Friday, 8am ??? 5pm (some travel required for field visits), • Work Location: must live in Corpus Christi/Nueces SDA for field visits 3???4 times/week, • Perform 3???4 in-home visits with members per week to complete Waiver Upgrade Assessments., • Complete documentation and assist in coordinating services for members (e.g., contacting MD offices, clinics, DME companies, and other providers to set up appointments, services, supplies, and equipment)., • Travel averages 1???2 hours one way; in-home visits typically take 2???3 hours plus travel time., • Collaborate with RN???s, Social Workers, and Care Management Support Coordinators through Microsoft Teams, phone, email, and video., • Participate in remote training, with some in-person training required for field visits.
