    Job Summary The Care Coordinator plays a vital role in ensuring that clients receive the highest quality of care and support. This position involves coordinating services, managing schedules, and facilitating communication between clients, families, and healthcare providers. The ideal candidate will possess strong organisational skills and a compassionate approach to client care. Responsibilities Coordinate and manage care plans for clients, ensuring all services are delivered effectively and efficiently. Liaise with healthcare professionals, families, and clients to facilitate communication and address any concerns. Maintain accurate records of client information, appointments, and care plans in compliance with regulatory standards. Monitor client progress and adapt care plans as necessary to meet changing needs. Provide support to clients and their families by answering questions and offering guidance on available resources. Transport customers into the community, and to and from there homes. Experience Previous caring experience is essential, demonstrating strong skills. Experience in a healthcare or social services environment is highly required. Proficiency in using systems for record keeping and communication purposes. Excellent interpersonal skills with the ability to build rapport with clients and their families. Strong problem-solving abilities and attention to detail are crucial for success in this role. Responsibilities: • Operate flatbed lorries and other commercial vehicles in compliance with all road safety regulations and company policies., • Safely load, secure, and unload cargo to prevent damage during transit., • Plan routes efficiently to ensure timely delivery of goods across designated locations., • Conduct routine vehicle inspections before and after journeys, reporting any maintenance issues promptly., • Maintain accurate documentation of deliveries, mileage, and fuel consumption., • Adhere to all legal requirements related to driving hours, load limits, and safety protocols., • Communicate effectively with dispatchers and clients regarding delivery schedules and any unforeseen delays., • Uphold a professional appearance and behaviour when representing the company on the road. Skills • Proven experience in commercial driving, particularly with flatbed vehicles., • Strong knowledge of driving regulations and safety standards within the UK., • Excellent driving skills with a clean driving record., • Ability to operate large vehicles confidently and manoeuvre in tight spaces when necessary., • Good organisational skills for route planning and documentation management., • Physical fitness to handle loading/unloading tasks when required., • Previous experience as a delivery driver is advantageous but not essential; training will be provided for specific vehicle types if necessary.
