Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained. Job Description The Clinical Care Manager (CCM) Specialist- Children’s Complex Care (High-Fidelity Wraparound) is part of a specialized team, responsible for providing high intensity care management, that works collaboratively and directly with members and providers to ensure effective, holistic, member-centric care. As a member of CBH’s Complex Care team, training and certification in High-Fidelity Wraparound, an evidence-based care management intervention, will be provided. The CCM Specialist will also be responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, and treatment quality monitoring. As a member of the Children’s Complex Care team, you will have an opportunity to apply your understanding of behavioral health and your cultural competence to work collaboratively with CBH members, their caregivers, their supports, other professionals, and community partners to screen/assess, and develop individualized, family-anchored, comprehensive, and strength-based plans of care. The CCM Specialist must utilize clinical knowledge of best practices and evidence-based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. Job Description POSITION SUMMARY: The School-Based Health Center (SBHC) Nurse Practitioner manages the SBHC and provides school health nurse services and primary care services to students in the assigned schools. POSITION SUMMARY The Behavior Consultant (BSC) performs assessments of a child’s, youth’s or young adult’s needs and development of an individualized treatment plan (ITP) that includes the interventions that will be used to meet the identified needs. The BSC also coordinates and consults with other service providers regarding behavior plans and interventions and discussion with the child’s, youth’s or young adult’s treatment team about the behavioral interventions that should be used and when they should be used. BSC services can be used to address behaviors in the school, home, or any other place where a child, youth or young adult needs behavior support (i.e.- across settings) and to learn strategies to better succeed. POSITION RESPONSIBILITIES • Collaborate with other members of the treatment team in order to design and direct the implementation of an individualized treatment plan (ITP) to be implemented across settings of the child., • Develop a trusting partnership with family members and other team members., • Ensure clinical interventions are properly formulated in order to design, implement, monitor and adjust the behavioral interventions for the child to ensure progress can be made over time and in accordance with the ITP., • Observe child in the home/school/community setting as part of the initial assessment in preparation for designing the individualized treatment plan and implementation of the written order. Functional Behavior Assessments (FBA) will also be needed (if properly trained with certificate)., • Design data collection tools to be used by Behavior Health Technician (BHT) and/or other team members to accurately assess effectiveness of established therapeutic interventions., • Develop Individualized Treatment Plans (ITP) to instruct other members of the therapeutic team (including teacher, teacher’s aides, other school personnel, family, BHT, MT and/or other designated individuals providing direct service or supports to the child) in their roles and responsibilities in implementing the established interventions., • Complete and submit accurate and timely documentation which includes but is not limited to: Individualized Treatment Plans (ITP), Encounter Forms, Progress Notes, Progress Summaries, Addendums, Crisis/Safety Plans, FBAs, Assessments, Structured Tools, Incident Reports, ITM summaries, Discharge Summaries, etc. by assigned deadlines., • Provide individual mentoring and supervision of all assigned BHT staff, • Conduct Functional Behavior Assessments (if properly trained with certificate) –FBA for BSC-ASD/BC-A requires a professional license plus FBA certification from DPW or a professional license plus BCBA. For non ASD FBA’s – the FBA certification is sufficient., • Supervision Requirements:, • BSC clinicians who provide services must receive at least one hour of individual face-to-face supervision per month which includes oversight of the following: The interventions being implemented, the child’s, youth’s, or young adult’s progress towards the goals of the ITP, consideration of adjustments needed to the ITP and the staff person’s skill in implementing the interventions in the ITP., • If the individual who provides BSC services supervises BHT treatment team members, the BSC must receive the following supervision in addition to the above: One hour of face-to-face supervision per month which must include a discussion of the BHT services being provided., • On-Site Supervision Requirement: BSC clinicians who provide services must receive 30 minutes of direct observation of the services being provided every 6 months., • BSC's must complete at least 40 hours of training within the first year and 20 hours each year thereafter. These training hours must be Department-approved and related to the specific job functions and in accordance with their individual training plan. The training hours will also include mandatory agency and/or funder or state requirements., • Valid Act 34, Act 151 and Act 114 clearances. EDUCATION/CREDENTIALS/EXPERIENCE As taken from Annex A, Title 55: Human Services, Part III: Medical Assistance Manual, Chapter 1155: Intensive Behavior Health Services General Provisions, individuals who provide individual services through BSC must meet one of the following: • Be licensed in this Commonwealth as a behavior specialist., • Have a certification as a BCBA or other graduate-level certification in behavior analysis that is accredited by the National Commission for Certifying Agencies or the American National Standards Institute., • Have a graduate degree in ABA from a college or university accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation or have an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. or the National Association of Credential Evaluation Services. Job Description The Clinical Care Manager is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Care Manager must utilize clinical knowledge of best practices and evidence based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, Clinical Care Managers consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. Job Description Title: After-School Program Site Director Department: Youth Outreach Adolescent Community Awareness Program (YOACAP) Reports To: Unit Director Program Overview The Youth Outreach Adolescent Community Awareness Program (YOACAP), is a program partner of the Urban Affairs Coalition (UAC). Our agency’s mission is to enhance the quality of life and health for Philadelphians through community-based and culturally competent interventions. Since 1989, we have addressed health concerns, workforce development, and life skills in low-income, underserved communities. Today, our mission has expanded to help youth and young adults learn job readiness skills, academic achievement, and experience using technology. Position Summary The Site Director will be responsible for the implementation and general management of YOACAP’s OST program (after-school and summer camp). Responsibilities include program planning, coordinating daily activities, working directly with students, managing the Activities Leaders, and written evaluation of staff on a regular basis. Project BUILD is designed to provide middle school youth with a diverse range of age-appropriate experiences, which assists in the development of personal competencies and skills through specific program content. Contextualizing program content through STEM and other interest areas will enhance youth engagement and program success. The program operates after school during the academic year and hosts a summer camp. Project BUILD uses a facilitated club model with an emphasis on structured activities. Understanding the importance of youth having unstructured time, this is incorporated into the daily schedule with Fridays being Freedom Fridays. Job Description The Clinical Care Manager is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Care Manager must utilize clinical knowledge of best practices and evidence based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, Clinical Care Managers consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. Job Description Position Title: Mobile Therapist Reports to: Clinical Administrative Manager/Clinical Coordinator (as assigned) Department: Administration Adoption Behavior Health HBDS IPS PEP Foster Care TIPS AERC Program: FLSA Status: Exempt Non-exempt Fee-For-Service Location: Coulter Street POSITION SUMMARY A Mobile Therapist (MT) delivers child centered, family-focused psychotherapy utilizing evidence-based modalities as described in the child’s Individual Treatment Plan (ITP). Services are delivered in the home or community setting within the Intensive Behavioral Health Services (IBHS) program. Therapy is focused on resiliency factors and connection to community-based supports as identified by the treatment team. The MT utilizes a strengths-based approach to empower children with emotional and behavioral needs to thrive and to help the child and family reach their goals. Mobile therapy services consist of individual therapy, family therapy, development and revision of the ITP (as needed), assistance with crisis stabilization and assistance with addressing problems the child, youth or young adult has encountered. POSITION RESPONSIBILITIES: • Provide evidence-based, skill-based psychotherapy in the home/community environment respectful to the child/family schedule and as prescribed medically necessary services based on clinical need., • Collaborate with other members of the treatment team in order to design and direct the implementation of an individualized treatment plan (ITP) to be implemented in the child’s natural setting(s)., • Develop a trusting partnership with the child, family members and other team members., • Ensure clinical interventions are properly formulated and implemented., • Observe child in the home/school/community setting as part of the initial assessment (as assigned) in preparation for designing the individualized treatment plan and implementation of the written order., • Develop ITP (as designated) to instruct other members of the therapeutic team (including teacher, teacher’s aides, other school personnel, family, and/or other designated individuals providing direct service or supports to the child) in their roles and responsibilities in implementing the established interventions., • Complete and submit accurate and timely documentation which includes but is not limited to: Individualized Treatment Plans (ITP), Encounter Forms, Progress Notes, Progress Summaries (as assigned), Addendums (as assigned), Crisis/Safety Plans, Assessments (as assigned), Structured Tools, Incident Reports, Discharge Summaries (as assigned), etc. by assigned deadlines., • Supervision Requirements:, • MT clinicians who provide services must receive at least one hour of individual face-to-face supervision per month which includes oversight of the following: The interventions being implemented, the child’s, youth’s, or young adult’s progress towards the goals of the ITP, consideration of adjustments needed to the ITP and the staff person’s skill in implementing the interventions in the ITP., • If the individual who provides MT services supervises BHT treatment team members, the MT must receive the following supervision in addition to the above: One hour of face-to-face supervision per month which must include a discussion of the BHT services being provided., • On-Site Supervision Requirement: MT clinicians who provide services must receive 30 minutes of direct observation of the services being provided every 6 months., • MTs must complete at least 40 hours of training within the first year and 20 hours each year thereafter. These training hours must be Department-approved and related to the specific job functions and in accordance with their individual training plan. The training hours will also include mandatory agency and/or funder or state requirements. As taken from Annex A, Title 55: Human Services, Part III: Medical Assistance Manual, Chapter 1155: Intensive Behavior Health Services General Provisions, individuals who provide individual services through mobile therapy services shall meet one of the following: • Be licensed in this Commonwealth as a psychologist, professional counselor, marriage and family therapist or clinical social worker., • Be licensed in this Commonwealth as a social worker or a behavior specialist and have a graduate degree that required a clinical or mental health direct service practicum from a college or university accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation or have an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. or the National Association of Credential Evaluation Services. The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency., • Have a minimum of 1 year of full-time experience in providing mental health direct services to children, youth or young adults and a graduate degree with at least nine credits specific to clinical practice in psychology, social work or counseling from a college or university accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation or an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. or the National Association of Credential Evaluation Services. Job Description JOB DESCRIPTION JOB TITLE: Case Manager JOB CLASSIFICATION: Exempt Full Time (40 hours total) DEPARTMENT: Catholic Social Services, McAuley House Housing and Homeless Services Division REPORTS TO: Program Supervisor DIRECT REPORTS: None SUMMARY: Provide Social Worker-Case management services to clients of McAuley House Residential Program, according to social service standards of Catholic Social Services and the Office of Homeless Services PRINCIPLE DUTIES AND RESPONSIBILITIES: • Complete intake paperwork on new clients and orient them to program., • Complete full assessment of each assigned client., • Develop a comprehensive Individual Service plan, based on identified needs that include Long Term Goals, Objectives and incremental behavioral Action steps with specific target dates and appropriate timeframe for completion., • Monitor progress of Service Plan goals and objectives., • Assist clients with individual medical, mental health, Social Security and County Assistance Office appointments, as needed., • Maintain and update (monthly) individual client Medical Logs and Narcotic prescription distribution sheets in the Medical Office and in individual client charts., • Create and update Medical Providers List and Medications List in client chart., • Maintain client medications in file, assisting clients with the organization of medications, removing unused medications, and disposing of them properly., • Enter Progress Notes into client electronic chart on a weekly/biweekly basis or whenever pertinent information regarding client medical, mental health and drug and alcohol history needs to be recorded., • Document all pertinent housing information into Progress Notes, i.e., required AACO Housing documentation needed to complete AACO (Department of Public Health and HIV Health) Application and all other meetings with housing providers, visits to new apartment locations, transitioning to independent living, clients moving out and any discharge information that is needed to close the chart., • Maintain statistical information regarding program outcomes for regular reports to Catholic Social Services (Harmony), OHCD (Care Ware) and OHS/HMIS (Client Track.), • Meet with all new clients at least once a week or more if necessary during first month of residency. Bi-weekly meetings after the initial intake period., • Monitor health and all prescribed client medications., • Monitor compliance in all treatment programs., • Complete housing applications., • Maintain case files for all assigned clients emphasizing accuracy and order while meeting with the professional standards of Catholic Social Services and other funding providers., • Provide individual counseling within competency limits., • Conduct resident group meetings as scheduled., • Meet with McAuley House Program Supervisor for individual supervision weekly or as scheduled., • Develop and maintain a comprehensive collection of resources and referral sources available to clients.
