About the Position We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health team in Fresno, CA. As a Registered Nurse in Home Health, you will be responsible for providing skilled nursing care to patients in their homes, ensuring their safety, comfort, and well-being. You will work closely with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans for each patient, and provide education and support to their families. Your ultimate goal will be to help patients achieve their highest level of independence and quality of life. Coverage Area and Availability: We are seeking a candidate who is available Full time OR Part time Monday-Friday in Fresno and surrounding areas. Optional weekend shifts may be available based on employee interest and organizational needs. Accountabilities and Metrics: Our Registered Nurses provide 5-star patient care and experiences, train and mentor others, and participate in agency programs to enhance clinical quality and ensure compliance. With these accountabilities in mind, the Registered Nurse is capable of meeting the following metrics: • Conduct initial and ongoing assessments of patients' health status and needs, • Develop and implement individualized care plans based on patients' conditions and goals, • Administer medications, treatments, and procedures as ordered by physicians, • Monitor and document patients' progress and response to care, • Provide education and support to patients and their families, • Actively supervises aide and LVNs according to State and Federal guidelines, • Willingly participates in mentoring programs including ride-alongs, aide skills pass off, and continuing education, • Ensures personal employee file is kept up-to-date, • Actively participates in clinical training and committees, • Consistently submits documentation within 24 hours and hits productivity expectations Requirements: • Must possess a current CA Registered Nurse license, • One year of nursing experience (1+ year of home health experience preferred), • Current CPR certificate, • Willing/able to successfully pass background check, • Must have valid driver license, proof of auto insurance, and reliable transportation, • Willing to comply with health screening and immunization requirements Skills: As a Home Health RN, you will utilize your clinical expertise, critical thinking, and problem-solving skills on a daily basis. You will need to be proficient in administering medications, treatments, and procedures, as well as in assessing and documenting patients' health status and progress. Excellent communication and interpersonal skills are essential, as you will be working closely with patients, families, and other healthcare professionals. You will also need to be able to work independently and as part of a team, and be comfortable with using technology and electronic medical records to manage patient care. Why Work With Us?
