We are an EEO/AA Employer. What you’ll do: Provides direct treatment, rehabilitative and supportive services with a focus on Housing needs in a community setting to ACT participants. Works as an ACT team member to assess participant needs, participates in development of service plans and assessments. Will serve as the ACT specialist for housing and will collaborate with other ACT team members to provide housing related services and education for all ACT team participants. • Meets with the participants in the community (80% of time) in accordance with established policies and procedures and clinical protocols to provide housing resources, skill development and other support services as needed., • Works as part of a clinical team to facilitate recovery and inspire hope for participants while supporting their achievement of service plan goals.
