The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress. Full-time position pay range $100K - $115K / Year depending on experience. • Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and physical therapy needs. Consults with the physician in the development of the therapy plan of care., • Provides services within the scope of practice as defined by the state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the health care team., • Observes and reports the patient's response to treatment and any changes in his/her condition to the nurse supervisor and/or the physician., • Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice and evidence based techniques., • Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals., • Visits patients within the entire service area of their respective agency and other nearby LHC agencies as needed., • According to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHC policy., • Completes all patient evaluations and develops the PT plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to nurse supervisor within 24 hours of the evaluation., • Participates in staff conferences and committees as requested by the agency., • May supervise home health aides per LHC policy if allowed per state regulation., • Provides supervision of the PTA as per LHC policy and state regulation., • Schedule flexibility is needed in this role to ensure we provide care to the patient as needed. Requires the ability to participate in a weekend rotation and potential availability on observed holidays to ensure we are meeting the demands of our patients., • Prepares a written discharge summary and/or a written summary report on patient's condition at least every 60 days., • Provides in-service education as requested by agency leadership., • Adheres to LHC Therapy Standards of Practice in provision of services to all patients., • Is responsible for accessing education to maintain and enhance clinical knowledge and skills that are up to date with the current standards of care for the profession and to maintain state issued licensure., • All other duties as assigned. Maintain timely and ongoing communication with ORR DHUC for complex cases and clinical recommendations. · Participate in treatment team meetings and other meetings pertaining to the needs of the children in care. · Complete internal audits to ensure that the clinical records meet the requirements of standards set by OCFS and ORR. · Assist in facilitating training and shadowing for new clinicians. · Maintain confidentiality of sensitive and protected information according to HIPAA standards. · Integrate a collaborative approach with all different departments in the program to ensure clear communication, quality service provision and teamwork to meet the needs of the children in care. · Demonstrate commitment to departmental and organizational goals. · During times of influx, the lead clinician could be temporarily assigned a small caseload if needed to support the program. · Assumption of other duties as assigned. Job Description The Mental Health Association is seeking Per Diem Crisis Outreach Supervisors for our new program, 988 Mobile Crisis Outreach Team, operating out of the Hudson County location. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. MCORT will work in coordination with the 988 Suicide and Crisis Lifeline crisis centers. (The 988 Suicide and Crisis Lifeline was formerly the National Suicide Prevention Lifeline.) MCORT is to be comprised of a two-person team in the field under remote supervision by a third professional from a centralized location. Shifts are on an as-needed basis for this position. Specifically seeking holiday and weekend coverage. Qualified applicants for the position of Per Diem Crisis Outreach Supervisor will have, at minimum: • A Master’s Degree in a mental health related field from an accredited institution, a professional license (LSW, LAC) and two years of post-master’s full time equivalent professional experience in a psychiatric setting required., • A professional with experience in a crisis setting is preferred., • A valid NJ driver’s license and a good driving record., • The ability to provide effective crisis response and de-escalation as related to community mental health in the community. Job Description The Mental Health Association is seeking a Crisis Outreach Supervisor for our new program, 988 Mobile Crisis Outreach Team, operating out of the Hudson County location. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. Under the direction of the appropriate level supervisor, the DSP performs various tasks and duties to assure that the living environment is properly maintained and kept in a clean, neat and orderly condition. • Performs/teaches developmentally disabled individuals routine housekeeping duties such as dusting, washing floors and walls, cleaning bathrooms, and vacuuming rugs., • Assists developmentally disabled individuals in bed making., • Follows safety procedures in living unit by mopping up spills, reporting unsafe conditions, teaches and reinforces fire safety procedures. Checks to see that fire exits are clear, night-lights and exit signs on, windows locked, etc., • Stores supplies and assures that storage areas are kept in a neat and orderly condition., • Responsible for storage and replacement of linens., • May assist in various household inventories., • Performs/teaches minor and routine maintenance tasks such as changing light bulbs, fuses, replacement of faucet washers; and obtains repair services for appliances., • Performs/teaches routine removal of trash, cleaning of trash containers, ashtrays, etc., • Teaches/performs outdoor maintenance tasks and repairs of residence, yard, garden, walks; reports need for or obtains repair services for automobiles, vans, mowers, etc., • Checks fire equipment and reports any malfunction., • Teaches and reinforces use of a variety of communication skills., • Guides individuals through motor exercises designed to increase physical coordinative functions., • Teaches and reinforces principles of human growth and development, including human sexuality, as determined by each person's developmental level and program plan., • Teaches acceptable work habits, e.g., punctuality, dress and grooming, to enable individuals to develop readiness for work outside the home, or in support of individuals' habilitative goals., • Accompanies developmentally disabled individuals to community activities including worksites, cultural, religious and social events, day programs, and visits to health practitioners; aids them in developing awareness and use of community resources and in interacting with others., • Provides support for maximum community integration., • Transports/arranges for transport of individuals to community events; and aids them in development of their mobility skills., • Maintains an adequate supply of and accounting of personal clothing and possessions., • Teaches money management principles to developmentally disabled individuals; and completes/aids in completion of banking activities., • Provides an accurate accounting of all financial transactions made on behalf of individuals in their care., • Helps developmentally disabled individuals to develop/maintain positive interpersonal relationships with their peers, with others in their families, and with others in the community. Job Description Our Waste Collection CDL Driver’s main responsibility is to safely operate a rear-loading truck, for the pick-up service of bags or containers filled with waste or recyclable material. Drivers transport the material to a disposal, transfer or recycle facility, and may make one or more trips throughout the shift. Primary Responsibilities: • The driver is responsible for safely operating the truck in compliance with Mamais’ safety standards as well as OSHA and DOT regulations, and local traffic laws., • Performs pre- and post-trip inspections of assigned vehicle using utilizing standard DVIR (Driver Vehicle Inspection Report); notes all issues on DVIR form for corrective review/repair., • Ensuring that the container and its content are always secure. This includes periodically checking tarps and ties, and may require the driver to climb onto containers and/or reach overhead., • Perform pre- and post-route inspections of assigned truck and document any safety issues on standard reports., • Continuously monitor for safety and service-related issues, as well as containers of other equipment in need of repair, and report any issues to the Supervisor., • All Mamais employees are expected to be responsible and accountable for the safety for themselves, their colleagues, the trucks, and other equipment and property. Drivers must immediately notify their Supervisor of any incidents, accidents, injuries, or property damage., • Communicate professionally with customers and supervisors at all times. This includes receiving complaints about schedules and quality of service.
