Job DescriptionLocation: Downtown, Los Angeles, CA Position Purpose: The Site Monitor is responsible for overseeing program participants and ensuring compliance with program rules and expectations. This position is key to maintaining smooth and consistent facility and residence operations while actively participating as part of a multidisciplinary team, contributing to client engagement, retention, safety, and support. Program Population: The Pathway Home Housing Project is a HFH funded program, operated by JWCH. The Team consist of a cohesive team of MSW, case managers, site monitors and other professionals who collaborate to provide housing and critical services to People Experiencing Homelessness (PEH) and those who have previously experienced homelessness. The clients/residents may come with behavioral and/or substance abuse issues. The program utilizes the Harm Reduction Model and we “do whatever it takes” to encourage success of the client. Principal Responsibilities: • Monitor program participants to ensure adherence to program rules, expectations, and safety protocols., • Build positive and professional relationships with participants to foster trust, engagement, and cooperation., • Provide guidance and support to participants, addressing any immediate concerns or needs., • Model and encourage positive behaviors, promoting a respectful and inclusive environment., • Perform regular rounds to maintain a safe, clean, and orderly environment in the facility and residential areas., • Document and report any incidents, safety concerns, or rule violations in a timely manner., • Assist in maintaining security by monitoring entrances, exits, and common areas., • Perform light cleaning and organization tasks as needed to ensure a welcoming environment., • Recognize and respond appropriately to individuals experiencing mental health or substance abuse-related challenges, such as yelling, cussing, or outbursts of anger., • Use de-escalation techniques to manage challenging situations and prevent escalation of conflicts., • Collaborate with participants to identify barriers to progress and develop coping strategies for managing negative behaviors., • Provide emotional support and connect participants with additional resources or services as needed., • Participate actively in multidisciplinary team meetings to share updates on participant progress and challenges., • Collaborate with other staff members to ensure consistent program operations and participant care., • Support program goals by contributing ideas and feedback to enhance service delivery., • Accurately complete and maintain records, logs, and incident reports in compliance with program policies., • Monitor and document participant activities, progress, and concerns for internal records and reporting purposes., • Communicate critical information to appropriate team members and supervisors in a timely manner., • Complete and submit general reports as required, ensuring accurate and timely documentation of activities, incidents, and compliance with organizational or program standards., • Demonstrate understanding and practice harm reduction methods, • Assist with clients’ room searches and/or temperature control as needed., • Act as site lead during fire drills and real emergencies, and as lead in performing necessary evacuations., • On occasion will need to work an additional 4 hours after regular shift or 4 hours before the regular shift if situation warrants., • Assist clients in walkers and wheel chairs if needed., • Maintain shift reports and communicate with NENB Team when client issues arise, • Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors., • Participate in required safety and compliance trainings and apply learned practices in daily work activities., • Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace., • Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards., • Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role., • Participate in JWCH’s customer service training called AIDET on an annual basis., • Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions., • Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes, • Stand or walk constantly (for up to an entire shift) on various surfaces (tile, concrete, carpet)., • Climb stairs, ramps, or ladders occasionally during shift., • Must be able to kneel, squat, bend for any duration of time and run if needed., • Occasionally bend/twist at waist/knees/neck to perform various duties, including lift or carry up to 50 lbs., • Constant use of hands, arms, and fingers in reaching/handling/grasping while using radio, phone, notepad, writing reports and other administrative tasks., • Minimum high school diploma or equivalency, associate degree in social services preferred, • One - two years’ experience in behavioral health or social services environment, • Previous experience working with homeless persons or population is preferred., • Strong ability to communicate with homeless individuals who are actively participating in substance abuse and other factors related to homelessness., • Bilingual (English/Spanish) preferred, • Must be comfortable using computers to navigate web-based tools, complete online training, and utilize software for communication, reporting, and documentation purposes., • Experience handling confidential documents and detail-oriented., • Ability to use email and working knowledge of word processing (Microsoft Word) and spreadsheet programs (Excel)., • Ability to communicate clearly verbally and in writing, • Ability to assist with emergency and behavioral health personnel with aggressive patient situations. *All JWCH, Wesley Health Centers workforce members are recommended to be fully vaccinated against COVID-19.
