Job Description Salary: Range $18- 20/h Part-time: 16-24 h/wk. Working days can be arranged with PI during Monday – Friday, 9am-5pm. Position Description: The Department of Oral Biology is seeking motivated individuals with background in veterinary technology, veterinary surgical nursing and/or animal models to join our team at UB. The work will be performed in a fast-paced regenerative dentistry lab. Prior experience working with lab animals (rodents/ferrets/cats) is preferred. We are a multidisciplinary group seeking individuals to join our team who are excited to learn new things and are dedicated to the animals and project goals. Successful applicants are expected to assist with the planning and implementation of current and new animal models by: o Performing diagnostic tests, x-rays and blood or fluid analyses. o Safely restraining animals. Administering medications. o Preparing animals for surgery, including intubation, catheter placement, sterilizing tools, administering anesthesia, monitoring vital signs and anesthesia recovery. o Lab maintenance (handling/disposal of hazardous waste, inventory, ordering supplies). o Maintaining procedure logs and anesthesia records. o Supporting in vivo studies in a strongly collaborative environment. o Supporting investigator during surgical procedures. o Tissue embedding/sectioning/histology (No experience needed. Candidate can be trained). o Microscopy and micro-CT scanning and data analysis (No experience needed. Candidate can be trained). Our lab works closely with scientists to develop new dental regenerative therapies and conduct translational studies, often in collaboration with different centers and lab partners. Solid organizational skills are a must. The candidate should be able to manage multiple detail-oriented tasks with minimal supervision, capable of trouble-shooting and critical thinking, self-motivated, dedicated, and creative. Excellent organizational skills, ability to work well with others, and willingness to learn new lab techniques and follow instructions are key. Minimum Qualifications: o Associate degree in a related field o Current Veterinary Technician license or equivalent experience o At least one year of experience in a veterinary or laboratory setting o Experience in anesthesia of small/mid-size animals and assisting in/performing surgery o Microsoft Office proficiency Preferred Qualifications (if any): o Experience with laboratory techniques (histology sample processing and micro-CT) o Data organization experience in MS Excel o Ability to troubleshoot laboratory protocols o Interest in contributing to publications Breakdown of job duties (by percentages): o Work with investigator on animal procedures, anesthesia, monitoring and follow up care (35%) o Sample processing and analysis for histology, micro-CT (50%). o Maintain lab in working condition (keeping inventory of supplies and reagents, placing of orders, update protocols, arranging maintenance services for lab equipment, etc. (10%). o Meet weekly with main investigator to discuss experiments and lab maintenance (5%). Job Description GLIN IPA seeks a dedicated and proactive Care Manager Registered Nurse (RN) to join our multidisciplinary healthcare team on a Full-Time basis. In this role, the Care Manager RN will provide patient-centered coaching and care coordination in a primary care and telephonic setting, ensuring seamless navigation for patients with complex needs. The Care Manager RN will identify at-risk patients and collaborate with providers to develop and implement processes for preventative care, transitions of care, and chronic disease management. Using defined protocols and clinical expertise, the Care Manager RN will support patients following clinical events, address nursing care needs, and promote improved health outcomes. This role is essential in coordinating patient care, attributed to value-based contracts, and working closely with primary care providers and medical home teams to deliver high-quality, cost-effective care. Duties and Responsibilities (including but not limited to) • Evaluate patients for care management services, determine the appropriate level of care coordination management for the patient, • Complete a comprehensive assessment to identify patient risk and develop a care plan utilizing clinical expertise and judgment to evaluate the need for alternative services as needed, • Work collaboratively with physicians and in-house resources, including pharmacists, registered dieticians, social workers, and other disciplines, to create a person-centered care plan with measurable SMART goals, • Monitor and update the care plan to include progress toward achieving established goals and self-management activities, • Interact with patient, family and providers, and multidisciplinary care team to assess the options of care, including use of benefits and community resources to update the care plan. Utilize developed systems, processes, and initiatives to engage patients in relevant case management activities necessary to promote wellness and care at the right place and time., • Work collaboratively with physicians and in-house resources, including pharmacists, registered dieticians, social workers, and other disciplines, to support patient adherence to medical plan of care., • Supervise and act as a resource for non-clinical staff (i.e., care coordinators and social workers)., • Verify that appropriate home care, hospice care, and other ancillary services (DME, infusion services, etc.) are in place and are being delivered as directed by the care team, • Coordinate necessary referrals and authorizations within care management areas, • Facilitate the information flow between the hospital, long-term care, specialists and home health representatives and the care team, • Use available data and work with physicians and office staff to help identify high-risk, high-need, and potentially high-cost patients, • Coordinate care and communicate with multiple providers, internal and external to the practice., • Identify and utilize cultural and community resources and align with the patient’s cultural preferences as much as possible, • Ensure that members are screened for behavioral health issues, such as depression and substance abuse, and that they are receiving the necessary interventions and support., • Facilitate any necessary follow-up behavioral health needs with local behavioral health providers., • Attend required training and collaboration sessions [i.e., learning sessions, care management meetings, and practice team meetings] as scheduled., • Provide and facilitate open communication with physicians and office staff regarding patient status., • Obtain records from other physicians/labs/diagnostic centers as requested by the physicians and as needed for care coordination efforts., • Build collaborative relationships with the internal GLIN IPA population health team, participating providers, and community resources., • New York State Licensed RN, • Bachelor’s degree in nursing preferred; Associate degree in nursing is minimum requirement., • 2-3 years of experience in acute inpatient care, rehabilitation, sub-acute care, skilled nursing facilities, home care, ambulatory care management, or managed health plans., • Preferred: Certified Case Management (CCM) certification, • Principles of utilization management; care management principles; basic knowledge of health plan contracts and benefit eligibility requirements; Hospital structures, Managed Care and payment systems, • Timely and accurate documentation of day-to-day activities in designated technology platform, • Adaptable to new technologies and software, • Proficiency in EMR system(s), Outlook and data entry experience preferred, • Basic PC skills (MS Word/Outlook/PPT/Excel) We offer an outstanding benefits package including health, dental, 401K, vacation, PTO, and a great working environment. POSITION OVERVIEW Under supervision of the Clinical Supervisor the CASAC Counselor will provide counseling services in integrated outpatient program. The CASAC Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC Counselor is also responsible for referrals and coordination of care in consultation with other professionals. KEY ESSENTIAL FUNCTIONS • Intervention and employing a range of treatment modalities to include person centered evidence-based treatment approaches to include but not limited to motivational interviewing, stages of change, trauma informed care, anger management, medication assisted treatment, dialectical behavioral therapy, cognitive behavioral therapy and contingency management., • Ability to perform SUD and mental health screenings and intakes, orientation, SUD assessment, SUD diagnostic Impression, level of care as determined by LOCADTR, treatment planning, individual/group counseling, family interventions, psychoeducation, case management, crisis intervention, client education, referral, reporting and recordkeeping, care coordination and administrative supervision with regular supervision., • Participates in supervision regularly., • Develops and conducts group and individual counseling on a regular scheduled basis to caseload clients within the integrated outpatient program., • Obtain random observed urine samples according to established protocol., • Orients clients about program goals, objectives, rules and regulations, client’s obligations, right’s and program operations, • Knowledge of the obligation to engage in prevention as well as treatment techniques., • Maintains and updates referral services and resources that may be useful to our client population; makes referral when necessary and appropriate., • Ability to demonstrate proficiency in English including the ability to speak, write, comprehend aurally, and read at a level necessary to perform as a CASAC Counselor., • Participates in outreach functions to reengage client lost to contact and maintain census capacity., • Ability to complete all client progress update reports in a professional and timely manner to ensure coordination of care with client linkages to include the criminal justice system and entities., • Participates in program meetings and required trainings., • High School Diploma or GED., • Associate Degree preferred., • Valid unrestricted OASAS certification as a CASAC., • Minimum of three (3) years working in a Substance Use and Human Service setting., • Must obtain Mandated reporter (2 hours) training/certificate prior to hire date., • Must obtain training/certification within thirty (30) days of hire., • Domestic Violence, • Infection Control BASIC (non-medical/nursing staff), • HIV, • Child Abuse, • Tobacco Cessation, • Must obtain training/certification during in-house orientation., • Supporting Recovery with Medications Addictions Treatment (MAT), • Must obtain SUD training/certificate within thirty (30) days of hire (if applicable), • Screening, Brief Intervention and Referral to Treatment, • CASAC Canon of Ethics (6 hours), • Confidentiality related to 42CFR (3 Hours), • Valid unrestricted OASAS certification as a CASAC., • Excellent organizational, communication and time management skills, • Ability to work on a strong team of professionals in a culturally diverse environment.
