The Care Management Coordinator manages a team of care managers who provide comprehensive assessment and evaluation, care planning and coordination, outreach and crisis intervention, linkage to medical, financial, social, rehabilitation and psychiatric services, advocacy, development of social supports and procurement of needed resources.
