As a Pregnancy Mother and Baby Practitioner you will provide specialist family-focused support to pregnant women mothers and their babies where there are substance use concerns. The role is key in safeguarding and promoting positive outcomes for both mothers and their children through early intervention effective partnership working and tailored support. Location:Liverpool Full Time Hours: 37.5 per week (Monday to Friday) Full time Salary Range:27861.26 - 32002.35 dependent on experience (based on full time working hours pro rata for part time hours) • Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours. About the role: • Hold a specialist caseload of pregnant women and new mothers affected by substance use ensuring care is holistic and family-centred., • Provide advice information and structured interventions around substance use during pregnancy and early parenting reducing risk and promoting health and wellbeing., • Develop SMART recovery and care plans in collaboration with families and professionals reviewing progress regularly., • Act as a named point of contact for mothers ensuring consistent and trusted support throughout pregnancy and postnatal care., • Lead on identifying and escalating safeguarding concerns relating to unborn babies and children in line with CGL safeguarding policies., • Strong understanding of pregnancy early parenting and child development particularly in the context of substance use., • Knowledge of safeguarding processes for unborn babies infants and young children., • Experience of multi-agency working ideally with social care and health services., • Ability to deliver family-focused interventions and build trust with mothers who may be facing stigma and barriers to support., • Confident in managing sensitive information and maintaining professional boundaries., • 25 days of annual leave plus bank holidays.
