About the Role The Home Study Case Worker plays a critical role in evaluating a potential sponsor’s ability to provide a safe, stable, and supportive environment for an unaccompanied child prior to reunification. This includes conducting in-depth, trauma-informed home visits; verifying information gathered by the care provider; and assessing all potential risks, protective factors, and support needs. The HS Case Worker collaborates closely with the HS Supervisor and relevant stakeholders including the care provider’s case manager and clinician, the sponsor, and, when necessary, family members of the sponsor or minor to ensure the child’s safety and well-being. Home Study services are provided throughout the New York City tri-state area and nationally. This role requires frequent travel across states and may include overnight stays to complete in-person assessments as needed. What You’ll Do • Initiate contact with the referring care provider (e.g., case manager, clinician) upon case activation to review the child’s background and coordinate information-sharing, • Review the child’s history to guide a trauma-responsive approach and ensure age-appropriate, culturally sensitive engagement throughout the assessment process., • • Provide Home Study services by conducting in-home assessments and interviews with sponsors and household members to evaluate safety, caregiver capacity, and readiness for reunification., • • Identify and flag any red Flags or safety concerns (e.g., abuse, neglect, trafficking, special needs, or risk of exploitation) and maintain high-level communication with the HS Supervisor to discuss concerns, timelines, or case support needs., • • Educate sponsors on reunification responsibilities, including providing a safe and stable home, supporting access to education and healthcare, complying with immigration obligations, and fulfilling the Sponsor Care Agreement. Close out cases in accordance with ORR and program guidelines, including timely updates to the UC Portal and internal trackers., • Report any suspected abuse, neglect, or trafficking in accordance with state-specific mandated reporting laws and ORR procedures., • Be available and Flexible to accommodate evening, after-hours, and weekend work as needed to meet case deadlines and report requirements., • Complete all required ORR and agency trainings and remain current on child welfare policies, best practices, and program protocols, • Participate in regular supervision, case staffing, and team meetings, both virtually and in person., • Must have excellent verbal and written communication skills, including the ability to maintain clear, consistent, and timely communication with supervisors. MercyFirst’s Qualified Residential Treatment Program (Group Home) provides 24-hour residential care and supportive services for youth with a history of trauma and the presence of complex needs. Three agency-operated group homes provide youth with structure and supervision within community-level settings designed to provide evidence-based treatment to promote emotional and physical safety and stability, practice positive coping skills, and build healthy relationships, as well as case management services to support discharge planning through family reunification or independent housing, and youth development services to foster growth and competency in daily life skills . POSITION SUMMARY: The Aftercare worker for the QRTP program will be responsible for assessment, engagement and case management services for clients and families towards successful transition out of QRTP placement. This qualified candidate will be a part of a multi-disciplinary team and work alongside the QRTP caseplanner and will refer guardian for supportive services, including medical, clinical and concrete services. Upon the youth’ transition out of the QRTP program, the Aftercare Caseworker will provide the youth and their family with continued casework support up until the youth's placement order expires, to ensure successful reintegration into the community and in the home. Case planner will present the program’s efforts in legal proceedings, ACS/OCFS meetings and other interagency conferences. The qualified candidate will complete the required progress notes, assessments, Psychosocial reports, FASPs, court reports, etc within timely mandates. Specialized Adoption Recruiter The Wendy’s Wonderful Kids recruiter will connect youth who wait for permanent homes with families who wish to adopt by implementing and coordinating effective recruitment, matching and support services for a caseload of 12-15 youth in active recruitment status. The recruiter will use the evidence-based, child-focused recruitment model to identify legally permanent resources for youth in the county’s permanent custody or APPLA status who are over the age of nine, have special needs and/or are part of a sibling group. Principle Responsibilities: • Work in partnership with agency staff to identify youth for referrals to the WWK program, • Review the case record, interview the youth, caseworker, other agency staff with knowledge of the case and any available family members and/or other supportive connections to assist in the recruitment process, • Utilize internet search tools to identify resources within the youth’s network, • Develop an initial written adoption readiness assessment describing the youth’s understanding and feelings about adoption, • Create child-focused recruitment plans for the youth, • Meet with the youth on a monthly basis, • Communicate with the caseworker on a monthly basis to review the progress of the recruitment plan, • Meet with family members and supportive connections to discuss the progress of locating permanent resources for the youth, • Once matched, work with the caseworker, youth and the adoptive resource to ensure pre-placement visits are implemented, • Assist with the development and coordination of the transition plan with the youth and matched family. Work with the youth and family during the transition process to assist with placement support, • Work in conjunction with the caseworker to ensure that the adoptive family is working towards adoption finalization, • Attend educational opportunities provided by the Dave Thomas Foundation for Adoption including, but not limited to: WWK classroom training, database training, webinars, and the Wendy’s Wonderful Kids Summit hosted by the Foundation, • Qualifications for the WWK recruiter are per the agency’s policies. Job Description Split Shift Supervisor for Youth reception Center (YRC) Reports to: Program Manager Job Summary: The split shift supervisor is responsible for assuring staff’s compliance with daily routines and structure of the facility during the shift of duty and responsible for maintaining agency standards of quality of care and service provision for the children/youth residing in the facility. The Shift Supervisor will supervise all child care workers, their deployment and activities, uphold the daily programming, routines, discipline, program structure and tone. Summary Job Description: Performs professional-level casework duties in providing services for children, biological, and foster parents or planning resources under the care of the agency; performs related work as required in providing permanency planning to expedite the safe discharge of children from the foster care system. Depending on the area of assignment, the employee may be required to use his or her own vehicle in carrying out field assignments. Work involves gathering information and providing services to meet clients' needs, focusing on permanency planning; dealing with governmental and community agencies; counseling individuals, and transmitting information on cases through the completion of agency and governmental documents. Work is reviewed through conferences with a Casework Supervisor. Principle Responsibilities: • Investigates cases referred to the agency to determine need and proper method of care; interviews the individual, the family, community and governmental agency personnel, and any other resource person who might provide information necessary for a proper psycho-social diagnostic evaluation and the preparation of a treatment plan; works with other divisions of the agency to evaluate the progress of cases and to carry out the treatment plan., • Works with public welfare agencies; the courts, and others to assist individuals with financial, legal, medical, and personal problems., • Attends periodic staff conferences; confers with supervisors weekly and as needed; prepares dictation, reports, and correspondence on assigned cases and maintains records of the status of each case through established channels., • Expected to respond to case emergencies after hours or on weekends., • All progress notes and FASP will be recorded online in Connections by the caseworker., • Every action or event must be recorded by the caseworker no later than 72 hours after the action or event and must be forwarded to the immediate supervisor for review. The corrections must be submitted to the Supervisor, no later than 3 days after the supervisor requested the corrections. We believe every person has the right to be safe, empowered and free from violence. In this role working Monday to Friday, 10 am to 6 pm (3 days) and 12 pm to 8 pm (2 days) and some Saturdays as needed, you will be provide case management to clients of the Rapid Rehousing and Transitional Housing programs. Responsibilities: • Complete psychosocial intake with new clients, work with clients to establish goal plans and monitor goals., • Provide de-escalation to clients during crisis and provide on-call crisis support as needed to the clients and team., • Work with clients one-on-one to provide coaching in the areas of independent living/self-sufficiency skills including but not limited to: budgeting, financial planning, shopping, meal planning, personal organization, etc. Facilitate or co-facilitate groups with supervisor or other team members., • Meet with residents weekly and/or bi-weekly in-person, in the community or office, via Zoom or phone, as deemed appropriate by the needs of the clients and approval of supervisor., • Perform advocacy work as needed (i.e. legal, police, medical, etc.) in correlation with the prescribed goal plan.
