Palliative Care Nurse (RN) The registered nurse is a member of the core interdisciplinary group whose primary function is to assess physical, medical, psychosocial, emotional and spiritual needs of the seriously ill patient and family. The BSC also coordinates and consults with other service providers regarding behavior plans and interventions and discussion with the child’s, youth’s or young adult’s treatment team about the behavioral interventions that should be used and when they should be used. BSC services can be used to address behaviors in the school, home, or any other place where a child, youth or young adult needs behavior support (i.e.- across settings) and to learn strategies to better succeed. POSITION RESPONSIBILITIES • Collaborate with other members of the treatment team in order to design and direct the implementation of an individualized treatment plan (ITP) to be implemented across settings of the child., • Develop a trusting partnership with family members and other team members., • Ensure clinical interventions are properly formulated in order to design, implement, monitor and adjust the behavioral interventions for the child to ensure progress can be made over time and in accordance with the ITP., • Observe child in the home/school/community setting as part of the initial assessment in preparation for designing the individualized treatment plan and implementation of the written order. Functional Behavior Assessments (FBA) will also be needed (if properly trained with certificate)., • Design data collection tools to be used by Behavior Health Technician (BHT) and/or other team members to accurately assess effectiveness of established therapeutic interventions., • Develop Individualized Treatment Plans (ITP) to instruct other members of the therapeutic team (including teacher, teacher’s aides, other school personnel, family, BHT, MT and/or other designated individuals providing direct service or supports to the child) in their roles and responsibilities in implementing the established interventions., • Complete and submit accurate and timely documentation which includes but is not limited to: Individualized Treatment Plans (ITP), Encounter Forms, Progress Notes, Progress Summaries, Addendums, Crisis/Safety Plans, FBAs, Assessments, Structured Tools, Incident Reports, ITM summaries, Discharge Summaries, etc. by assigned deadlines., • Provide individual mentoring and supervision of all assigned BHT staff, • Conduct Functional Behavior Assessments (if properly trained with certificate) –FBA for BSC-ASD/BC-A requires a professional license plus FBA certification from DPW or a professional license plus BCBA. For non ASD FBA’s – the FBA certification is sufficient., • Supervision Requirements:, • BSC clinicians who provide services must receive at least one hour of individual face-to-face supervision per month which includes oversight of the following: The interventions being implemented, the child’s, youth’s, or young adult’s progress towards the goals of the ITP, consideration of adjustments needed to the ITP and the staff person’s skill in implementing the interventions in the ITP., • If the individual who provides BSC services supervises BHT treatment team members, the BSC must receive the following supervision in addition to the above: One hour of face-to-face supervision per month which must include a discussion of the BHT services being provided., • On-Site Supervision Requirement: BSC clinicians who provide services must receive 30 minutes of direct observation of the services being provided every 6 months., • BSC's must complete at least 40 hours of training within the first year and 20 hours each year thereafter. Mobile therapy services consist of individual therapy, family therapy, development and revision of the ITP (as needed), assistance with crisis stabilization and assistance with addressing problems the child, youth or young adult has encountered. Review all advertising sources, including property website, on a weekly basis and immediately communicate to the Community Manager any discrepancies or areas for improvement. Conduct market surveys monthly and provide trend reports. Shop competition and be aware of neighborhood market conditions. The TIPS program will provide a comprehensive, intensive early intervention services for youth exhibiting truancy behaviors. The programming builds upon youths’ and families’ resiliency and sense of self-worth for positive outcomes and redirection, strongly networking with community resources. Assignments are based on a 40-hour week. Weekly schedules are developed based upon the caseload needs. There are no traditional office hours. Case managers will work primarily in the afternoon and evenings, specifically during the school year and at weekends to provide maximum involvement with the family and the identified clients. There may need to be a need to start early to help a participant get to school. The case manager will be placed on an on-call roster and will be responsible for providing emergency assistance to clients assigned in the program 24 hours a day while on duty. Case longevity is determined by court and or DHS personnel., • The case manager’s role is to be able to report to the court that identified issues have been resolved through a collaborative effort with school personnel and community resources., • Case manager must meet with the supervisor bi-weekly for supervision., • Case manager must meet with the family and see all children at least twice each month, and coordinate work with others involved with the family. DHS contact stipulations must be adhered to., • Case manager will form an “after hours on call” roster, with alternating back up of the program director. In this way, families can access our staff round-the-clock., • Case manager will ensure that services and counseling are commensurate with identified issues, and may need to be increased beyond minimum standards., • Following assessment of family’s identified needs, case manager will identify community resources, and work collaboratively with others to assist families with linking them to the appropriate community resources., • Case manager is responsible for ensuring that school age children receive the appropriate services that meet their educational needs., • Case manager will ensure that he/she keeps accurate documentation specifically all unsuccessful efforts to maintain the expected level of intensity and monitoring. All documentation and communication will be directed to the assigned DHS worker or court personnel via e-mail correspondence, telephone calls, and with hard copies to be kept in client’s file., • Case manager will facilitate the family’s transportation for appointments with other service providers by providing directions, tokens, as needed, and accompanying on public transportation or driving them to appointments and meetings in their own vehicles., • Case manager must attend team sessions to determine and monitor service appropriateness for each youth and to inform the team of the client’s adherence to orders of the Court and to the program., • Case manager must attend all mandatory TIPS trainings and activities hosted by the Department of Human Services or designee., • A full case record is maintained in accord with performance standards, with monthly supervisor case review., • As this is a new program, this job description will change over time, and new duties may be added and existing duties modified.
