BronxWorks provides permanent supportive housing with an array of social services to more than 400 formerly chronically unhoused individuals and families throughout the Bronx. BronxWorks is seeking one Case Management Supervisor to join a diverse and enthusiastic team at Cooper Gardens, a multi-building supportive housing program located in the Bronx, NY (walking distance from Arthur Avenue.) Cooper Gardens provides services and permanent housing to 95 formerly homeless households, consisting of both single individuals and families with children. Work schedule is Mon-Fri, 9am-5pm. Staff are required to work one Saturday per month from 8am-4pm. All interested applicants must complete the application on our agency website at . RESPONSIBILITIES • Provide direct services to clients and manage a small caseload, which include home visits, escorts in the field, documentation of progress notes, assessments and service plans., • Maintain case records and statistics for program with the goal of analyzing trends and improving service delivery., • Assist the Assistant Program Director and Program Director in recruiting and training direct service staff., • Administer personnel policies and procedures under the guidance of the Program Director., • Hold weekly supervision meetings with assigned staff and monitor staff performance, including performing case reviews and structured observations., • Ensure all programs meet contractual goals and reporting requirements., • Submit internal progress reports on a monthly basis, or more frequently as required., • Participate in, and encourage staff to become involved in agency events., • Act as liaison with the community, including the area community district and board., • Manage a team of Case Managers and provide Clinical Supervision to Social Work staff, if applicable., • Act as Liaison with SSA, PPOH Psychiatrist, and medical clinic staff on and off-site., • Coordinate clinical services for tenants including discharge planning and hospitalizations in collaboration with Team Leaders., • Complete certification for Medication Monitoring Training and provide service on-site for tenants., • Provide clinical leadership and collaborate with the Program Director and Assistant Program Director to ensure program compliance with DOHMH and HRA contracts., • Hold monthly clinical supervision/trainings with staff supervised., • Participate in all staff meetings and training sessions., • Work one Saturday shift (8:00AM-4:00PM) every month., • Master’s degree in Social Work or in a related field of study with 1-2 years’ experience providing case management services required, or a Bachelor’s degree in Social Work or related field of study with 3-5 years’ experience providing case management services required., • Prior supervisory experience working in a social service agency strongly preferred., • Minimum of two years program experience in supportive housing strongly preferred., • Proficiency in modern business communications including person computers, electronic mail, voicemail, facsimile and copier equipment., • Ability to use a computer for prolonged periods., • Ability to occasionally lift and/or move up to 10 pounds., • Ability to stand, walk, or sit for long periods of time., • Ability to bend and retrieve objects and/or documents., • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation., • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
