Job DescriptionDescription: The Targeted Case Manager – D&A Specialist will provide services to participants, living in the community with persistent mental health and drug and alcohol challenges. After assessing needs, the Drug and Alcohol Specialist helps the participant with obtaining resources and services in the community, in a partnership that focuses on recovery and the achievement of the participant’s goals for community living. The Drug and Alcohol Specialist also is instrumental in assisting homeless individuals to locate adequate shelter/residence. The Drug and Alcohol Specialist also engages individuals who are incarcerated acting as a community link in reestablishing a productive life within the community once released. The D&A Case Manager is a full time position, Monday - Friday, 8:30 AM - 5:00 PM (with the opportunity to create a flexible schedule with manager's approval). This is a hybrid work from home position, with required in-person participant visits and office days and occasional work from home options. The rate of pay for this position will be an exact rate of $24.00/hour. As an MHP full-time employee, you will have access to the following benefits: • Generous Paid Time Off (Up to 39.5 paid days off per year!), • Medical, Dental & Vision Insurance (Effective after first month!!), • No Cost Life and Disability Insurance, • Flexible Spending Accounts, • No Cost Employee Assistance Program, • Retirement Plan with Annual Discretionary Employer Contribution, • Employee Referral Bonuses, • Tuition Reimbursement, • Federal Student Loan Forgiveness Program WHAT YOU'LL DO • Provides assessment of substance use to determine the need for treatment and other services including specialized housing and other recovery supports in the community., • Develop individualized plans with the participant and other members of the case management team to address substance use treatment needs., • Links the participant with the appropriate substance and alcohol use services and monitors these services with other case management team members., • Provides consultation and support to case managers in identifying and engaging individuals who are experiencing substance use issues., • Supports individuals in the community with obtaining necessary substance and alcohol use treatment., • Facilitates a weekly Drug and Alcohol Recovery group., • Continually engages ambivalent individuals to assess their readiness for treatment or services, assesses their stage of change and actively works with them utilizing motivational interviewing., • Conduct outreach to locate individuals on their caseload who may be missing and have a concern about substance use., • Collaborates with participants to identify, develop, and access resources and supports to increase their success in self-sufficiency, community integration, and community inclusion., • Collaborates with participants to assess their hopes, strengths, accomplishments, and challenges and create goals; assists in identifying support and arranging services and resources to achieve goals., • Supports, teaches and models recovery and recovery tools and models personal responsibility, self-advocacy, and hopefulness., • Coordinates and/or chooses relevant supports to support participants in developing their self-directed recovery and crisis plans; facilitates the participants' self-review of their progress upon each encounter., • Provides crisis intervention and de-escalation to participants as necessary., • Obtains necessary information during the intake process and collaboratively documents participant progress in MHP’s electronic health records system at the time of service. Credible; ensures that all data is correct by verifying the information with the participant., • Supports and assists participants in the community when necessary and ensures participants safety when participating in events, visits, and other interactions., • Complies with all other established quality and regulatory standards, including supervision and training requirements., • Applies general knowledge of Recovery services to complete small projects or conduct a series of tasks with a limited degree of supervision., • Creates weekly schedules, two weeks in advance, that meet program productivity standards and supervision., • Maintains compliance with all MHP policies and procedures. Reports known conduct taken by any MHP employee (including yourself) who violates any MHP policy or law on which you have been trained or should know of, such as laws criminalizing theft or violent crimes, and reports the matter to management or to the Director – Quality Improvement & Compliance and/or Human Resources., • Bachelor’s degree with major course work in one of the following areas: sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling or education; or be a registered nurse whose duties will be limited to TCM responsibilities only., • Minimum of 2 years of experience in a behavioral health work environment with a focus in substance use., • Must know the local community and statewide substance use treatment facilities and other resources., • Must know treatment techniques and procedures as applied to a substance or alcohol use., • Must have a BHTEN certification or be able to obtain one within one year of hire date., • Must possess a valid driver’s license., • Must be able to qualify for and obtain a prison ID., • Must possess basic computer skills to perform job duties, including desktop computing, e-mail, timesheet management, electronic health records, internet research, etc., and the ability to learn software applications relevant to your position., • Must have access to a car to use for work on a daily basis., • Must have automobile insurance coverage at the state requirement levels and current state registration., • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices., • Must be able to work under pressure and meet deadlines and billable unit requirements while maintaining a positive attitude and providing exemplary customer service., • Strong skill sets in oral and written communication, cultural competence, interpersonal relationships, and teamwork, attention to detail, customer service, time management, autonomy, problem-solving, and organization.

Job Description Program: Supportive Services for Veteran Families Location: Philadelphia, PA Reports to: Program Manager Status: Full-Time, Non-Exempt (hourly) Summary: This position provides case management services to approximately 15-30 veteran households, supporting the development and achievement of individual housing stability plans. Ensures client service delivery through adherence to Veteran Affairs (VA) and agency requirements. Coulter Street POSITION SUMMARY: The Truancy Intervention and Prevention Services (TIPS) Supervisor is an integral member of the TIPS program. The Program Supervisor reports to the TIPS Director and deputizes on their behalf during absences. The TIPS Supervisor must communicate all concerns and issues upwards to the TIPS Director and in their absence to the Executive Director. The TIPS Supervisor will work with TIPS Director and a team of Truancy Case Managers that are assigned to specific schools in the Northwest section of the City of Philadelphia. The program works in collaboration with the Philadelphia School District and the Philadelphia’s Department of Human Services (DHS), Community Umbrella Agencies (CUA), and other stakeholders with the mission to help children and youth improve their school attendance and address the issues that are impacting their ability to attain satisfactory school attendance and school achievement. EDUCATION/CREDENTIALS/ EXPERIENCE: A Master Degree in Social Work or related field with at least two years of supervisory and specific social service work experience. (I.e. related fields are social services, sociology, psychology, theology, education, and public administration) is required. SKILLS: • The TIPS Supervisor must have the knowledge, skills and abilities to motivate and supervise a team of professionals and clients from diverse backgrounds., • Must possess the skills necessary to be a dynamic and engaging public speaker., • Must be able to network and have the capacity to build partnerships and foster community linkages that will help families eliminate barriers to truancy and prevent children from being placed out of their home., • Supervisor must have excellent and impeccable writing skills and communication skills., • Supervisor must possess a positive and helpful disposition and have the ability to adapt to change and motivate others to do the same., • Must have the ability and experience to train new staff, understand the program standards, the departmental data base and other systems associated with the program., • Demonstrated ability to effectively work and communicate with diverse staff with a strong commitment to supporting a team environment., • Ability to use a computer to send and receive written correspondence., • Ability to create spreadsheets and other documents using Microsoft Office software such as Excel., • Ability to work collaboratively with other professionals., • Ability to lead and co-facilitate focus group sessions., • Understanding of educational, child welfare, and judicial systems and the knowledge, skills, and ability to navigate these systems successfully., • Strong organizational skills with the ability to multi-task without compromising quality. POSITION RESPONSIBILITIES: • The TIPS Supervisor must provide bi-weekly supervision to Case Managers. Supervision must be documented in a supervisory log book to ensure that home visits are made, all case management services, court mandates, and database entries are in compliance. Troubleshoot any barriers and provide guidance and follow-up. Staff supervision is subject to review by DHS., • The TIPS Supervisor will lead the Truancy Case Managers in resolving issues and barriers that families face and help eliminate the barriers by providing intervention and advocacy services., • The TIPS Supervisor is responsible in ensuring that the comprehensive plan is developed in collaboration with the youth, parent, and others involved with the case, and that community linkages are in place prior to the case being discharged., • The TIPS Supervisor must attend all truancy meetings and trainings., • The TIPS Supervisor will monitor updates and complete a detailed CBPS database report every two weeks to ensure DHS & Agency Compliance., • The TIPS Supervisor is responsible in completing a detailed file review report of each case manager’s case load, every month to ensure DHS & Agency Compliance., • The TIPS Supervisor is responsible to ensure that all TIPS responsibilities regarding court are maintained –court reports reviewed and signed, and the timely submission of court reports to DHS Liaison., • The TIPS Supervisor is responsible to ensure that all CM’s to appear in Regional Court by no later than 8:30am with their detailed court sheet completed with the TCM Supervisor’s signature (as requested in court services section) to submit for the Hearing Officer to review. The Supervisor will attend court hearings and provide additional support to case managers while at court., • The TIPS Supervisor will co-chair mandated bi-weekly supervisory unit meetings as well as all other program meetings and case conferences, including multi-disciplinary conferences. Sessions must be documented in a supervisory log with the agenda, goals, and outcomes recorded., • The TIPS Supervisor must model positive, professional, and ethical work habits and have high standards of staff., • The TIPS Supervisor will assist the Program Director for expeditiously assigning cases to all staff within the department and to assure that cases open immediately. No delay is acceptable., • The TIPS Supervisor is responsible for the program’s case management service coordination and monitoring the data base to ensure that all documentation is up to date on a weekly basis., • The TIPS Supervisor will evaluate their employees’ performance by completing the necessary evaluation tools., • The TIPS Supervisor is responsible for ensuring that each truancy case manager is providing the families with the expected number of home visits and school visits., • When issues arise with a resistant family, the TIPS Supervisor will intervene and go on home visits with the assigned case manager to help resolve the problem., • The TIPS Supervisor will provide direct service to families when needed, inclusive when case managers are out on PTO (vacation, personal or sick time), • The TIPS Supervisor is responsible for conducting bi-weekly supervision sessions directly and through a record of supervision to be kept for audits., • The TIPS Supervisor will ensure compliance with all JJC’s policies and procedures, including confidentiality and reporting suspected abuse in accordance with CPSL, 23 Pa.C.S. § § 6301—6385, and JJC policies and procedures., • The TIPS Supervisor will ensure that all court mandated transfers to another Agency are reviewed (database and documentation) and sent by two days after court date., • Implements all documentation requirements and ensures that reports are complete, thorough, timely, and follows the prescribed format. PHYSICAL DEMANDS/WORK ENVIRONMENT: • The ability and willingness to adjust work hours, and days, in accord with program and client need., • The use of a currently inspected car, including the possession of current automobile insurance and driver's license.
