When assisting with medication administration, carefully watch each resident, assuring that: The right resident is taking the right medication in the right dose at the right time by the right route., • Medication Storage: Assure that all medication is stored appropriately in original labeled container, in accordance with safety requirements refrigerated medications stored in refrigerator, oral medications stored separately from topical medications, controlled substances stored in double locked box, medication room always locked when not occupied by staff, medications in resident rooms stored according to DOH/OMH regulations., • Medication Orders: Assure that no medication is dispensed without written orders from the prescribing practitioner., • Documentation of Medication Orders: Assure that the Medication Administration Register (MAR, or Med Log) accurately reflects the written orders of the resident’s prescribing practitioner(s). Specifically, in the event less than 3 day supply of medication, order refill, send in new prescription, or refer problem immediately to the assigned caseworker., • Medication Changes: Changes are made only when there is a prescribing practitioner’s written order, or when there is a Confirmation of Telephone Order in place. If there is a new medication prescribed, assure that the pharmacy gets the prescription and delivers new medication. If there is a medication with dosage change and the actual pill changes, assure pharmacy gets the prescription and delivers the new medication (making sure to pull the supply of the previous dosage). If there is a medication with a dosage change, but no new prescription (for example, the doctor changes the order from 2 pills twice a day, to one pill twice a day), the MAR is changed to reflect the change and assure that the prescription container is tagged with the change., • Incontinence Supplies: Assist residents with monitoring and management of supplies used to assist with incontinence issues. Work closely with outside providers, insurance companies, pharmacy, Fleming House Social Service and PCA staff to ensure that residents are receiving proper amounts of supplies at appropriate times., • Medication Updates: Create/collect/ maintain all Medication Update forms related to resident appointments with outside providers. Assure that all information related to medical appointments is properly disseminated to appropriate caseworker. Our program recognizes that all children in foster care have experienced trauma, that their needs may change over time, and that their strengths must be supported so they can reach their full potential. This position is conveniently located in S. Ozone Park, Queens, close to Mass Transit. POSITION SUMMARY: A Caseworker works with children and youth who reside in a foster home as well as family members and other adults serving as supports to the young person. In addition, we support our foster parents, who can either be relatives or could be individuals who are not related to the children. Caseworkers work as a team with Socio-Therapists to provide supports to the children, the birth families and the foster parents to ensure that every child is making progress towards permanency. REQUIRED QUALIFICATIONS: • Bachelors in social work or related field, • 1 year of related work experience in serving children and/or families, • Knowledge of and/or training in child development stages, • Knowledge of and/or training in family dynamics, • Excellent communication skills (verbal and written), • Proficiency in Microsoft Word and Outlook, • Ability to engage with clients, • A valid driver’s license is strongly preferred, • Understanding and willingness to commit to the agency’s Mission, core values, Sanctuary Commitments and Diversity, Equity, Inclusion and Belonging (DEIB). Program Description: Under general supervision of a Director, the Case Planner Supervisor is responsible for providing professional and administrative guidance to a small unit of up to 4 case planners in providing comprehensive casework and building partnerships with each family of the children in the care of Little Flower. The Case Planner Supervisor oversees the development of action plans with each Case Planner to address all relevant concerns of the families that lead to the foster care placement of their children. The action plans are developed to create meaningful change so permanent solutions for the children and their families can be achieved. The Case Planner Supervisor assigns and reviews cases; trains case planners; interprets rules and regulations and assists in resolving casework problems. Performs all relevant work as assigned. Principle Responsibilities: • Supervises a unit at a maximum of 4 case planners and conducts individual and group conferences with casework staff to complete assignments and to guide, train, and develop casework skills., • Evaluates staff performance, reviews case records, and maintains appropriate records and controls., • Conduct structured supervision consistently around both issues of compliance and permanency timelines to give workers feedback, provide motivation, insight, and constructive criticism., • Ability to step into difficult or hostile situations, model behaviors to handle difficult situations, and provide mediation., • Resolves workers’ concerns and assist struggling workers by creating corrective action plans or other developmental plans when necessary for changes to occur., • Provide overall structure and organization to unit to develop the skill set of staff, so they can create positive solutions for families., • Act as an effective and professional liaison between all internal staff and external organizations., • Ensure coordination of all specialists’ contacts into the case plan and foster good teamwork among case planners and specialists of other staff with whom they share a case., • In absence of a caseworker, supervisor is ultimately responsible for covering the caseload until a replacement is identified., • May be required to cover for another casework supervisor in the event of their absence., • Ensure compliance with all rules and regulations of the agency, and as well as those of all local, state, federal, and other governing bodies., • Maintain a time oriented, deadline focused approach to documentation for your unit, including progress notes, psychosocials, and FASP’s., • Aides staff to locate and engage extended support networks for families served by unit., • Create a clear set of job expectations for all staff within the unit and hold staff accountable for those expectations through weekly supervision of each staff member. Summary Job Description: Performs professional-level casework duties in providing services for children, biological, and foster parents or planning resources under the care of the agency; performs related work as required in providing permanency planning to expedite the safe discharge of children from the foster care system. Performs both field and office work in handling a caseload for the agency. Depending on the area of assignment, the employee may be required to use his or her own vehicle in carrying out field assignments. Work involves gathering information and providing services to meet clients' needs, focusing on permanency planning; dealing with governmental and community agencies; counseling individuals, and transmitting information on cases through the completion of agency and governmental documents. Work is reviewed through conferences with a Casework Supervisor. Principle Responsibilities: • Investigates cases referred to the agency to determine need and proper method of care; interviews the individual, the family, community and governmental agency personnel, and any other resource person who might provide information necessary for a proper psycho-social diagnostic evaluation and the preparation of a treatment plan; works with other divisions of the agency to evaluate the progress of cases and to carry out the treatment plan., • Works with public welfare agencies; the courts, and others to assist individuals with financial, legal, medical, and personal problems., • Attends periodic staff conferences; confers with supervisors weekly and as needed; prepares dictation, reports, and correspondence on assigned cases and maintains records of the status of each case through established channels., • Expected to respond to case emergencies after hours or on weekends., • All progress notes and FASP will be recorded online in Connections by the caseworker., • Every action or event must be recorded by the caseworker no later than 72 hours after the action or event and must be forwarded to the immediate supervisor for review. Actions/events include, but are not limited to: 72 Hour Conference; 30 Day Conference; Birth Parent/Child Contact; Birth Parent/Caseworker Contact; Child Care Worker/Caseworker Contact (RTC); Collateral i.e.: DLS Attorney, Law Guardian, Therapist; ACS Caseworker/Child Contact; Sibling Visit; Caseworker/Discharge Resource Contact-Non Birth Parent; Foster Home Visit; Caseworker/Foster Parent Contact; School Contact; Theme of the Month; Service Plan Review; Court Appearance; Case Conference (Internal or with ACS), • The supervisor must review the progress notes, to determine clarity, no later than 5 days after it was received from the worker. The caseworker must make the necessary corrections and return the corrected progress note to the supervisor for further review. The corrections must be submitted to the Supervisor, no later than 3 days after the supervisor requested the corrections.
