The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress. Full-time position pay range $100K - $115K / Year depending on experience. • Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and physical therapy needs. Consults with the physician in the development of the therapy plan of care., • Provides services within the scope of practice as defined by the state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the health care team., • Observes and reports the patient's response to treatment and any changes in his/her condition to the nurse supervisor and/or the physician., • Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice and evidence based techniques., • Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals., • Visits patients within the entire service area of their respective agency and other nearby LHC agencies as needed., • According to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHC policy., • Completes all patient evaluations and develops the PT plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to nurse supervisor within 24 hours of the evaluation., • Participates in staff conferences and committees as requested by the agency., • May supervise home health aides per LHC policy if allowed per state regulation., • Provides supervision of the PTA as per LHC policy and state regulation., • Schedule flexibility is needed in this role to ensure we provide care to the patient as needed. Requires the ability to participate in a weekend rotation and potential availability on observed holidays to ensure we are meeting the demands of our patients., • Prepares a written discharge summary and/or a written summary report on patient's condition at least every 60 days., • Provides in-service education as requested by agency leadership., • Adheres to LHC Therapy Standards of Practice in provision of services to all patients., • Is responsible for accessing education to maintain and enhance clinical knowledge and skills that are up to date with the current standards of care for the profession and to maintain state issued licensure., • All other duties as assigned. Job Description The Mental Health Association is seeking Per Diem Crisis Outreach Supervisors for our new program, 988 Mobile Crisis Outreach Team, operating out of the Hudson County location. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. MCORT will work in coordination with the 988 Suicide and Crisis Lifeline crisis centers. (The 988 Suicide and Crisis Lifeline was formerly the National Suicide Prevention Lifeline.) MCORT is to be comprised of a two-person team in the field under remote supervision by a third professional from a centralized location. Shifts are on an as-needed basis for this position. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. In all programs, we provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. We want all children we support to move forward with their lives and thrive. About the Role The Clinical Supervisor oversees the clinical services of the UC program. We are seeking a Clinical Supervisor with a license to provide clinical services (i.e.: LMHC or LMSW), and we are invested in supporting their continued professional development by providing clinical supervision towards further clinical licensure. Maintain timely and ongoing communication with ORR DHUC for complex cases and clinical recommendations. · Participate in treatment team meetings and other meetings pertaining to the needs of the children in care. · Complete internal audits to ensure that the clinical records meet the requirements of standards set by OCFS and ORR. · Assist in facilitating training and shadowing for new clinicians. · Maintain confidentiality of sensitive and protected information according to HIPAA standards. · Integrate a collaborative approach with all different departments in the program to ensure clear communication, quality service provision and teamwork to meet the needs of the children in care. · Demonstrate commitment to departmental and organizational goals. · During times of influx, the lead clinician could be temporarily assigned a small caseload if needed to support the program. · Assumption of other duties as assigned. Job Description The Mental Health Association is seeking a Crisis Outreach Supervisor for our new program, 988 Mobile Crisis Outreach Team, operating out of the Hudson County location. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community.
