Are you an experienced LCSW passionate about supporting holistic recovery while delivering goal-driven care in a collaborative, autonomous environment? 1:1 patient care focused on meaningful, real-world independence and function • Therapy delivered in home and community environments, • Flexibility to create your own schedule - no weekends or holidays, • Opportunity to apply and grow your neuro rehab expertise in adult populations, • Collaborate with an interdisciplinary team while maintaining clinical autonomy, • Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families, • Sets measurable objectives that are formulated in conjunction with the rehab team, • Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems, • Instructs/counsels' patients and families in treating and coping with social and emotional response connected with illnesses, • Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs, • Master's Degree in Clinical Social Work (LCSW), • Current Licensed Clinical Social Worker Licensure in the state of practice, • Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis, • Current BLS (Basic Life Support) First Aid and CPR certification required, • Ability to lift 50 pounds, • Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures, • Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
