Title/Position: Housing Case Manager Status: Non-exempt, hourly, temporary position Department: Empowerment Programs - Housing Reports To: Housing Program Manager Employment Type: Regular, Full-time Location: Los Angeles, CA / On-Site PURPOSE OF POSITION This position requires thorough knowledge and understanding of a human rights, strengths-based approach in serving clients. The Shelter Support Advocate should be familiar with and employ key social work values, including client empowerment, professional boundaries, and self-care. independent decision-making ability, and excellent interpersonal and team-building skills. Full-time, non-exempt position. Some shifts may include holidays to ensure adequate coverage of the shelter program. The Housing Case Manager will provide home-based case management services for survivors of human trafficking in scattered site apartments throughout Los Angeles. The primary goal is to assist survivors of human trafficking in addressing barriers to maintaining permanent housing. ESSENTIAL DUTIES Housing Case Management: Provide victim centered, trauma informed approach to finding safe housing, safety planning, and offering short term housing assistance as well as supportive services to help survivors transition into and remain permanently housed. • Provide strength-based case management to participants which includes crisis intervention, supportive counseling as well as assistance in reducing barriers and promote permanent housing and self-sufficiency;, • Review participant's budget and work on longer-term stabilization goals, which address employment/income, money management strategies, self-development, vocational/educational needs;, • Identify barriers and provide linkages to appropriate resources, follow up, and tracking outcomes;, • Meet with participants in their homes, monthly or more often as appropriate, to assess needs specifically related to housing, or any factors that may jeopardize housing stabilization;, • Report appropriate financial assistance needs, including rental and utility subsides, by required deadlines;, • Provide or coordinate client accompaniment and/ or transportation as needed;, • Manage case files and provide detailed progress notes in hard copy files and in database on a daily basis;, • Manage data entry into all database systems in compliance with Cast Grant Reporting;, • Ensure protection of identify and confidentiality in line with survivor's safety, needs and preferences, • Perform other duties in line with Empowerment Program and/or CAST goals, as identified and assigned;, • Participates in team discussions regarding client progress and lack of progress, with possible solutions to ensure best support for the client's success Able to work independently and respond to crisis and/or emergency situations;, • Ensure that residents' rights and needs are protected;, • Advocate for cultural and linguistic services, as needed;, • Able to work independently and respond to crisis and/or emergency situations;, • Conduct housing assessment to determine participant eligibility, • Assess housing barriers to determine housing needs, • Develop Individualized Housing Plans based on the needs assessments, to guide participants toward long-term permanent housing, • Conduct safety assessment to determine safe location for housing, • Conduct housing inspections to ensure habitability of the housing unit, • Provide mediation and advocacy with landlords on the participant's behalf to develop a workable plan to obtain and/or maintain housing., • Answer hotline calls from CAST's 24/7 hotline, and respond accordingly by:, • Perform client intake, screen new referrals, triage needs, create initial action plans, and coordinate with client, CAST staff, and partners to address emergency needs., • Inform on-call Emergency Response staff of any urgent calls that require immediate follow-up from the hotline., • Make appropriate referrals when space is not available at shelter, or they are not eligible for shelter services., • Document all hotline calls in the database., • Coordinate transportation to the shelter program,, • Coordinate and manage resident move in and orientation; and, • Cover shift for other shelter staff as needed or requested;, • Answer hotline calls from CAST's 24/7 hotline, and respond accordingly by:, • Perform client intake, screen new referrals, triage needs, create initial action plans, and coordinate with client, CAST staff, and partners to address emergency needs., • Inform on-call Emergency Response staff of any urgent calls that require immediate follow-up from the hotline., • Make appropriate referrals when space is not available at shelter, or they are not eligible for shelter services., • Document all hotline calls in the database., • Coordinate transportation to the shelter program,, • Coordinate and manage resident move in and orientation; and, • This position requires working in the field and eligible mileage will be reimbursed according to company policy., • Employees are responsible for accurately tracking and reporting mileage for reimbursement., • Bachelor of Arts or Science degree, or extensive experience and demonstrated skills and aptitude considered in lieu of education., • Shelter or group home experience preferred., • Bilingual preferred (Spanish, Arabic, one or more of the API languages), • Ability to work independently and with minimal supervision; Must be organized, flexible, detail-oriented, and multi-tasking., • Must be a self-starter, able to work in a team environment and manage time effectively., • Strong assessment skills and experience to determine client needs;, • In depth knowledge of issues related to providing services to survivors of human trafficking, human rights and violence against women., • Demonstrated sensitivity to and knowledge of issues involved in working with diverse populations and/or with individuals who have limited English proficiency., • Knowledge of multidisciplinary and multicultural approaches;, • Ability to maintain a positive, cooperative, and professional demeanor with clients, partner organizations, volunteers/interns, staff/board, donors, consultants, and members of the public., • Excellent oral and written communication skills;, • Ability to operate computer systems, including Internet, email, databases, and word processing software (Microsoft Office Suite);
