundraising, or corporate marketing or equivalent combination of education and experience Preferred Experience: 2+ years experience in an academic environment Required Skills, Knowledge and Abilities: Must be an excellent writer with excellent interpersonal and verbal communication skills Strong analytical and project management skills Ability to plan and implement events Working knowledge of word processing, spreadsheet, and database software Preferred Skills, Knowledge and Abilities: Experience with specialized fundraising databases In compliance with NYC's Pay Transparency Act, the annual base salary range for this position is USD $70,000.00 to USD $84,700.00.
