Risk Management: · Evaluate the appropriateness of services and ensure the implementation of the plan of care through regular assessments. · Escalate complex cases to the Supervisor and report critical incidents. · Attend Behavior Support Plan (BSP) meetings. Additional Duties: · Assist individuals/LRP in choosing service providers, ensuring objectivity. · Utilize Admission, Discharge, and Transfer (ADT) information to support members admitted, transferred, or discharged from a facility promptly. · Collaborate with the care team and service providers to develop plans reflecting the individual’s needs and desired life goals. · Complete all other responsibilities as assigned by the supervisor. · Visiting members at their living arrangements, such as their own home, a relative's home, a care facility, or any other location, is essential and required to ensure the delivery of high-quality care. The Medical Assistant will assist the health care and administrative team of the medical office, provide optimum health care to patients and prepare them appropriately for the examination with the provider. Essential Duties and Responsibilities • Perform basic prep according to Advance Community Health guidelines., • Assist RN or provider with patient triage, to include telephone triage., • Complete information gathering segment of triage with specified patient population., • Refer all problems and urgent needs to RN or provider and conduct patient follow-up post encounter., • Make follow-up calls to patients as directed by the provider., • Manage provider messages and test result reports in accordance with established site procedure., • Manage prescription and refill medication request paperwork under the direction of the provider and Schedule appointments., • Must have certification from a medical assistant program or military medical corpsman program., • 1-2 of experience in a medical setting., • A high school diploma, bachelor's degree, or equivalent.
