Coordinator?Champion meaningful participant support by completing initial, change of condition, semi-annual, and emergency assessments for participants in a timely manner.Keep care plans accurate and up to date by documenting all assessments and Care Plans in the EMR with strong attention to detail.Collaborate as part of the care team by participating in IDT meetings and completing participant care plan activity interventions and other assigned components on time.Help move services forward smoothly by participating in Service Determination Requests, assisting IDT members with SDR tasks, and helping ensure service delivery is completed by expected effectuation dates.Create well-planned daily experiences by scheduling participants for Day Center activities in partnership with the Center Manager and or Senior Manager.Bring energy and creativity to programming by coordinating and leading recreational activities for individuals and groups alongside Physical and Occupational Therapists to help participants maintain or improve their physical functioning.Design programming that feels inclusive and relevant by coordinating culturally appropriate social programming and activities within the day center.Support specialized participant needs with care by coordinating programming and activities for the memory care day center.Keep the center lively and engaging by designing and communicating current events and activity offerings throughout the day center.Partner in day center success by working closely with the Center Manager and or Senior Manager to oversee activities within the care center.Build bridges beyond the center walls by developing relationships with community resources and coordinating outings in the community.Support outreach and community presence by partnering with Enrollment and Outreach Specialists during outreach events.Help create a welcoming environment by assisting with activity set-up and clean-up to ensure spaces are ready, safe, and inviting.Lead with flexibility and heart by guiding or assisting participants in large group, small group, and one-on-one activities.Support team development by providing training and programmatic feedback to Activities Aides, PCAs, and other designated support staff.Adapt programming to each participant by assessing abilities and changing needs and adjusting group activities accordingly.Set thoughtful goals that matter by establishing appropriate activity-related goals for each participant.Promote safety in everything you do by demonstrating and implementing daily activity practices that include infection control, handwashing techniques, disease prevention, food handling, and disposal of soiled articles.Pitch in to support the participant experience by assisting the Dietitian, Food Service Supervisor, and or Kitchen Staff with meal service as needed.Help maintain audit readiness and compliance by participating in DHCS, CMS, and other audits as required by the PACE program.Stay open to team needs by taking on other duties as assigned in support of participants and center operations, including PCA role and responsibilities Attributes of an Ideal CandidateBring a strong educational foundation with a High School Diploma or GED.Have experience in healthcare environments with prior experience working in healthcare facilities.Understand the needs of the population we serve through experience working with frail elderly populations.Meet at least one of the following qualification pathways:Two years of experience in a social, recreational, or educational program within the past five years, including at least one year full-time in an activities program within a healthcare, mental health, or handicapped program setting.Current qualification as an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist, occupational therapist assistant, or qualified social worker.Completion of at least 36 hours of state-approved training designed specifically for this role, along with regular consultation from an occupational therapist, qualified social worker, or recreation therapist.Be ready to support participant safety by obtaining CPR and First Aid certification within 60 days of employment.Communicate with warmth and professionalism with strong skills in working with participants, families, caregivers, and team members.Use sound judgment in care settings with the ability to apply appropriate safety measures while supporting participants.Stay organized and adaptable by being dependable, flexible, resourceful, and able to manage changing priorities.Bring language skills that strengthen connection with bilingual abilities in English and Spanish, English and Cantonese, English and Mandarin, or English and Vietnamese preferred.Be prepared for community-based work with proof of a valid California driver’s license, personal transportation, good driving record, and auto insurance as required by state law.Pertinent Information Be part of our Compton community at our center located in Compton, California 90221.Enjoy a consistent onsite schedule working Monday through Friday from 8:00 a.m. to 5:00 p.m.Work closely with supportive leadership in a role that reports directly to the Center Manager.What are the Physical Requirements?Ability to stand and walk for extended periods of time throughout the day center, clinic, and community settings.Ability to lead, support, and participate in group and individual activities, including those that may require moving throughout activity spaces, transporting materials, and actively engaging with participants.Ability to bend, stoop, reach, kneel, crouch, and twist as needed to assist with activity set-up, clean-up, participant support, and access to supplies or equipment.Ability to lift, carry, push, and pull up to 25 pounds regularly and up to 35 pounds occasionally, with or without reasonable accommodation, including activity materials, supplies, and equipment.Ability to assist participants with mobility during activities and provide safe physical support within the scope of the role, including helping with seating, positioning, and transfers as appropriate.Ability to use hands and fingers for repetitive tasks such as writing, typing, documenting in the EMR, preparing activity materials, and handling supplies.Ability to communicate clearly and effectively in person, by phone, and in group settings with participants, families, caregivers, and team members.Ability to observe participant behavior, engagement, and changes in condition and respond appropriately in a fast-paced care environment.Ability to work in a variety of environments including day center spaces, participant dining areas, activity rooms, outdoor settings, and community locations.Ability to follow infection control and safety protocols while working closely with a medically complex older adult population.Ability to maintain regular attendance and dependable punctuality to support participant care, activity programming, and team coordination.What is the working environment?Work in a dynamic PACE setting across
