Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Position Overview Under supervision of the Site Medical Director, the Physician Assistant will provide comprehensive, compassionate, and thoughtful care to our patients in the New York City jail system. The Physician Assistant will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services Responsibilities include: • Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated., • Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable., • Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits., • Implement plans for patientcare utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care., • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate., • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented., • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed., • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags)., • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care., • Ensure that all progress notes and orders are signed before the end of the shift., • Respond to emergencies in a timely and professional manner., • Notify the staff MD and Urgicare about 3 hour runs and EMS activation., • Complete special housing rounds when assigned., • Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards., • Maintain clinical competency by participating in all CME and CHS training and in-service requirements., • Maintain your schedule as directed with particular attention to punctuality and timely notification of absences., • Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed., • Complete tasks as delegated by a Site Medical Director or other supervising clinical team member., • Maintain and provide documentation of all required credentials., • Maintain professional attitude and appearance., • Adhere to Occupational Health Services requirements. Minimum Qualifications 1. Valid limited permit to practice as a Physician Assistant issued by the New York State Education Department (NYSED); or, 2. Valid license and current registration to practice as a Physician Assistant issued by the NYSED; and, 3. The position assumes full responsibility and authority in areas delegated. May direct a group of services across multiple jail facilities. Responsibilities include: (Detailed Task) • Serve as the clinical director for Complex Care, providing clinical supervision to clinicians (1 NP, 1 RN) who supporting care delivery and coordination for the more medically complex patients in the jails., • This role encompasses the following responsibilities:, • Establishing clinical criteria for inclusion., • Defining the overarching vision and program objectives., • Attending weekly Complex Care rounds to ensure the effective delivery of care., • Consulting on Complex Care patients and offering clinical guidance and support to front-line providers within jail facilities., • Providing direct patient care at least 50 percent of schedule, • Serving as a clinical point of contact for colleagues from other departments as needed., • Delivering patient education related to complex diagnoses., • Ensuring that patients are assigned to medically appropriate housing areas on Rikers Island., • Providing clinical input and support for medically appropriate discharge plans for patients being discharged to the community, including necessary medications and follow-up appointments., • Ensure safe transitions and coordination of care between the jail and external facilities, including the following tasks:, • Obtaining and reviewing external medical records for comprehensive patient assessment., • Reviewing the daily hospital census of Correctional Health Services (CHS) patients., • Collaborating with the Complex Care Nurse Practitioner to schedule appropriate specialty appointments, track these appointments, and communicate care plans to all relevant parties within CHS to ensure proper execution., • Engaging with hospitalists and inpatient specialty teams when CHS patients are hospitalized and as they transition back to the jail or community., • Arranging expedited admissions to the hospital and ensuring the success of planned admissions for Complex Care patients., • Collaborate with other CHS teams regarding shared patients, with particular emphasis on those involved in Mental Health Discharge Planning and Re-entry and Transition Services., • Participate in clinical release work and support legal advocacy in concert with the Clinical Court Advocacy (CCA) team by:, • Acting as a resource to resolve clinical inquiries from CCA staff (social workers) and providing input regarding patient eligibility for CCA services., • Interfacing with legal representatives to furnish relevant clinical information pertinent to a patient’s legal case., • Composing letters that detail medical conditions for legal documentation., • Assuming on-call responsibilities for critically ill patients, ensuring timely communication with hospital staff, CHS leadership, and legal representatives regarding urgent cases., • Engage in the planning, development, and implementation of programs, policies, procedures, and evaluative monitoring techniques designed to fulfill the objectives established by CHS and Health and Hospitals Corporation (HHC) leadership. Minimum Qualifications 1. Master’s degree from an accredited college or university in Hospital or Health Care Administration, Public Health or a related discipline and five (5) years of high-level responsible experience in Hospital, Business, or Public Administration or a Health Care Administrator in a position of direct responsibility for operations of a major part or all of a, health care setting, including substantial exposure in meeting community health needs; or, 2. Bachelor’s degree from an accredited college or university in disciplines, as listed in “1” above; and six (6)years of high-level experience in areas as described in “1” above.
