Key Responsibilities: • STAR Kids Screening and Assessment, • Individual Service Plan, • Emergency Disaster Plan, • Other forms as applicable: 1500 forms, Service Backup Plan, Electronic Visit Verification, • Assessing the member’s current health status, resource utilization, past and present treatment plan and services, short- and long-term goals, treatment and provider options., • Follow up on requests for items/services, • Speaking to providers, • The RN position on the STAR Kids Assessment Team serves Medicaid recipients from birth through age 20 who have disabilities. This role involves conducting in-person assessments to evaluate members for initial evaluations, annual reassessments, changes in condition, and MDCP Interest List (IL) eligibility., • These assessments help determine the need for services such as Personal Care Services (PCS), Private Duty Nursing (PDN), Durable Medical Equipment (DME), therapy, and the MDCP waiver program., • While visits are conducted in person, documentation can be completed remotely from home., • This position specifically covers members residing in El Paso and Hudspeth counties., • Our team culture is rooted in compassion, collaboration, and a shared commitment to serving children and young adults with disabilities., • We prioritize person-centered care and strive to make every interaction meaningful and respectful., • Team members support one another through open communication, flexibility, and a strong sense of accountability., • While we work independently in the field, we stay connected through regular check-ins, resource sharing, and a collaborative spirit that ensures no one feels alone in their role., • We value professional growth, celebrate successes, and foster an environment where every nurse’s voice matters and every member’s needs are at the heart of what we do., • Each nurse is expected to complete 6 successful visits per week, with assessments submitted within 24 hours and all other documentation finalized within five business days., • While visits are generally scheduled Monday through Friday between 8 a.m. and 5 p.m., compliance with contractual turnaround times is essential., • Because we serve a pediatric population, some visits may occur after 5 p.m., and occasional Saturday appointments may be assigned on a rotating basis if no volunteers are available., • All assessments are conducted in members’ homes, requiring travel within a radius of over 100 miles, including surrounding counties., • Travel time is considered when evaluating weekly productivity, allowing flexibility based on distance and scheduling demands. Key Requirements and Technology Experience: • Skills-Pediatrics, RN , Home Health, • Minimum 4 years of RN experience, • Bilingual, • Pediatric RN Experience, • Bilingual, • Pediatric Experience, • Minimum of 4 years RN experience, • Private Duty Nursing, • Home Health, • School Nurse, • Case Management, • Time management, • Successful visit completion rate, • Form accuracy and completion, • Compliance with turnaround time, • Follow-up efficiency, • Provider communication, • Member centered assessment quality, • Minimum 4 years RN experience, • Short-term roles across multiple organizations (excluding contract work), • Requesting time off within 10 weeks of start date. No time off will be permitted due to strict training schedule, this includes full days or partial hours.
