Job Description Position Purpose: The Graveyard Support Staff position is located at a Residential Treatment Program for women and their children in South Los Angeles. This position must have a Flexible Work Schedule as needed for 24-hour residential program. The Support Staff is integral in ensuring clients follow program schedule and rules. This position upholds the JWCH mission to improve the health status and wellbeing of under-served segments of the population of Los Angeles County. Job Description Position Description: The Street Medicine HIV Case Manager is a key member of the Street Medicine multidisciplinary team responsible for delivering intensive, field-based case management, counseling, and care coordination services to individuals living with HIV who are experiencing homelessness or housing instability. This role supports Medical Care Coordination (MCC) and Ending the HIV Epidemic (EHE) program goals by improving linkage to care, retention in care, and viral suppression outcomes. The Case Manager / Counseling Navigator assumes primary responsibility for patient tracking, follow-up, care coordination, and interdisciplinary communication to ensure continuity of care across highly transient populations with complex medical, behavioral health, and substance use needs. Job Description In the role of a Physician, you will provide concierge emergency medical care to Sollis Health patients in our clinics. This is a pivotal role as part of a growing organization providing seamless care to both adult and pediatric patients for cases that would usually be treated in a traditional emergency room. Part Time, 7am-3pm and 3pm-11pm - Beverly Hills - 16 hours/week Responsibilities • Performing medical evaluations and formulating a differential diagnosis and treatment plan for urgent and emergent care within the scope of emergency medicine., • Performing medical procedures that include but are not limited to obtaining IV access, phlebotomy, administering IV medications and IV fluids, and performing EKGs., • Leading the medical management of the patients in our clinics by working closely with a team of PAs, nurses, and technicians to discuss your assessment., • Assessing and treating Covid-related cases which includes obtaining clinical samples to test for the virus, performing point of care testing, obtaining blood for antibody blood tests, providing general symptom management and recommendations., • Documenting your clinical evaluation and treatment in the electronic health record, Athena Health., • Working closely with the Medical Director and other clinical team members to ensure all policies are adhered to and that you deliver quality care in an innovative setting., • Coordinating additional care with our After Care, and house calls teams or with one of our Medical Partners., • Making patients and family members comfortable, patiently and professionally answering any questions they have and using patient-centered empathetic communication at all times., • Contribute to team effort by accomplishing related operational projects as needed. Job Description Location: Skid Row, Downtown, Los Angeles CA Position Purpose: The Nurse Manager is responsible for the overall clinical and programmatic oversight of two nurse-led community respite programs operating within multi-service Harm Reduction Health Hubs located in Skid Row, Los Angeles. These programs provide low-barrier, compassionate, and evidence-based care to individuals experiencing homelessness and people dealing with substance abuse. This Nurse Manager is responsible for overseeing the day-to-day clinical operations of the community respite programs and ensuring the delivery of high-quality, harm reduction–focused care. This position provides direct clinical supervision to RNs, LVNs, EMTs, and CNAs, and is responsible for staff onboarding, training, scheduling, and performance management. Additional duties include medication and clinical supply inventory management, coordination of staffing coverage, and oversight of quality improvement and quality assurance (QI/QA) activities to ensure compliance with organizational policies, regulatory requirements, and evidence-based standards of care. This position requires a strong commitment to providing patient-centered, non-judgmental care to individuals from marginalized communities, including people experiencing homelessness, individuals with substance use disorders, individuals engaged in sex work, and those with current or prior involvement in the carceral system. About the Crocker Care Campus & Harm Reduction Model: The Harm Reduction Health Hubs located in Skid Row operate through a collaborative partnership among three agencies and provide integrated, low-barrier health and supportive services. The Hubs offer a range of resources, including outdoor rest and programming spaces, shower and laundry facilities, drop-in services, harm reduction and overdose prevention counseling, safe supply distribution, an opioid treatment program (methadone), a low-barrier medical clinic, and community respite services. The community respite programs provide short-term, non-congregate shelter with on-site clinical support for individuals experiencing acute physical or behavioral health needs that can be safely managed in a community-based setting. Services support individuals recovering from acute intoxication or overdose, initiating medication-assisted treatment, seeking post-assault care, and managing acute illnesses or injuries that do not require emergency department or inpatient hospital care. Principal Responsibilities: • Provide compassionate, low-barrier nursing care in alignment with harm reduction principles., • Provide clinical supervision, mentorship, and support to nursing staff
