Job Description About the Role Youth Counselor (NSD) are the guardians of the daily milieu, and their role is absolutely central in our efforts to help the youth residing in the facility. Supervise their assigned children in everyday life from the time they get up in the morning to the time they go to bed, and through the night. They participate in daily living routines. From morning to night, the Youth Counselort guides the children in establishing and maintaining health promoting and socially desirable habits, regarding personal hygiene, grooming, diet and mealtime behavior, leisure time activities and interaction with peers and adults. They support and maintain standards of good care, safety and order. Generally, they provide a strong sense of security and well-being for each youth by assuring protection from being hurt physically or abused emotionally. They are concerned about youth’s health needs. Under medical or nursing supervision, they may care for a youth who is ill, give first aid and dispense medication. They encourage and, as necessary, assist youth in their formal education by helping with homework and conveying a positive attitude toward learning. Through participation in maintaining clean and attractive housing conditions, they help the youth develop constructive attitudes toward work as well as pride in their living environment. They are involved in planning and organizing leisure time activities with the children. They encourage participation in group games and development of skills. Youth Counselor conveys a sense of empathy and encourages a trusting relationship between youth and themselves. They interpret to youth the agency’s/DJJ policies and requirements. They consult, share information and participate with members of other disciplines in planning and evaluation conferences. What You’ll Do • Provides direct supervision and is always accountable for the whereabouts of residents., • Manages person-supported behavior with the goal of teaching the young people to make positive, lasting changes in their behavior., • Implements daily routines, schedules and procedures., • Assures the safety of the person-supported and maintains security., • Ensures high standard of sanitary conditions throughout the house including heavy cleaning and laundry., • Participates in the planning and execution of educational, recreational, and personal enrichment activities., • Participates in ongoing meetings, conferences, training programs as required., • Interacts and communicates with Rising Ground staff in matters related to, but not limited to, the needs of the persons supported and the program, in general., • Responsible for providing safe transportation for person-supported and attending various appointments (including driving persons supported to and from school, medical appointments, and off-site family visits)., • Attend all mandatory training required by ACS, OCFS and the Agency., • Responsible for adhering to all ACS, OCFS and agency regulations., • Uphold rules, repertoire of responses, confrontation techniques, application of the restriction system, negotiating restrictions, Force Fields, utilization of Safe Crisis Management techniques when necessary., • Manages the peer group, understands peer group dynamics, promotes and maintains a positive peer group culture., • Attention to and support of medical issues, student educational needs, casework, clinical appointments, etc. Monitors residents in Family Court., • Completes log reports, complete behavior assessment reports, submit documentation, incident reports., • Informs the Case Planner and supervisors of any issues related to the youth or their families as they become aware of such information, • Ability to work various hours and days of the week, • Performs other duties as assigned. Job Description About the Role: The Coach serves as a consistent, long-term strategic partner for young people, navigating the critical transition from adolescence to economic independence. Operating through a "whole-person" lens, the Coach manages a dedicated caseload of 15–20 participants, providing unwavering support from age 14 through 21. What You’ll Do Given the nature of their work, Coaches should also be able to be reached outside of a 9-5 office schedule. Build relationships with up to 15- 20 young people. Provide ongoing social/emotional support. Work 1:1 with young people to conduct an interest/strength inventory in order to develop individualized academic and career development goals, implementation of measurable steps to effectively assist young people in reaching their desired goals. Connect young people to quality schools, programs, resources, and opportunities in line with their goals. Help young people persist in these settings, navigate any challenges that arise, celebrate successes, and plan/prepare for the next step on their journey. Collaborate with Case Planners, Tutors, Specialists, and foster/birth parents, as needed, to help youth achieve their goals and ensure their overall well-being. Connect young people to peer group supports and build positive relationships. Ensure young people who are aging out can access and maintain stable, affordable housing and build independent living skills. Re-engage disconnected youth and help them re-enroll in an academic setting. Assess the fit between the young person and their current academic setting and help them transfer into a better-fit high school or high school equivalency (HSE) program, if needed. Visit the young person’s school/program and build relationships with school staff, review transcripts and academic data, provide educational advocacy, and discuss progress towards graduation/HSE obtainment. Ensure young people are attending school and have the supports and resources they need to be successful, such as tutoring, individualized educational plans (IEPs), 504 Plans, or other special accommodations. Expose students to post-secondary pathways, such as college and/or accredited vocational programs, starting in the 9th grade (or as early as possible). Assist students with post-secondary planning, along with the guidance counselors and/or College Specialists and Career Development Specialists. Connect the student to the College Specialist to apply to colleges, opportunity programs, and all forms of financial aid, funding, and scholarships. Provide college persistence coaching, including ensuring students are receiving all the financial and academic supports they need to succeed each semester. Collaborate with all relevant partners, such as Case Planners, guidance counselors, Specialists, Tutors, teachers, family members, and caregivers. Works closely with Case Planners to insure they have the important data needed to make entries into Connections and to verify the status of Vital documents. Assist young people in obtaining working papers, if needed. Conduct career exploration activities with young people. Assist ALL young people in selecting at least one career development experience each year that is in line with their interests and academic situation/background. Work with the young person and Career Development Specialist to identify potential longer-term career pathways based on the young person’s interests, strengths, and academic/vocational plans. Connect the young person to the Career Development Specialist, if needed, to explore workforce development programs, vocational programs, and for assistance with job applications. Connect young people, as needed, to agency-based job readiness and/or internship programs (e.g., YA WORC or the Mentored Internship Program). Provide persistence coaching to young people enrolled in any external program by checking in with them regularly. Ensure that the young person coordinates with a Housing Specialist (and/or Case Planner, if the agency does not have a Housing Specialist) to ensure that all young people’s housing applications are submitted as early as possible (by age 18). Accompany the young person to visit supportive housing residences, when needed. Help young people maintain their housing by providing independent living and life skills supports such as budgeting, obtaining a bank account, and advocating with landlords, if needed. Job Summary: The Case Planning Supervisor oversees the day-to-day case management activities for the assigned NSP and LSP programs, including Aftercare. The Case Planning Supervisor oversees the assessment, planning, family engagement, and evaluation process for case planners, ensuring compliance with all guidelines, quality assurance standards, and performance indicators for Close to Home programs. Work includes guiding referrals and discharge activities, coordinating and facilitating the treatment teams, monitoring, and guiding referrals and discharge activities as well as coordination of linkages with community-based support programs and services, and overseeing aftercare current aftercare case planners. The Case Planning Supervisor manages all social service needs and ensures that all ACS, OCFS and the Courts’ mandated timeframes are met for all the youth supported within the programs. This person must provide quality services in the areas of leadership, supervision, case reviews and monitoring, progress note reviews, and advocacy for our clients in placement in order to meet and exceed expectations with respect to program goals, quality services, compliance, and best practices. The Case Planning Supervisor will also assist with intakes for all youth coming into CTH NSP’s and LSP. More than half of our clients are immigrants and refugees from around the globe. Job Summary: The Case Planner provides comprehensive case management services for all youth who reside in their respective Site i.e. Non-Secure Placement or Limited-Secure Placement facility and their families. The Case Planner should be committed to a service philosophy and objectives that is youth-centered and outcome focused. The position is demanding and highly interactive with the youth and their families, staff and community. Case Planners must be comfortable working with youth and families in the context of the facility, the office and in the community of origin. The Case Planner will maintain a caseload of 15 youth both in the facility and in the community on aftercare. The role of the Case Planner is to streamline services and service plan goals, guide the discharge readiness planning beginning once youth are placed, and follow through with the youth/family in the application of the service plan in the community. Case management responsibilities include all necessary tasks to complete case file reviews, case planning, family engagement, and group service delivery. Case Planner is responsible for developing and maintaining meaningful contact with youth, their families, and service providers for medical, mental health, social, educational, and other related service needs. About the Role As the VP of Clinical Strategy & Population Health, you’ll be responsible for designing the clinical foundation of Arlo’s health plans—making care more affordable, accessible, and high-quality for our members nationwide. You’ll work across product, actuarial, and operations functions to define population health strategies, develop partnerships with providers, and build models that deliver better outcomes at lower costs. What You Will Do Some initiatives you will lead: Health Plan Design: Shape plan structures that drive preventive care, reduce wasteful spend, and promote high-value care. You’ll use data and clinical insight to design benefit structures that actually make people healthier. * Population Health Strategy: Identify key needs across Arlo’s member populations and develop scalable, data-driven programs to improve outcomes and reduce the total cost of care. * Provider Partnerships: Build relationships with value-based providers, specialist networks, and health systems. Craft incentive structures that align economic outcomes with quality improvement. * Care Navigation Models: Help guide how Arlo’s concierge and care support services proactively engage members—especially those with complex or rising risk conditions. * Clinical Voice: Serve as the clinical expert in product, claims, and underwriting discussions—ensuring that decisions are backed by sound medical reasoning and real-world impact. What We Are Looking For * Clinical Background: You have a clinical degree (e.g., MD, DO, RN, NP, MPH) and experience working directly with patients or health systems. * Population Health Experience: You’ve developed or led population health programs or value-based care models in a health plan, provider group, or ACO. * Health Plan Design Acumen: You understand how health plan structures impact member behavior and provider incentives—and have ideas for doing it better. * Strong Communication Skills: You can translate clinical insights into business recommendations and vice versa. You’re just as comfortable in a product roadmap meeting as in a clinical roundtable. * Bias for Impact: You like to build. You’re comfortable working in ambiguous environments and excited by the opportunity to shape a health plan from the ground up.
