Job DescriptionDescription: Summary The Nurse Practitioner / Physician Assistant provides high quality, patient-focused health care services to all patients in collaboration with the other staff members to ensure that the individually planned needs and desires of patients are met, regardless of age or gender, on a continuing basis by performing the duties outlined below. The Nurse Practitioner will practice/perform the medical services as delegated or directed by the supervising physician as long as they are within the scope of practice for the RN/APRN. The NP will abide by the standards and laws governing the practice of nursing and advanced practice registered nursing as established by the designated State Board and specified State Professional and Occupational Standards. It is the obligation of each Nurse Practitioner to ensure that their supervisory requirements are adhered to, that the relationship of and access to the supervising physician is defined, and that a process for evaluation is established. It is the obligation of each Nurse Practitioner to keep current with required continuing education and licensure. Essential Duties and Responsibilities include the following. • Maintains appropriate clinical standards. Follows the guidelines established by the Company policy and procedure manuals., • Examines patients for symptoms and/or physical information while ensuring the delivery of high quality, patient-focused health care all while recognizing the patient’s individual rights., • Aides the patients in their right to be informed and to participate in making decisions in all aspects of their medical care and treatment and initiates a patient education plan according to the individualized needs of the patient, including patient and family instruction. Ensures that all aspects of care have respect for personal values and beliefs, and is focused on the individual needs of each patient., • Elicits and reviews information about patient’s medical history., • Orders or executes various tests, analyses, results, reports and diagnostic images to provide information on patient’s condition in a timely manner to determine appropriate treatment and follow up., • Administers or prescribes treatments and medications appropriate for patient condition., • Promotes health by educational instruction as well as advising patients concerning diet, hygiene, and methods for prevention of disease., • Refers patients to medical specialist for consultant services when necessary for patient’s well-being., • Accurately, efficiently, and completely documents necessary data for patient records to facilitate completion of billing processes., • All entries into the medical record whether electronic or on paper will be legible, complete, dated, timed and signed with appropriate credentials. Documentation will be completed prior to discharge from the facility when possible but no later than 48 hours after discharge from the facticity., • Utilizes all equipment, supplies, facilities, and resources of the Company in a prudent and efficient manner., • Participates in department quality improvement activities, staff educational programs, attends departmental meetings, and demonstrates and maintains clinical competency in the delivery of patient care services in accordance with Company guidelines., • Participates in Peer Review and Chart Audit processes as designated by the Medical Director., • Maintains supervisory duties over staff as required., • Must maintain collaboration with the physician to deal with issues outside of their scope of practice., • Maintains competency in skills relevant to the scope of services provided in the facility. The scope of services in specific markets will require employees to complete relevant training and to obtain additional certifications or licensure. (DOT Certification, Occupational Medicine Certification, Dispensing Permits, Telemedicine permits, etc.), • Other duties as needed. Requirements: Education and/or Experience Master’s Degree in Nursing Certificates, Licenses, Registrations State Registered Nursing License; State Advanced Practice Registered Nurse License; Collaboration Agreement; CDS Controlled Substance License as applicable per prescriptive authority; DEA Drug Enforcement Agency Registration Number as applicable per prescriptive authority; Basic Life Support Certification. Active medical board certification required per state requirements. Other Qualifications Successful completion of Advanced Cardiac Life Support (ACLS) is preferred for full time NP. Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Reasoning Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills Ability to perform this job successfully, an individual should have basic knowledge of EMR systems, specifically DocuTap, Excel spreadsheet software and MS Word Processing software. Job Description SouthCoast Health is looking for a Family Nurse Practitioner for our Eisenhower Location Under the clinical supervision of a licensed physician, incumbent is responsible for diagnosis, treatment and management of the patient's health care, promoting health, and providing care in keeping with parameters established for mid-levels. As a primary care provider, extenders will provide services appropriate to their specialty. REQUIREMENTS: Current GA NP licensure; Current BLS/ACLS CPR Certification. QUALIFICATIONS: • Knowledge of professional nursing theory, practices and regulations to give and evaluate patient care., • Knowledge of how to use medical equipment and instruments to administer patient care., • Skilled in taking medical histories to assess medical conditions and interpret findings., • Skilled in proper documentation in patient charts., • Ability to communicate clearly and establish/maintain effective working relationships with physicians, patients, medical staff and the public., • Performs patient assessments and develops management plans for patients with acute and chronic health problems., • Collaborates with family practice/internal medicine/pediatric physicians, as appropriate, in managing acute and long-term medical needs of patients., • Initiates referrals to physician specialists and other providers as needed., • Initiates appropriate laboratory tests, EKGs, X-rays and other testing required for the evaluation of an illness; may perform minor office surgical procedures as allowable by established standards., • Takes medical histories, performs physical examinations and establishes presumptive diagnosis; documents findings., • Provides instructions in health maintenance and illness management to patients and their families., • Assures patient care needs are met and where possible exceeded., • Documents findings and actions in the medical record in a concise, accurate and timely manner., • May prescribe or administer medications and treatments as authorized by licensure or certification standards. Discusses suggested medications with primary physician or follow protocols established by supervising physician., • Assists in the utilization review process and quality assurance programs of the clinic as may be necessary., • Communicates with and assists physicians, clinical/nursing staff, coworkers in a professional and courteous manner.
