Job Description SRVS-Direct Support Professional (DSP) Position Purpose: The Direct Support Professional (DSP) is a member of the clinical team that provides compassionate support and assistance to persons with intellectual, developmental, and behavioral health challenges and require assistance to achieve their fullest potential. The DSP provides the assistance necessary to allow the people we serve to live independently and ensure their wellness, health, and safety. The DSP, in collaboration with an interdisciplinary team, provides a wide range of services to meet the environmental, physical, psychosocial, and personal needs of the persons in our care. The DSP is responsible for supporting and implementing the individuals' vision and outcomes per the Person-Centered Support Plan. The DSP will ensure agency compliance with policy and procedures to ensure that the persons supported are safe, appropriately supervised and have the support they need to live independently. The DSP embraces the mission of organization by fostering a culture of compassion, hope, service, and collaboration. Major Responsibilities: DIRECT DUTIES: • Participates in the circles of support to assist with the development and implementation of outcomes that are agreed on by the circle of support. Staff will be expected to document the person supported progress toward their vision in the daily participation logs, using techniques identified by the support plan. • Completes all State and agency-related training and implements training goals assigned to the person supported circle of support. • Keeps an accurate account of each person supported progress on all PCSP goals by recording his or her responses via daily notes recorded in the electronic health record. • Assists and makes sure that the person supported with all daily living skills are protected from harm., • Attends all scheduled staff and agency meetings, and training for recertification, and other as assigned; does not bring children or relatives to the meetings. • Provides data collection and implements Behavior Supports Plan developed by the Behavior Specialist or Analyst • Completes orientation to pass all State and agency requirements for training at 80%; and completes any future training assigned by the supervisor or because of periodic job evaluations • Maintains a current driver's license and provides the agency with documentation of personal vehicle maintenance, and proof of vehicle insurance of any outside or agency vehicle being used for community participation or other activities by which the staff transports person supported • Transports person supported social, recreational, and religious activities as scheduled on the activity calendar. Does not transport person supported to his or her own personal home without the approval of the Director and conservator if applicable. • Assists in the maintenance and care of the home, supplies, and equipment (to include keeping vehicles clean and the vehicle log up to date), and is accountable for the use and inventory of the equipment, supplies, and appliances purchased for the person supported. Does not smoke, eat, or drink in agency vehicles. • When assigned, administers routine oral medication as prescribed according to agency policies and procedures, as per instructions for medication administration training for unlicensed personnel • Completes required incident reports (GERs) and calls in reportable incidents within the required timeframe of discovery or witnessing any abuse, neglect, exploitation, or reportable staff misconduct; Notifies family/conservators, ISC/SC of any incidents within the required time frame. • Makes accurate and complete entries in the home's daily log according to policies and procedures to assure 24-hour logs are present and accurate. • Ensures passengers and self are safely secured in seat belts before moving vans or other vehicles from a parked position. All persons supported must be assisted when needed on and off all vehicles. • Report for duty at the scheduled time, properly utilizing the time and attendance system currently in place • Goes off duty when appropriately relieved, • Can account for the whereabouts of people supported during on-duty hours., • Demonstrates respect for all agency persons supported families, neighbors, and co-workers daily., • Reports and documents via the worker’s compensation First Report of Injury and other forms all staff injuries within 24-hours of occurrence to the supervisor and to Human Resources. • Is aware of agency on-call procedures to report emergencies related to persons supported, home/facility or vehicle after office hours, and on weekends or holidays • Assists with personal care needs of the person supported which will include, but are not limited to bathing, feeding, ironing, laundry, personal grooming and other activities of daily living. • Assists with the preparation of well-balanced and culinary diverse meals to meet nutritional needs as outlined in the approved meal plan. • Assists the Residential Manager with quality inspections of the home/facility to ensure that the person supported home meet sanitary and safety compliance. • Assist person supported in the preparation of belongings for travel (e.g. home visits, overnight visits with friends, vacations, etc.) • Documents the person supporting expenses monthly via a cash expenditure envelope and ensures receipts are secured in the cash expenditure envelope. • Maintains an accurate record of each person supported PCSP by documenting daily outcomes, action steps, and significant events. • Ensures compliance with required dress code and agency identification badge guidelines., • Uses universal precautions and follows the infection control policy when assisting person supported and/or staff. • Responsible for appropriate implementation of behavior and therapy plans., • Ensures accountability for person supported medications, MARS, vehicles, personal items, monies, food, and all other daily essentials. OTHER RESPONSIBILITIES: • Assists, encourages, and supports the person supported with practicing cleanliness and safety. Staff are aware of policies, procedures, and laws for supporting the assigned person supported. • Practices and encourages professional ethical standards to ensure consistent and fair supervision and training. • Assists and supports the person supported with keeping a friendly, clean, and organized home setting., • Assist the person supported and supervisory staff with compliance for all regulatory inspections., • Serves on agency committees as assigned., • Completes all special projects assigned by the Residential Manager, QIDP, Home Coordinator, Assistant Residential Director or the Regional Director. Job Qualifications and Competencies: Education and Experience: • High School Diploma or GED or at least one year of direct care experience Job Competencies: • Strong verbal and written communication skills;, • Strong documentation skills;, • Strong judgment, problem solving, organizational, and time management skills;, • Strong interpersonal skills and ability to communicate effectively with individuals with disabilities, families, co-workers, staff, supervisors, funding sources, and other social service agency personnel; • Physical ability to fulfill job requirements; -The ability to supervise and/or assist persons supported of Residential program; -The ability to work independently with minimal supervision. Other Requirements: Must be at least 18 years old. Must possess a valid driver's license from TN, AR, or MS. Hourly Pay: $15.00 $150 signing bonus received after 90 days of employment.$200 referral bonus if you refer someone and they are hired. Received after 90 days of referral's employment.
